[32.5.6a] Adjacent parcels information. o Provide the correct adjacent parcels owner information, which is currently partially incorrect. o Changes needed include, but are not limited to: Provide the full name of the owner for parcel 05900- 00- 00- 0800O3 which is: Ford, Linda K Trustee of the Linda K Ford Revocable Trust o The Korean Community Church property information is not entirely legible. Revise to make legible. o Provide the correct tax map and parcel and zoning for adjacent parcels, which is currently partially incorrect. o Changes needed include, but are not limited to: o It appears that the Korean Community Church tax map and parcel is not listed correctly. What is the correct tax map and parcel for this property? The correct tax map and parcel appears to be 05900- 00- 00- 023G0. Please verify and revise. o The zoning for the Korean Community Church is not CO. Revise as HC. 8. [32.5.6a] Minimum setback lines, yard, and building separation requirements. o Provide the above information. 9. [32.5.6b] Proposed uses and maximum acreage occupied by each use o Provide a table /schedule of proposed uses on the site. o Include acreages for each proposed use. 10. [32.5.6b] Maximum number of employees. o Provide a count of the maximum number of employees. 11. [32.5.6b] Maximum height of all structures. o Provide the maximum height of the Kirtley warehouse. 12. State that the property lies in the South Fork Rivanna Reservoir Watershed. 15. [32.5.6i] Existing and proposed streets. Provide ROW widths, centerline radii, and pavement widths. 16. [32.5.6b] Maximum amount of paved parking and vehicular circulation areas. Provide a sum figure for maximum paved parking and vehicular circulation areas. 17. [32.5.6032.7.9.4.04.3] Existing landscape features. For the treeline adjacent to the parking lot directly in front of the Kirtley Warehouse, provide details on existing landscape features. 18. [32.5.6p]/[32.7.9] Landscape Plan. Provide a landscape plan for review and approval. Sheet C2.0 has a note "Adjust treeline as needed for construction." Remove this note. Revise as recommended by the ARB reviewer. On the landscape plan, include information regarding which trees will be preserved, which trees will be removed, and which trees will replace the trees to be removed. 19. [32.7.8.2/4.17] Lighting Plan. If any lighting changes are proposed, provide a lighting plan for review and approval to include cut sheets for light fixtures in order to verify that they are full cut -off, and a photometric plan. 20. [32.7.9.4b] Tree Conservation checklist. Provide a copy of the tree conservation plan checklist, signed by the owner, on a current or additional sheet. 21. [32.5.6s] The agent ... may require additional information ... as deemed necessary ... 1. Revise all overlapping and illegible text. o Ex: "Remove ex. 12' CMP" is directly on top of the note beneath it. o Ex: The text by edge of the treeline on Sheet C2.0 is also illegible. 2. Easements are shown on the adjacent Korean Community Church property that do not appear to have been recorded. If the construction easement is existing, provide the deed book and page number. If the construction easement is proposed: submit an easement plat that includes the variable width construction easement and any other necessary easements for review and approval prior to approval of the minor site plan amendment. Submit an accompanying deed of easement for review and approval by the County Attorney, which has to be recorded. 3. Provide the deed book and page number for the existing 30' access easement. 4. The proposed parking spaces and retaining wall are partially on the Korean Community Church property. Permission must be granted by the Korean Community Church property owners. Obtain the necessary easements for this area and include this information on the easement plat and deeds of easement required above. 5. Show existing conditions as they are existing. Field review indicates the following: o The 3 narrow parking spaces immediately along the Kirtley Warehouse, closest to the longest building segment on the front side, (shown on the plan close to the "Remove ex. gravel" note) are currently 2 ADA spaces with a striped space in between. o 14 parking spaces are shown along the treeline. Only 12 spaces are currently along the treeline. 6. Please label which portions of the warehouse at 640, 642, and 644 Kirtley Ln. will be used for the Ivy Fire Station and which remain in UVA Health Services use. 7. Site plan amendments made to the Kirtley property that affect the Korean Community Church property must be reflected on the Korean Community Church site plan amendment currently under review and vice versa. The overlapping areas do not match. 8. Please revise the typo in the company name of the surveyor for source of topography /survey. 9. All site data that is currently listed as N/A is required, is not N /A. Add the correct site data information as requested above. 10. Provide the entire project address. 11. Not enough copies of the site plan were provided. Health Dept. review is needed. Please adequately address the following comments for final approval: A) Site Development plans (SDP201200015) 1) Perpendicular Parking spaces along Kirtley road cannot exceed 5% slope. Please remove these parking spaces. [18- 4.12.15.c] 2) Please raise or slope dumpster pad so stormwater does not flow through or back up into dumpster area. 3) Site plan appears to conflict with the Korean Church site plans SDP201200019. Please verify site development can occur as proposed for both sites. 4) Please submit copies of permanent and temporary offsite grading and erosion control agreements /easements. B) Stormwater Management Plan (WP0201200030) 1) Stormwater quality analysis must use formulae on page 8 of 18 of the County Design Manual. Calculations indicate 0.37 lbs. of removal are required not 0.10 lbs as stated in submittal. 2) If infiltration's desired, then a soil test for perk rates will be required per 3.10 VSMH. Bottom of detention will have to be shown on the plans to be level prior to filling with stone. C) Erosion Control Plan (WP0201200030) 1) Please update the E &SC notes to match the County Design Standards Manual requirements. 2) Please show the temporary paved construction entrance detail from the County Design Manual. 3) Inlet protection for DI -1 will need to be one that works in pavement. Std. & Spec. 3.07 block and gravel inlet protection or acceptable alternative. Once these comments have been addressed, please submit 3 copies of the revised plans, calculations, and narratives to Current Development Engineering along with the required review fee and transmittal form.
