This road is bonded, so technically final plats can be approved. A recent bond inspection and
3s -built plan review has revealed that some reconstruction, or re- platting of the easements may be
seeded. These easements are currently shown with deed book references on this plat. It would be
Dest if re- platted easements were created before lots were sold, as some may be needed on these

I have spoken with Paul Byer regarding the above noted subdivision plat. Since this plat only shows
previously recorded ACSA easements, I am going to approve this plat. The construction plans have
expired and need to be resubmitted. Usually we make the plat match the approved plans, but in this
case we will make the new plans match the plat. If any changes are needed in the future a corrected
plat will be recorded.

Hello Kevin and Paul,
Please consider this e-mail as review comments for the most recent submittal, which was received
on April 3, 2012. The plat for SUB - 2011 -129 may be signed once the remaining maintenance
agreement is approved by the County Attorney's office. The latest version, a miscellaneous submittal
received on April 17, 2012, has been forwarded for re- review and approval. The current maintenance
agreement appears to still have errors contained within it. Additional revisions may be necessary.
Also, for records retention, please supply the most recent drainfield exhibit provided to the Health
Dept. Health Dept. approval is contingent on what was reviewed with the newer drainfield exhibit, so
please provide this documentation to me.
Once the maintenance agreement is approved, please obtain notarized owners' signatures and
submit for signature by the agent or designee. Record both maintenance agreements concurrently
with the plat.

"Drainfield Exhibit" sent by inter - office mail on 04/05/12 to Kelly Donahoe at the Health Dept.
According to the prepared drainfield exhibit I received today. The drainfield on lot 28 -or 178 is less
than 5 feet from the property line. Checking the scale on the soils work that was re- submitted (which
is granted a copy) it appears that it has changed from —8 feet to — 2 feet from the side property line.
Our regulations do require that 5 foot setback for all drainfields. This will need to be corrected to its
original setback prior to our approval.

received the modified drainfield exhibit from Mr. Shreiner. If all drainfield areas retain that 5'
property line setback that is specified on the exhibit it is approved. There are a few areas that it
appears very close and is difficult to tell with our scales. No part of the septic system can be located
within 5' of any property line. If there are any further questions or approval needs do not hesitate to
contact our office.

notarized owner's signatures.
