(The following comments are those that have been identified at this time. Additional comments or conditions may be added or eliminated based on further review.) Planning 1. [General Comment] Please have the property owners submit a completed affidavit form "Verifying Family Relationship of Grantor to Grantee" found on page 8 of the Family Division application. 2. [General Comment] Is there a body of water located on this parcel or within 100 feet of a proposed boundary line? If so, please show it on the plat and apply a 100 foot water protection ordinance buffer from the edge. Also, is it possible that part of the WPO buffer from Parcel 39A extends into the entrance to Parcel 41A? If so, please show it on the plat. If either of these comments is applicable, please add a note that states "The stream buffers shown hereon shall be managed in accordance with the Albemarle County Water Protection Ordinance." (I've included two GIS maps below to give some additional context for these comments.) 3. [Section 14- 302(A)(3)] Please show the width of Burnley Station Road. 4. [Section 14- 302(A)(4)/ Section 14 -3171 Is it your intent to keep Eagle Nest Lane as an access easement or would you like for it to be considered a private street? Creating a private street would enable land areas that are not adjacent to SR 641 to have frontage for the purpose of a potential future division. If so, please rename the proposed "new 30' access easement and gravel road" to instead be labeled as a "new 30' private street." Please disregard this comment if you would like to keep Eagle Nest Lane as an access easement. 5. [Section 14- 302(A)(10)] Please add a note regarding development rights. 6. [Section 14- 302(B)(2)] Please add the name of the person who prepared the plat. 7. [Section 14- 302(B)(7)] Please add a note to indicate this property is located within the Blue Run Agricultural - Forestal District. Please also note this property does not lie within a water supply watershed. 8. [Section 14 -309 and 14 -310] Information for the Health Department was transmitted on 7/9/14. 1 will let you know as soon as I receive any information back from them. 9. [Section 14 -316] Please submit evidence of approval of the entrance onto the public street from VDOT. Please contact J.T.
