MOBILE FOOD VENDOR CHECKLIST
PLEASE CHECK EACH BOX SO THAT IT IS CLEAR THAT YOU HAVE READ AND UNDERSTAND THE
REQUIREMENTS FOR MOBILE FOOD VENDORS:

Health Department Approval. Each mobile food vendor shall provide a copy of a valid Mobile Food Establishment
Permit issued by the Virginia Department of Health. No zoning clearance will be issued without prior approval from the
health department. Permit Expiration Date: 'Q r
Owner's Permission. Operation of a mobile vending unit on private property for any length of time requires permission
from the property owner.
Verification of Site Plan Compliance. Mobile food vendors may operate by right in any commercial zoning district.
Operation at a single location for more than two hours at a time is subject to compliance with a site plan. Mobile food
units must be no closer than 30 feet from any public right -of -way and 50 feet from any residential or Rural Areas zoning

LJ Commissary Facility. State regulations require that food sold from a mobile unit must be prepared and stored either
onboard the unit or in a health department permitted commissary facility. Food may not be prepared or stored in a home
1 kitchen.
IJ Parking. Each vending site shall provide a minimum of two parking spaces.
El Peddler's License. Mobile Food Vendors who operate in Albemarle County are required to obtain a peddler's license.

FOOD CART OPERATIONAL LEASE AGREEMENT

hereinafter called the "Lessees," o ("Lessee,"
WI TNESSETH:
LESSOR, for and in consideration of the rents, covenants, agreements, and
conditions herein contained on the part of the Lessee to be paid, k2pt, observed, and
performed, hereby Leases to Lessee, and Lessee hereby Leases from Lessor, subject
to the terms and conditions of this Lease, the following real property located in the
County of Albemarle, Virginia, all herein called the "Premises ":
That space within Oakhill Market and Deli Parking Lot having a street address of
667 Country Green Road Charlottesville, Virginia, 22902.
1. TERM. The term of this Lease shall be Month to Month commencing

right to increase the monthly rental amount by 3 %, and every year thereafter.

2. RENT AND SECURITY DEPOSIT. Lessee, in consideration of this Lease,

of the term for the first year of the term. All rental payments shall be payable on
the 1St day of each month in advance. (A security deposit of $40, 0 shall be paid
at signing)

Payments should be mailed or submitted in an enclosed and labeled envelope
to: SIDDH LLC, 667 Country Green Road, Charlottesville, Virginia 22902 or to
such other address as may be furnished to Lessee from time to time.
Lessee agrees to pay to Lessor, as a "late charge," Fifty dollars ($50.00) for any
rent payment herein required to be made by Lessee which is received more than
five (5) days late, to cover costs of collecting amounts past due. Further, the
Lessee shall pay to Lessor, an additional "late charge ", Fifty dollars ($50.00),
should the rent not be received by the end of the calendar month in which it is
due.
3. NON - RENEWAL. Lessee shall provide 60 days notice of termination of lease,
Lessor shall provide 60 days for termination of lease.
