Additional comments or conditions may be added or eliminated based on further review.): [Each comment is preceded by the applicable reference, which is to the Subdivision /Zoning Ordinances unless otherwise specified.] 1. [14- 302(A)l] Name of subdivision. Please name this phase " Creekside III Phase IIIA" to correspond with the correct road plans, WPO application, and previous phase naming convention. Rev1: Comment addressed. 2. [14- 302(A)4] Private easements. One sidewalk easement label points to an area where no sidewalk easement is shown (at C3, C4, C5). Please add an easement in this area or remove the label. Additionally, the road plans show that many of the required street trees (approved for bonus density) are proposed outside of the 10' sidewalk (and landscape) easement; please extend this easement to include all street tree locations. Rev1: Comment not fully addressed. The road plans show a 12.5' sidewalk and landscape easement in the same location as the proposed 15' sidewalk easement on this plat; the plat and plans must match. Rev2: Comment addressed. 3. [14- 302(A)5] Public easements. No sight distance easement appears to be present on the plat even though this type of easement is listed under "Easement Notes ". Please clarify if a sight distance easement should be shown. Revl : Comment addressed. However, VDOT will determine if the easement is required. 4. [14- 302(A)14] Land to be dedicated in fee or reserved. This plat contains area that is intended as open space that must be dedicated to the County, as approved for bonus density with the Preliminary Plat. Please label the area as "Open Space Hereby Dedicated to Albemarle County" and provide the acreage. Additionally, provide a Deed of Dedication for review and approval by the County Attorney's Office. This deed must be recorded with the plat. Rev1: Comment not fully addressed. The Deed of Dedication has not been submitted. Additionally, the incorrect acreage for this phase has been used in the calculation to determine the residue acreage; correct the phase and residue acreage on both Sheets 2 & 3. Rev2: Comment not fully addressed. The Deed of Dedication was submitted on December 3, 2013 and forwarded the same day for review by the County Attorney's Office. Comments will be provided upon receipt. 5. [14- 303(C)] Boundary lines. C13 spans the frontage of both lots 46 & 47; please provide separate curve data for these lots to show the correct boundaries and verify the frontage for each. Rev1: Comment addressed. 6. [14- 303(G)] Ownership of common areas. Please label the open space as indicated in comment #4. Rev1: Comment addressed. 7. [14- 303(L)] Public utility, drainage and sight distance easements. Please provide a Deed of Easement for all drainage easements for review and approval by the County Attorney's Office. This deed must be recorded with the plat. Rev1: Comment not addressed. Rev2: Comment not fully addressed. The Deed of Easement was submitted on December 3, 2013 and forwarded the same day for review by the County Attorney's Office. Comments will be provided upon receipt. 8. [14- 303(S)] Control points. At least four control points are required, but it appears only two are provided on the plat. Please add two additional control points. Rev1: Comment addressed. 9. [14 -311] Infrastructure improvement plans, computations and documents. The Phase III Road Plans and WPO application must be approved and bonded prior to approval of this plat. Rev1: Comment still valid. Rev2: Road Plans and WPO application have been deemed approvable, but bonds must be posted. 10. [14 -312] Location of existing buildings. GIS shows an existing structure in the area above lots 46 & 47; please show this structure on the plat. Rev1: Comment addressed. 11. [14 -317] Instrument evidencing maintenance. A maintenance agreement for the proposed landscape and sidewalk easements shall be submitted for review and approval; as with previous phases, a supplemental declaration should be sufficient. This supplemental declaration must be recorded with the plat. Rev1: Comment not addressed. Rev2: Comment not fully addressed. The Supplemental Declaration was submitted on December 3, 2013 and forwarded the same day for review by the County Attorney's Office. Comments will be provided upon receipt. 12. [Comment] There is a stray "L17" label on lot 37 from a previous phase; please remove. Rev1: Comment addressed. 13. [Comment] This plat cannot be approved until VDOT and ACSA complete their reviews and grant their approval. Inspections, E911 and Fire /rescue have reviewed the plat and have no objections. Engineering comments have been provided. Rev1: Comment not fully addressed. This plat cannot be approved until VDOT completes their review and grants their approval; do not resubmit this plat until VDOT comments have been received. ACSA, Inspections, E911 and Fire /rescue have reviewed the plat and have no objections. Engineering comments have been provided. Rev2: Comment addressed. 14. [Comment] This plat cannot be approved until all bonds for this phase have been posted. Additionally, the Deed of Dedication and Deed of Drainage Easement must each be reviewed by the County Attorney's Office. Once the plat is approvable, the Deeds must be modified with the latest revision date and resubmitted for County Executive signature. The supplemental declaration must also be reviewed by the County Attorney's Office, but no County signature is required. The approved plat cannot be released until all associated documents are approved and signed. Rev1: Comment still valid. Rev2: Comment still valid.
