RE: [SDP : 2009 - 00031: - Collins Medical Center. Preliminary Site Plan]
Dear Mr. Burkholder:
The Department of Planning and Community Development hereby grants administrative approval to the
above - referenced preliminary site plan.
The approval of the preliminary site plan is valid for (1) one year in accordance with Zoning Ordinance
Section 32.4.3.1. Therefore, the preliminary approval shall expire July.23, 2010. If the preliminary site plan
approval expires, -a new application must be filed and processed.
Please address all of the requirements and conditions listed below and submit eight (8) tentative plan copies to
the Department of Planning and Community Development. This letter must be submitted with the tentative
plans, as a checklist, to document that you have addressed all requirements or conditions, or the tentative plan
will be denied. A submission and review schedule is attached for your information.
The Erosion and Sediment Control Plan and the BMP Stormwater Management Plan, with the associated
applications and fees, may also be submitted with the eight (8) tentative Plans. Failure to do so will result in a
separate application to be filed at a later date. (Note —some of these plans .for this project may have been
submitted previously).
After the initial tentative plan submittal, each department/agency will contact .you to discuss any remaining
conditions. After all aforementioned department /agencies have granted a tentative approval, you may submit
the final mylars (l sets), two paper copies, fmal site plan application, final site plan fee, and a copy of all
written tentative approvals to the Department of Planning and CommunityDevelopment. Assuming that the
final site plan mylars reflects all tentative approvals, signing of the plans will occur within one week.
The final site plan will be subject to all final site plan requirements (Zoning Ordinance Section 32.6), in
addition to the following. conditions.
The Community Development Department shall not accept submittal of the final site plan for signature until .
tentative approvals for the following conditions have been obtained. The final site plan shall not be signed
until the following conditions are met:
Zoning &Current Development approval to include:
-❑ [32.7.9.4(b)] A conservation plan checklist must be completed and incorporated into the final
site plan with notes to show how any individual trees and groups of trees designated to
remain will be protected during the construction of this project. The location of any tree
protection fencing that coincides with the limits of clearing and other methods of protection
from the checklist must be shown on the plan for clear identification during field inspections.
❑ With the final site plan, please be sure to show the resultant area that was multiplied by a
factor of 1.25 to come up with the shown canopy figure. {see plant schedule on page 1. of 2)
❑ [4.1.2.13.e] Each site plan that depicts a commercial or industrial building of four thousand
(4,000) gross square feet or more shall provide a dumpster pad that does not impede any
required parking or loading spaces, nor any pedestrian or vehicular circulation aisles.
[4.12.19] Minimum Requirements and Improvements for Dumpster Pads
i• a. Materials: Dumpster pads shall be concrete.
�• b. Design. The pad shall extend beyond the front of each dumpster so that the front wheels of
a truck servicing the dumpster will rest on the concrete, but in no case shall the length of a
concrete pad be less than eight (8) feet beyond the front of the dumpster. The site shall be
designed so that stormwater does not run through, and drains away from, areas where
dumpsters are located in order to minimize the potential for contaminating stormwater runoff
due to contact with solid waste.
• c. Screening. Dumpsters shall be screened as required by section 32.7.9 and, where
applicable, section 30.6.
• The dumpster_pad is missing on the site -plan. Clearly delineate a dumpster pad on the site
plan. The dumpster pad must meet the minimum requirements and improvements for
dumpster pads outlined above.
❑ [Sec. 4.17.4] Please provide cut sheets and photometric layouts for any /future proposed
lighting that will emit more than 3000 lumens.

EnLyineerim approval to include:
.❑ The angle of the dumpster pad creates an awkward backup situation. It is recommended that
the dumpster pad be rotated 90 degrees and centered within the.24' aisle width. [18- 32.7.2]
(Rev 1) Dumpsterpad has been.removed.
.❑ Please show site distance lines at the entrance onto Westfield Road, easements may be
required. (Rev.1) Comments have not been addressed per' "VDOT standards in the Design
Standards Manual.
❑ Final Site Plan-Comments The following comments are not required to be addressed at this
time. They are provided for informational purposes only.
• Depending on the depth and size of the proposed detention pipe, an easement may be
'required on TMP 61 W- 02 -OB-5 +..Please see Albemarle County Design Standards Manual
Section 6 for required easement widths.
• The stormwater management plan must meet MS -19 of the Virginia Erosion and Sediment
Control Handbook. Please provide information showing that the pipe and channel
downstream is adequate.

Albemarle County Service Authority approval-to include:
❑ All Albemarle County Service Authority comments must be addressed before final site plan
approval can be granted.

Fire & Rescue approval to include:
Verify adequate fire flow is available. Must comply with the Virginia Statewide Fire
Prevention Code. Approval is subject to field inspection and verification.

'VDOT approval to include:
-❑ Final plans need to show the proposed entrance profile
-❑ Final plan needs to show details for CG -6, CG -11, and CG -12B and detectable warning.
-❑ A Land Use Permit will be required for construction within the public road right of way.

ANYiOTHER DEPARTMENT /DIVISION /AGENCY approval-to include:
❑ Architectural Review Board comments dated June 18"' 2009 must be addressed and a
Certificate of Appropriateness received prior to final site plan approval. Please contact Brent

-❑ Albemarle County Division of Planning: Comment from Judy Wiegand: Staff s earlier
comments about conformity with the Comprehensive Plan and the close proximity of three
separate entrances onto Westfield Road have not been addressed. Staff still recommends that
the building be placed close to the street with the parking in the rear and that some
consolidation of entrances be considered.
