The specific deficiencies and measures required to correct those deficiencies are as follows: Deficiency: HDPE "smooth walled" pipe not installed to manufacture's standards. Required Corrections: Install HDPE "smooth walled" pipe in accordance with the manufacture's recommended installation methods. 2. Deficiencv: Outlet Protection STR #I is not installed in accordance with 3.18 of the VESCH O {� Third edition, dated 1992. ti Required Corrections: Clean and Install outlet protection in accordance with 3.18 of the VESCH Third edition, dated 1992. 3. Deficiencv: Outlet structures 92 and #3 are currently holding water and have not been adequately sealed to prevent the infiltration of water and to allow for positive drainage. Required Corrections: Clean and Install Structures in accordance with the Erosion & Sediment Plan dated 10/21/2011 C) ('C'---4. Deficiencv: Sediment traps 2, 3 and 4 Weirs are not installed in accordance with 3.13 of the VESCH Third edition, dated 1992. Required Corrections: Repair Sediment traps in accordance with 3.13 of the VESCH Third edition, dated 1992. Unless subsequently amended in writing by this office, it is expected all of these corrective measures will be completed by no later than December 20, 2012. Box 281 Charlottesville, VA 22902 Note: 17- 207(2) "Permanent vegetation shall be installed on all denuded areas within nine (9) months after the date the land disturbing activity commenced, except for areas that the program authority finds are necessary parts of the construction that are subject to an active building permit and areas where erosion is prevented by a non - erosive surface. For the purposes of this section, a "non- erosive surface" includes, but is not limited to, roadways and sidewalks covered by gravel, asphalt pavement, or concrete; trails or paths covered by gravel, stone dust, or mulch; buildings and other permanent structures; and such other surfaces that the program authority determines would adequately provide a permanent barrier to erosion. " Under Sections 17 -213, 214 and 215 of the Albemarle County Code, failure to make these corrections by the required date can have the following results: • Your permit may be revoked. • You may be required to submit a new Erosion and Sediment Control Plan and have it approved before additional land disturbance will be allowed. Review of a new plan can take up to 45 days for each submission and it is not unusual for several submissions before the plan can be approved. This requirement would include a new application, fee and bond for the work proposed under the plan. • The County may issue a STOP WORK ORDER and seek relief in the Circuit Court. The Circuit Court can fine the property owner up to $2,000 per violation, convict the property owner of a Class I misdemeanor and require the property owner to pay any damages resulting from a failure to comply with this notice. Should you have a legitimate reason you can not complete the corrective measures by the above given date, you must immediately contact me at (434) 296 -5832. Unless you have contacted us and alternative arrangements have been agreed to in writing, we will expect the corrections to be completed by the above date. The specific deficiencies and measures required to correct those deficiencies are as follows: 1. Deficiency: Sediment observed escaping underneath the outlet pipe of ST#4. Required Corrections: Re- install HDPE pipe in accordance with the manufacture's recommended installation methods. 2. Deficiencv: Outlet Protection for ST #4 is not installed in accordance with 3.18 of the VESCH Third edition, dated 1992. Required Corrections: Clean and Install outlet protection in accordance with 3.18 of the VESCH Third edition, dated 1992. 3. Deficiency: CWD and SCC have not been installed in accordance with MS-4. Required Corrections: Install CWD and Stormwater conveyance channels in accordance with the approved Erosion and Sediment control plan dated 2011. 4. Deficiencv: Damaged Silt Fence observed throughout site. Required Corrections: Repair Silt Fence in accordance 3.05 of the VESCH Third edition dated 1992 and Remove Silt Fence in areas that are permanent stabilized. 5. Deficiencv: Construction Entrance has formed rills and gullies due to concentrated runoff. Required Corrections: Repair Construction Entrance in accordance 3.0 of the VESCH Third edition dated 1992 to allow access to Storm Water Conveyance channel.
