Preliminary comments for the following divisions of the Department of Community Development and other agencies, as applicable, are below: Albemarle County Division of Planning Services (Planner) — 1. Staff has requested that the applicant conduct a community meeting. 2. A balloon test is required to ascertain visual impact. 3. A critical slopes waiver may be necessary. If a waiver is needed, it must be approved by the Board of Supervisors as a special exception. See engineering comments. Albemarle County Engineering Division (Engineer) — l. Attached. Albemarle County Division of Planning (E911) — 1. Approved. Albemarle County Division Architectural Review Board — 1. A balloon test is needed to determine potential impacts on the Entrance Corridor and historic resources. Albemarle County Building Inspections - 1. No objection. Albemarle County Department of Fire Rescue - 1. No objection. Albemarle County Service Authority - 1. Approved. Virginia Department of Transportation - l. No objection. See attached comments. Comments reflect information available at the time the development proposal was reviewed, and should not be considered final. However, the Site Review Committee has attempted to identify all issues that could affect approval of the proposed project. Page 2 September 18, 2013 Please make the revisions that have been identified as necessary for preliminary approval by the Site Review Committee. If you choose not to make the requested revisions, please submit in writing justification for not incorporating such revisions. Submit (4) four [11" x 17 "] copies to the Department of Community Development including responses to each of the attached comments of the Site Review Committee. Failure to submit this information by this date will result in suspension of the review schedule. Review will resume when revisions are submitted along with a reinstatement fee of $65. The comments are those that have been identified at this time. Additional comments may be added or eliminated based on further review. 1. It appears that the proposed entrance is disturbing critical slopes. Please provide a critical slopes waiver or change the road layout to avoid critical slopes. See the attached image for my recommendation. 2. The access road is too steep. I recommend the gravel access road to not exceed a 12% grade. 3. It is also difficult to see the contours at entrance. Please show proposed grades clearly and label contour elevations on access road at 5' intervals. 4. Verify if ditch near entrance is actually there for channel adequacy requirements. From GIS, the ditch is on the other side of Morgantown Road. 5. The submitted sheets do not appear to be printed to scale. Please print to scale. 6. The land disturbance is over 10,000 sf. Please submit an E &S application and address the following erosion control measures: a) It appears the slopes around tower /site are 2:1 slopes. Please provide soil stabilization blankets & matting per VESCH 3.36 for all areas with 2:1 slopes and show detail on plan. b) Please show the general construction notes for erosion and sediment control plans. c) The construction entrance should be 12'x 70'. Also, provide the Albemarle County paved construction entrance detail on plan. d) Extend silt fence to areas shown on attached image. e) The access road, north of site, drains towards site to a flat area. Address the outlet for this runoff. Looney — Personal Wireless Service Facility Dear Ms. Baldwin: We have reviewed the subject facility in the field and reviewed the plans identified as AT &T site number CV326A as submitted by BC Architects Engineers and dated 8/7113 and offer the following comments: 1. An entrance permit needs to be obtained from this office for the proposed entrance. Mr. Dennis Seale in our office should be contacted at (434) 589 -1053 so that he can verify the adequacy of the entrance location shown in the plans and work with the applicant in obtaining this permit. 2. It is likely that the slope along Route 738 in the vicinity of the proposed entrance will need to be graded away from the road somewhat so that adequate sight distance can be achieved. 3. It is likely that some of the vegetation along Route 738 in the vicinity of the proposed entrance will need to be removed so that adequate sight distance can be achieved.
