A. Stormwater Management Plan - requested
1. Fee /Application for SWM-
Please contact reviewer to discuss WPO (stormwater management review) prior to submitting fee,
plans, or supporting documentation. Pre - application discussion may limit need or extent of
possible revisions. Certain changes will be required under SWM review, and revision prior to
initial SWM application submission could save time. There is also a pending date, July 1St, when
the regulations will change, and this could affect this project.
B. Erosion Control Plan (WP0201400040)
1. Erosion and Sediment Control Plan Narrative, a 12 -p supporting document with Appendix should
be numbered. Transfer Erosion and Sediment Control VESCH details, instructions, or measures
to the plans. Unless ESC information is included on plan sheets, there is little chance it will be
noticed or enacted during construction.
2. Remove or re -title sheet C -4, titled Enlarged Site Plan. Site Plans cannot be approved as part of
WPO Application (Also, sect. C., item 1, below).
E &S Legend, Sheet C -4A, includes culvert inlet protection (IP) and outlet protection (OP). Please
show IP and OP for new 15" entrance pipe located at intersection of access drive and Lindsay
Road (sheet C -413, or C -41)).
4. Sheet C -4G, Note 8: "Perform site clearing to the limits shown on the plans." Limits of
disturbance are not shown; please show LOD on plans.
5. Identify off -site disposal areas. Identify construction staging area. Identify stockpile locations.
6. Show tree protection (TP) for 14" OAK, sheet C -4C, if tree is to be saved (tree shown near Alan
B. Howard deed book information).

7. Page 11 of the ESC Plan Narrative states that "Topsoil will be used to help provide a suitable
growth medium for vegetation." Please identify topsoil locations, with specific details (spec 3.30,
TO). Page 11 also states "Sediment shall be removed and the impoundment restored to its original
dimensions when sediment has accumulated to one -half the design depth." This statement appears
to relate to a sediment trap or basin not shown on plans. Please clarify.
8. Page 8 of the ESC Plan Narrative states that "During construction, the contractor will divert
runoff around disturbed areas to reduce the amount of stormwater which could come into contact
with exposed soils. One method of creating a diversion is by installing temporary dike /swales
within the area of work." Please identify diversion dikes in plan view that correspond with stated
plan to divert runoff during construction. Diverted runoff must release to a sediment trap or basin.
9. Plan views show an 11' -12' wide driveway. Narrative describes a 20 -ft driveway. Please clarify
(show access drive 20 -ft wide, or revise narrative).
10. Remove sheets C -5, A -1, but include typical access road cross section elsewhere.
C. Site Plan (SDP201400021)
1. Separate from WPO. Site plan may not be approved as part of a WPO.
2. ESC and a SWM Plan must be approved prior to final site plan approval (ESC Plan Review:
requested changes, above).
3. Entrance, sheet C -4G: Replace Private Entrance Detail with VDOT Private and Low - Volume

Brent,
I received your voice mail and this email but I'm in a meeting and not able to answer your call right now. I have
no problem with referring our application until the WPO review has been completed and is approved. Because I
see this as being no different than past projects when we have known that we cannot pull a building permit prior
to having approval of the E &SC plans. So my end this is nothing that out of the ordinary. It would also be my
understanding that we had already acknowledged that the shot clock is no longer relevant to this particular
application as we voluntarily deferred action on it by resubmitting the revised plans late and paying the
reinstatement fees after taking an additional week to get the full WPO in order for submittal.
Please let me know if you still need to speak further on this issue and I can return your call when I am free.

me an email requesting an indefinite deferral of SDP201400021, Verizon Cash Corner, Howard Property — to
allow needed time to complete the review and approval of the associated WP0201400040.

the following divisions of the Department of Community Development and other agencies, as applicable, are

Comments reflect information available at the time the development proposal was reviewed, and should not be
considered final. However, the Site Review Committee has attempted to identify all issues that could affect approval
of the proposed project.
Please make the revisions that have been identified as necessary for preliminary approval by the Site Review
Committee. If you choose not to make the requested revisions, please submit in writing justification for not
incorporating such revisions. Submit (4) four [I I" x 17 "] copies to the Department of Community Development
including responses to each of the attached comments of the Site Review Committee by Monday, April 21, 2014.
Failure to submit this information by this date will result in suspension of the review schedule. Review will resume
when revisions are submitted along with a reinstatement fee of $65.

