Dear Cheryl:
The Site Review Committee has reviewed the development proposal referenced above. Preliminary
comments for the following divisions of the Department of Community Development and other agencies,
as applicable, are below:
Albemarle County Division of Planning Services (Planner) —
1. [5.1.40(b)(2)]. Setbacks. Notwithstanding section 4.10.3.1(b), the agent may authorize a facility
to be located closer in distance than the height of the tower or other mounting structure to any lot
line if the applicant obtains an easement or other recordable document showing agreement
between the lot owners, acceptable to the county attorney addressing development on the part of
the abutting parcel sharing the common lot line that is within the facility's fall zone (e.g., the
setback of an eighty (80) foot -tall facility could, be reduced to thirty (30) feet if an easement is
established prohibiting development on the abutting lot within a fifty (50) footfall zone). If the
right -of -way for a public street is within the fall zone, the Virginia Department of Transportation
shall be included in the staff review, in lieu of recording an easement or other document.
Sheet A -0 Site Plan shows the fall zone for the proposed 91.8' tall monopole extending across
Interstate 64 eastbound right -of -way. Written permission from VDOT, allowing the fall zone in
the right -of -way, is required. .
2. [Section 5.1.40(c)(3)]. Antennas and Associated Equipment. Please confirm the size of the
Tower Mounting Amplifiers.
3. [Comment]. Advertisement Fees. There is a $200 outstanding advertisement fee which must be
paid.
Albemarle County Engineering Division —
The application plan appears to show over 10,000 square feet of proposed land disturbance
and requires a WPO (Water Protection Ordinance) plan, Application and fee. It is
recommended that the WPO plan must be approved prior to final site plan approval per
chapter 17 of the County of Albemarle Code.

Comments reflect information available at the time the development proposal was reviewed, and should
not be considered final. However, the Site Review Committee has attempted to identify all issues that
could affect approval of the proposed project.
Please make the revisions that have been identified as necessary for preliminary approval by the Site
Review Committee. If you choose not to make the requested revisions, please submit in writing
justification for not incorporating such revisions. Submit (4) four [11" x 17 "] copies to the Department of
Community Development including responses to each of the attached comments of the Site Review
Committee by Monday, February 3, 2014. Failure to submit this information by this date will result in
suspension of the review schedule. Review will resume when revisions are submitted along with a
reinstatement fee of $65.

Dear Mr. Sorrell:
We have reviewed the subject facility in the field and reviewed the plans identified as AT &T site
number CV327B as submitted by BC Architects Engineers dated 9/4;13 and offer the following
comments:
1. The existing entrance shown on the site plan needs to be upgraded to meet the standards
for a PE -1 entrance.
2. A Land Use Permit will need to be obtained from this office to perform the work
necessary to upgrade the existing entrance.
3. VDOT has no objection to the proposed facility as shown provided that a Land Use
Permit is obtained and the entrance is upgraded to PE -1 standards.

PROJECT DETAILS
• Proposed tower location is approximately 30' south of the I64 right-of-way and approximately 175' from the lanes of travel.
• Reference tree is a 24" gum with a height of 88.7', 691.5' AMSL.
• Establish a 2500 square foot lease area, with a 31' x 50' fenced (board -on -board ) compound and a 12' x 28' equipment shelter.
• Install a gravel access drive from Dick Woods Road to the lease area.
• Install a 91.8 -foot tall steel monopole; base at 601.0' AMSL, top at 692.8' AMSL.
• Install I array of 3 flush- mounted panel antennas. Proposed antennas measure approximately 6' tall x 12" wide x 7" deep.
• Pole, antennas and base station equipment to be painted SW 6090 java brown.

A Certificate of Appropriateness from the ARB is required for the base equipment portion of a proposed telecommunications facility in the
