County Code section 17 -410 and Virginia Code §62.1- 44.15:34 requires the VSMP
authority to act on any VSMP permit by issuing a project approval or denial. This project is
approved with a condition to address the following:

Once you obtain the coverage letter from DEQ, place a copy with SWPP onsite.
Revise the registration statement to 1.3 acres for total land area and total area to be
disturbed. I have delineated the area below. Please submit revise registration statement to
the County. This will be entered into the DEQ website by County staff. From there, DEQ
will contact applicant.

3) Update Site Inspection Log to current conditions. For example diversion dikes have been
removed behind constructed townhomes.
4) Update Record of Land Disturbance. The site is stabilized.
5) Project is located in the impaired water. Please list Rivanna River.

6) List all qualified inspector personnel. From the logs, Mike Myers and Allen Cross
completed inspections. Please provide a sheet with the list of qualified inspector
personnel.
7) There is no pollution prevention plan within the SWPP. Provide an 11 "x17" so that
contractors can later mark porta johns, concrete washouts, fueling stations, solid waste
disposals, etc.
8) The total fee paid is $2700. I have broken down fees and typical due dates. Since this
application is for compliance of VSMP regulations, $1944 is total fee to the County. We
will refund the $756.

9) Please note the annual permit and inspections fees are up to date and have been paid.
DEQ will review the application information based on local VSMP authority approval. At this
time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest
processing, this is done electronically with the emails provided on the application. DEQ should notify
applicants with instructions on how to pay fees. When DEQ approves the application, they will issue a
permit coverage letter. This should be copied to the county.
