The plan must represent actual site conditions. Please update. 2. The only pertinent details to this amendment appear to be on a stormwater management plan stapled to the back of this site plan. This is not a stormwater management plan review. That requires a WPO application. Please place wall and slope information on the site plan. Please remove the stormwater management plan. Please indicate all changes on the site plan. Bubbled areas appear to be from a previous amendment. 3. All existing wall construction should be certified as to as -built structure and methods by an engineer, including walls to either side of the failure area. Provide a current original certification and evidence of inspections. 4. In the failure area, the block layer laid on the failed slope as a quick -fix is a potential slip plane and should be removed or broken up. Similarly, the existing wall should also be removed so the slope can be tied into the existing fill. Placing fill against existing walls may create erosion problems or a gap form between fill and wall over time. A stone layer between fill areas prevents proper tie -in. Toe drains and graded filters for draining the slope should be specified, tested and certified by a geotech. The slope should be benched into the existing soil, or some sort of structural tie used through old and new fill. 5. The guardrail should be included in details with embedment and position dimensioned. 6. The problems with the biofilter not draining properly must be resolved, as this may be contributing to structural failure. It is possible that the wrong filter fabric was used, or there is a failure in the underdrain system. This must be investigated and certified. Erosion Control Plan (WP0201400029) 1. There do not appear to be any sheets for erosion and sediment control on this plan. It is titled a site plan amendment of ten sheets, with bubbles around items that appear previously built. There is a storm water management plan, sheet 1 of 1, stapled on the back. This appears to have some details. Please provide an erosion and sediment control plan, and omit all the other stuff. It must be a stand- alone plan, not part of the site plan. 2. An updated permit from the Army Corps will be needed for any additional stream disturbance. Please provide a copy with your submittal.
