Re: Old Trail Village, Block 3, Phase B Road Bond Inspection Request
Your bond reduction and release requests for Old Trail Village, Block 3, Phase B have been processed.
The current bond amount is $34,700 (for the first 218 feet of Momington Court [Private road] and the
first 235 feet of Momington Lane [private road])
Thank you for the recent submittal of the Stormwater system video with as -built utility plan. The video
appeared to show the pipes were not installed properly resulting in alignment problems creating
depressions for sediment and pockets of water for mosquito breeding areas. (The camera appeared to go
underwater on one section of pipe near structure 3 -4) There were also undetermined deposits that need
to be removed/cleaned from the stormwater system prior to acceptance. The video did not adequately
investigate the pipe connections and further investigation for adequate pipe connections is required.
The road bond amount cannot be reduced at this time. Please complete the required documentation
listed below prior to resubmitting a road bond request.
Per the previous bond inspection/review by Mr. Koslow the following Road Acceptance Check List is as
follows:
1. Tests and Inspections:
a. CBR test:
i. Please provide a location map for the CBR test results from Geo- Terrain
Organization, Inc. report dated 5/9/2012.
ii. Please provide CBR tests for Mornington Court.
b. Stone depth Inspections:
i. Please provide a location map for the pavement corings performed.
c. Fill compaction reports:
i. Not required due to insignificant fill required for construction of road.
d. Pipe and drainage structure video inspection or equivalent:
i. See comments above.
e. Base and surface pavement inspections, documenting materials, thickness and

compaction:
i. Please provide a location map for the pavement corings performed.
f. Concrete tests:
i. No concrete testing is required, but curbing is cracking in several locations and
may need to be repaired.
g. Structural and related inspections for any bridges and foundations:
i. Not required, no bridges or foundations for roads on plans.
2. As -built plans and plats (refer to As -Built Road Plan Policy)
a. Sign and date, and seal each sheet by a professional engineer or surveyor. Original
signature is required, no photo copied signatures.
b. Name and address of preparing firm:
i. Has been provided on as -built utility plan.
c. As -Built Label:
i. Has been provided on as -built utility plan.
d. Location of all proposed vs. Constructed items associated with the road construction must
be shown. The items include, but are not limited to the following:
i. Horizontal alignment:
1. Please provide this for all roads.
ii. Vertical alignment:
1. Profile for Mornington Court is completely missing from submittals,
please provide.
2. Show as -built centerline over the design centerline for Mornington Place.
iii. Edge of pavement:
1. Please display the constructed edge of pavement and width
measurements at every 50' road station.
2. Please show and label constructed edge of pavement radii for all
intersection.
iv. Culvert, pipes, and Drainage Structures:
1. Please state the types of pipes and structures installed.
v. Ditch Lines:
1. Not Applicable for this project review.
vi. Drainage Easements:
1. Please label the apparent drainage easement shown and list the recorded
deed book and page number.
vii. Guardrail:
1. Not Applicable for this project review.
viii. Right -of -way and Associated Easements:
1. Please provide recorded right -of -way easement plat and agreement with
deed book and page number.
2. Please provide recorded variable width utility easement with deed book
and page number.
3. Please provide recorded 20' access easement shown on the approved final
plat.

1. Please update the as -built plans to reflect the cored depths of pavement
and sub -base materials.
x. Street Trees:
1. Please provide the design and as -built locations and species for trees
included on plans with the most recent Letter Of Revision
(SDP201000045)
xi. Signs:
1. Not Applicable for this project review.
3. Bond Inspection:
a. Completed Bond Inspection request form and fee will be required for resubmittal.
b. As -built documents (see item #2 comments above)
c. Signed and sealed letter from a professional engineer listing and certifying the completed
improvements are built according to approved plan will be required for resubmittal.
d. Completed certificate of Completion will be required for release of bond.
4. The following is the additional punch list of deficient construction based on the site inspection
from 26 November 2012 and the latest inspection of 4 June 2014:
a. VDOT turnaround included with LOR (letter of revision) SDP201000045 for the south
end of Mornington Court has not been constructed.
b. Asphalt curb identified as "to be removed" is still in place.
c. 8" nyloplast roof drainage system cleanouts could not be found. Please provide as -built
locations for the roof drain system.
d. Pavement on Mornington Court and Mornington Lane appear to have uneven grade /riding
surface; this is evidence of potential subgrade problems. Please replace /repair or have
certified by geo- technical engineer that it meets or exceeds design requirements.
e. Base coat is exposed at intersection of Mornington Place and Mornington Lane.
f. Existing parking lot and island with mail boxes are not shown on as- built.
