This letter is to approve the letter of revision as shown and described in the revised plan
and letter of explanation dated September 13, 2012. This approval is viewed as your
second Letter of Revision for the above - mentioned final site plan, entitled SDP2010 -029
Martha Jefferson Outpatient Care Center Final Site Plan. Please note that a total of three (3)
Letters of Revision are allowed before all changes-to the site plan are required to be
submitted in the form of a new site plan amendment.