The Planner for the Planning Division of the Albemarle County Department of Community
Development will recommend approval for the plan referred to above when the following items
have been satisfactorily addressed. The following comments are those that have been identified
at this time. Additional comments or conditions may be added or eliminated based on further
review. Comments are preceded by the applicable reference to the Albemarle County Code.
[5.1.4(c)(4)] Prior to issuance of a building permit, the applicant shall submit a tree
conservation plan prepared by a certified arborist. The plan shall be submitted to the
agent for review and approval to assure that all applicable requirements have been
satisfied. The plan shall speck tree protection methods and procedures, and identify all
existing trees to be removed on the parcel for the installation, operation and maintenance
of the facility.
[5.1.4(c) (5)] The installation, operation and maintenance of the facility shall be
conducted in accordance with the tree conservation plan. Dead and dying trees identified
by the arborist's report may be removed if so noted on the tree conservation plan. If tree
removal is later requested that was not approved by the agent when the tree conservation
plan was approved, the applicant shall submit an amended plan. The agent may approve
the amended plan if the proposed tree removal will not adversely affect the visibility of
the facility from any location off of the parcel. The agent may impose reasonable
conditions to assure that the purposes of this paragraph are achieved
Sheet C -4B Enlarged Grading Plan shows proposed grading and the installation of
fencing within the drip line of an existing tree designated to remain. A tree conservation
plan prepared by a certified arborist verifying that the tree could survive this impact is
required for a building permit to be issued.
2. [5.1.40 (f] Trees. The height, caliper and species of all trees where the dripline is located
within fifty (50) feet of the facility that are relied upon to establish the proposed height or
screening, or both, of the monopole or tower. All trees that will be adversely impacted or
removed during installation or maintenance of the facility shall be noted, regardless of
their distances to the facility.
On Sheet C -2 Tree Inventory, the existing tree directly east of the facility does not have a
tree inventory number. Revise the drawing to indicate the tree inventory number.
3. [5.1.4(c)(7)] The owner of the facility shall submit a report to the agent by no earlier than
May or no later than July I of each year. The report shall identify each user of the
existing structure, and include a drawing, photograph or other illustration identifying
which equipment is owned and/or operated by each personal wireless service provider.
Multiple users on a single tower or other mounting structure may submit a single report,
provided that the report includes a statement signed by a representative from each user
acquiescing in the report.
This is not a condition of approval for the proposal under review, just a reminder.
4. [5.1.40 (4) (e]) Topography. Except where the facility would be attached to an existing
structure or an existing building, the topography within two thousand (2,000) feet of the
proposed facility, in contour intervals not to exceed ten (10) feet for all lands within
Albemarle County and, in contour intervals shown on United States Geological Survey
topographic survey maps or the best topographic data available, for lands not within
Albemarle County.
Sheet C -lA Area Map shows the parcels within a 2,000' radius of the site; however, the
topo within that 2,000 radius is not shown. Revise the drawing to include the topo, in
contour intervals not exceeding ten (10) feet.
S. [5.1.40 (d)(7]) Color of monopole, antennas and equipment. Each monopole shall be a
dark brown natural or painted wood color that blends into the surrounding trees. The
antennas, supporting brackets, and all other equipment attached to the monopole shall be
a color that closely matches that of the monopole. The ground equipment, the ground
equipment shelter, and the concrete pad shall also be a color that closely matches that of
the monopole, provided that the ground equipment and the concrete pad need not closely
match the color of the monopole if they are enclosed within a ground equipment shelter
or within or behind an approved structure, faVade or fencing that. (i) is a color that
closely matches that of the monopole; (ii) is consistent with the character of the area;
and (iii) makes the ground equipment, ground equipment shelter, and the concrete pad
invisible at any time of year from any other parcel or a public or private street.
On Sheet C -5 Civil Details, add a note to the sheet indicating that the fence will be
painted Java Brown to match the monopole.
6. Miscellaneous:
a. On Sheet G -1 Cover Sheet, the zoning is mislabeled as A -1 (should be RA), the
parcel is mislabeled as TMP 50 -42A (should be TMP 50 -41A) and the acreage is
listed as 96 (should be 61.86 acres). Revise the drawing to correct.
b. Sheet C -5 Civil Details shows a chain link fence detail but chain link fencing is not
proposed on the site plan. Remove the chain link fence detail if it is not to be used.

7. Page 11 of the ESC Plan Narrative states that "Topsoil will be used to help provide a suitable
growth medium for vegetation." Please identify topsoil locations, with specific details (spec 3.30,
TO). Page 11 also states "Sediment shall be removed and the impoundment restored to its original
dimensions when sediment has accumulated to one -half the design depth." This statement appears
to relate to a sediment trap or basin not shown on plans. Please clarify.
8. Page 8 of the ESC Plan Narrative states that "During construction, the contractor will divert
runoff around disturbed areas to reduce the amount of stormwater which could come into contact
with exposed soils. One method of creating a diversion is by installing temporary dike /swales
within the area of work." Please identify diversion dikes in plan view that correspond with stated
plan to divert runoff during construction. Diverted runoff must release to a sediment trap or basin.
9. Plan views show an 11' -12' wide driveway. Narrative describes a 20 -ft driveway. Please clarify
(show access drive 20 -ft wide, or revise narrative).

