The plan for Martha Jefferson Outpatient Care Center (Proffit) - Major amendment
submitted 24 September 2009 has received Engineering review. The plans may be
approved after the following changes /corrections are adequately addressed.
A. Site plans (SDP200900034)
1. VDOT approval is required. Comments will be forwarded when received.

2. Dumpster pad detail or specification to be a minimum 4" stone base and 6"
concrete of 3000psi at 28 days, or stronger, reinforced with a minimum grid of
wire reinforcing or #4 bars at 12" on center. [specs are policy]
B. Stormwater Management and Mitigation Plan (WP0200900049)
1. A completed copy of the standard stormwater maintenance agreement and fee for
recordation for any stormwater management facilities will be submitted and
approved prior to final approval. [ 17 -304E]
2. Copies of manufacturer's approval of the plan for any manufactured stormwater
management facilities (filterra, stormfilter, sandfilter, etc.) will be submitted prior
to final approval. [Policy]
C. Erosion Control Plan (WP0200900049)
1. The construction entrance does not appear to be an accessible route during the 2nd
phase of construction. Please clarify how the construction traffic will access the
site for the duration of the project.
2. Drainage swale down western edge of Leake Sq. does not appear to be properly
addressed. Please clarify how stormwater flows will be controlled down the edge
of the road and under the new entrance.
3. Sediment trap #2 appears to be deeper than the existing pipe currently in that
area. Please clarify this possible discrepancy.
4. Drainage profiles: 14 -311, 18 -32] drainage profiles for each pipe, structure or
channel must contain:

a. Any channel linings (channel along Leake Sq.)
b. A VDOT designation (MH -1, DI -3B, etc.) for each structure
c. Throat length for each drop inlet
d. Grate type for each grate inlet
e. Material and strength class or gage of each pipe
f. Concrete inlet shaping (IS -1) specified on any structure with a 4' or greater
drop
E &SC Narrative:
a. Existing site conditions description
b. Adjacent areas description
c. Off -site areas description including any off -site areas for borrow, waste or
other disturbance.
d. Soils descriptions
e. Stormwater runoff considerations: this can refer to the stormwater
management plan
E &SC Plan:
a. Dust control symbols (DC)
b. Temporary and permanent seeding symbols (TS, PS)
c. Protection provided for all stages of construction
d. No erosion control measures in the way of construction access or rgrading
(Diversion dikes or silt fence are not placed in the middle of the site, or
through access or grading. Sediment traps are not under or on top of fill.
Construction entrances are not on fill, etc. If the concept for site protection
is not adequate in this regard, further review of the plan may not be
possible)
e. Existing soil boundaries are shown with labels. Areas already disturbed
are indicated, with constructed fill depth or cut noted.
f. A stockpile location
g. A staging and parking area, or other construction related areas
