RE: SP201200005 — Verizon Wireless "1-64 East" Tier III Personal Wireless Service Facility
Dear Mr. Waller:
On June 6, 2012, the Albemarle County Board of Supervisors took action on SP201200005 to allow
installation of a three new flush -mounted antennas on TMP 07300-00-00-031 DO in the Samuel Miller
District. This special use permit was approved based on the following conditions:
Development and use shall be in general accord with what is described in the applicant's request
and site plans, entitled 1-64 East LTE 4G Upgrade", with a final zoning drawing submittal date of
4/13/12 (hereafter "Conceptual Plan"), as determined by the Director of Planning and Zoning
Administrator. To be in accord with the Conceptual Plan, development and use shall reflect the
following major elements within the development essential to the design of the development, as
shown on the Conceptual Plan:
a. Height
Minor modifications to the plan which do not conflict with the elements above may be made to
ensure compliance with the Zoning Ordinance.
Please be advised that although the Albemarle County Board of Supervisors took action on the
project noted above, no uses on the property as approved above may lawfully begin until all
applicable approvals have been received and conditions have been met. This includes:
• compliance with conditions of the SPECIAL USE PERMIT;
• approval of and compliance with a SITE PLAN amendment; and
• approval of a ZONING COMPLIANCE CLEARANCE.
In the event that the use, structure or activity for which this special use permit is issued is not commenced
within twenty-four (24) months from the date of Board approval, it shall be deemed abandoned and the
permit terminated. The term "commenced" means "construction of any structure necessary to the use of
the permit."

Dear Mr. Waller:
The Albemarle County Planning Commission, at its meeting on April 24, 2012, by a vote of 6:0
recommended approval of the above -noted petition to the Board of Supervisors.
Please note that this approval is subject to the following condition:
1. Development and use shall be in general accord with what is described in the applicant's request
and site plans, entitled 1-64 East LTE 4G Upgrade", with a final zoning drawing submittal date of
4/13/12 (hereafter "Conceptual Plan"), as determined by the Director of Planning and Zoning
Administrator. To be in accord with the Conceptual Plan, development and use shall reflect the
following major elements within the development essential to the design of the development, as
shown on the Conceptual Plan:
a. Height
Minor modifications to the plan which do not conflict with the elements above may be made to ensure
compliance with the Zoning Ordinance.
In addition the Planning Commission approved the following modifications, by a vote of 6:0:
• Zoning Ordinance Modifications:
— Section 5.1.40 (a)(4e)- topography.
— Section 5.1.40 (a)(4)(f)- height, caliper and tree species
— Section 5.1.40(a)(4g)- setbacks, parking and landscaping
Please be advised that the Albemarle County Board of Supervisors will review this petition and receive
public comment at their meeting on June 6, 2012.