The Planner for the Planning Division of the Albemarle County Department of Community
Development will recommend approval for the plan referred to above when the following items
have been satisfactorily addressed. The following comments are those that have been identified
at this time. Additional comments or conditions may be added or eliminated based on further
review. Comments are preceded by the applicable reference to the Albemarle County Code.
1. [5.1.4(c)(4)] Prior to issuance of a building permit, the applicant shall submit a tree
conservation plan prepared by a certified arborist. The plan shall be submitted to the
agent for review and approval to assure that all applicable requirements have been
satisfied. The plan shall speck tree protection methods and procedures, and identify all
existing trees to be removed on the parcel for the installation, operation and maintenance
of the facility.
[5.1.4(c) (5)] The installation, operation and maintenance of the facility shall be
conducted in accordance with the tree conservation plan. Dead and dying trees identified
by the arborist's report may be removed if so noted on the tree conservation plan. If tree
removal is later requested that was not approved by the agent when the tree conservation
plan was approved, the applicant shall submit an amended plan. The agent may approve
the amended plan if the proposed tree removal will not adversely affect the visibility of
the facility from any location off of the parcel. The agent may impose reasonable
conditions to assure that the purposes of this paragraph are achieved
Sheet C-4B Enlarged Grading Plan shows proposed grading and the installation of
fencing within the drip line of an existing tree designated to remain. A tree conservation
plan prepared by a certified arborist verifying that the tree could survive this impact is
required for a building permit to be issued.
2. [5.1.40 (f] Trees. The height, caliper and species of all trees where the dripline is located
within fifty (50)feet of the facility that are relied upon to establish the proposed height or
screening, or both, of the monopole or tower. All trees that will be adversely impacted or
removed during installation or maintenance of the facility shall be noted, regardless of
their distances to the facility.
On Sheet C-2 Tree Inventory, the existing tree directly east of the facility does not have a
tree inventory number. Revise the drawing to indicate the tree inventory number.
3. [5.1.4(c)(7)] The owner of the facility shall submit a report to the agent by no earlier than
May or no later than July 1 of each year. The report shall identify each user of the
existing structure, and include a drawing, photograph or other illustration identdying
which equipment is owned and/or operated by each personal wireless service provider.
Multiple users on a single tower or other mounting structure may submit a single report,

provided that the report
includes a statement signed by a representative from each user
acquiescing in the report.
This is not a condition of approval for the proposal under review,just a reminder.
4. [5.1.40 (4) (e]) Topography. Except where the facility would be attached to an existing
structure or an existing building, the topography within two thousand(2,000)feet of the
proposed facility, in contour intervals not to exceed ten (10)feet for all lands within
Albemarle County and, in contour intervals shown on United States Geological Survey
topographic survey maps or the best topographic data available, for lands not within
Albemarle County.
Sheet C-1A Area Map shows the parcels within a 2,000' radius of the site; however, the
topo within that 2,000 radius is not shown. Revise the drawing to include the topo, in
contour intervals not exceeding ten(10)feet.
S. [5.1.40 (d)(7]) Color of monopole, antennas and equipment. Each monopole shall be a
dark brown natural or painted wood color that blends into the surrounding trees. The
antennas, supporting brackets, and all other equipment attached to the monopole shall be
a color that closely matches that of the monopole. The ground equipment, the ground
equipment shelter, and the concrete pad shall also be a color that closely matches that of
the monopole,provided that the ground equipment and the concrete pad need not closely
match the color of the monopole if they are enclosed within a ground equipment shelter
or within or behind an approved structure, façade or fencing that: (i) is a color that
closely matches that of the monopole; (ii) is consistent with the character of the area;
and (iii) makes the ground equipment, ground equipment shelter, and the concrete pad
invisible at any time of year from any other parcel or a public or private street.
On Sheet C-5 Civil Details, add a note to the sheet indicating that the fence will be
painted Java Brown to match the monopole.
6. Miscellaneous:
a. On Sheet G-1 Cover Sheet, the zoning is mislabeled as A-1 (should be RA), the
parcel is mislabeled as TMP 50-42A (should be TMP 50-41A) and the acreage is
listed as 96 (should be 61.86 acres). Revise the drawing to correct.
b. Sheet C-5 Civil Details shows a chain link fence detail but chain link fencing is not
proposed on the site plan. Remove the chain link fence detail if it is not to be used.
