Chesapeake, Virginia 23323 RE: SDP - 2014 -018 Chick- Fil -A- Initial Site plan ZRINST1381 The Agent for the Board of Supervisors hereby grants administrative approval to the above referenced site plan. This approval shall be valid for a period of five (5) years from the date of this letter, provided that the developer submits a final site plan for all or a portion of the site within one (1) year after the date of this letter as provided in section 32.4.3.1 of Chapter 18 of the Code of the County of Albemarle, and thereafter diligently pursues approval of the final site plan. An Erosion and Sediment Control Permit may be issued after the following approvals are received: 1. Approval an Erosion and Sediment Control Plan meeting the requirements of Chapter 17 of the Code of the County of Albemarle. 2. Approval of a Stormwater Management Plan meeting the requirements of Chapter 17 of the Code. 3. Approval of all easements for facilities for stormwater management and drainage control. 4. Approval of a mitigation plan for the disturbance of Water Protection Ordinance buffers. 5. Submittal of a tree conservation checklist. The final site plan will not be considered to have been officially submitted until the following items are received: A final site plan that satisfies all of the requirements of section 32.6 of Chapter 18 of the Code. A fee of $1,500. Please submit 12 copies of the final plans to the Community Development Department. The assigned Lead Reviewer will then distribute the plans to all reviewing agencies. Once you receive the first set of comments on the final site plan, please work with each reviewer individually to satisfy their requirements. The final site plan will not be approved until the following conditions are met: The Department of Community Development shall not accept submittal of the final site plan for signature until tentative approvals for the following conditions have been obtained: Planning Division Approval of (2 copies): 1. A site plan meeting all the requirements of section 32.6 of Chapter 18 of the Code. 2. A landscape plan meeting the requirements of section 32.7.9 of Chapter 18 of the Code, including a tree conservation checklist. 3. [32.5.2(a)] State on the plans the conditions of the Special Use permit. 4. [32.5.2(b)] State on the plans the proposed maximum height of the building. The regulation is stated, but not the proposed height. 5. [32.5.2(e)] Show the utilities on the landscape plan to ensure that plantings are not occurring overtop of easements or pipes. 6. [32.5.2(e)] Provide street trees along the northeast side of the travelway, in front of the proposed retaining wall. 7. [32.5.2(i)] All proposed easements will need to be submitted as a plat and be recorded prior to approval of the final site plan. 8. [32.5.2(i)] Will the private access /travelway be owned by the property owner? A different owner? A maintenance agreement will need to be submitted with the plat for the easement for the travelway. 9. [32.5.2(n); 4.17] The LLF factor for the lighting needs to be 1.0. 10. [32.5.2(n); 4.17] Revise the lighting plan to have 0.5 footcandles at the property line. 11. [32.5.2(n); 4.17] The cutsheets for the lighting fixture need to be on the plan sheets. Add a sheet that shows the proposed fixture. 12. [32.5.2(n)] Will a free standing sign be on the property? If so, show the location of the sign. A separate sign permit will need to be submitted, however, you will need to show the location of the sign on the site plan. Engineering Division Approval of (2 copies of final; WPO will require more copies as stated on application): 1. An Erosion and Sediment Control Plan meeting the requirements of Chapter 17 of the Code of the County of Albemarle 2. A Stormwater Management Plan meeting the requirements of Chapter 17 of the Code. 3. All easements for facilities for stormwater management and drainage control. 4. A mitigation plan for the disturbance of Water Protection Ordinance buffers. 5. Road plans. 6. See attached comments for conditions of approval for the initial site plan, and for final site plan comments. ACSA approval to include (3 copies): The applicant should submit 3 copies of construction drawings along with water and sewer data sheets to the ACSA (Attn: Jeremy Lynn, PE). The ACSA has determined certain items that will be evaluated during the construction review that the applicant should be aware of: 1. See attached worksheet from ACSA. 2. Relocate the proposed fire hydrant to the eastern corner of the property. The location shown does not properly cover the site and will have reduced access due to the demolition of the existing road. 3. Extend the existing 8" water main that is located to the north of the property along the proposed road to the new proposed fire hydrant location. 4. Relocate the water meters to the proposed water main feeding the proposed hydrant. This will prevent a dead end water main with no demand. 5. Adjust the landscaping plan as required. 6. Advise if the grade will change over the existing ACSA sanitary sewer when a portion of the existing road is removed. 7. Advise if access along the ACSA sanitary sewer will maintain where the road is being demolished. Pantops Drive." That section of road is known as "Luxor Road" currently. Please replace the road name shown with the correct road name. Fire Hydrant location shall be at the entrance to the parking lot. Please work with ACSA for the location. 2. Fire Flow test required before final approval. ARB approval to include (1 copy): The ARB took the following action on the initial plan at the April 7, 2014 meeting: 1. Regarding requirements to satisfy the design guidelines as per § 18- 30.6.4(2), (3) and (5): None. 2. Regarding recommendations on the plan as it relates to the guidelines: None. 3. Regarding recommended conditions of initial plan approval: A Certificate of Appropriateness is required prior to final site plan approval. 4. Regarding conditions to be satisfied prior to issuance of a grading permit: a. Coordinate proposed grading and tree protection fencing to ensure the health of the three maple trees to remain in the right -of -way along the EC. Provide evidence of grading and planting easements. Will there be large truck traffic using the new private road? If so, a 30' radius at the entrance may not be large enough to accommodate such a vehicle. 2. The accessible ramps should be labeled as VDOT Standard CG -12's and the appropriate details from the Road and Bridge Manual should be included. 3. The temporary construction entrance should be located off of N. Pantops Drive instead of directly onto Route 250. 4. The traffic signal design must be approved by the Northwest Regional Operation staff. 5. The turn and through movement striping shown on the private road at the connection with Route 250 differs between the traffic signal design sheets and the site plan sheet on C -2.0. 6. The through lane from the new private road should be aligned with State Farm Blvd. A wider median on the new road may be necessary to achieve this. Site Development Plan (SDP201400018) This is a condition of the initial site plan. 1) Provide a road plan for the proposed N Pantops Drive. Also, it appears the horizontal curve of 200' per VDOT Road Design Manual, Appendix B, Section (B)- 3 Table 1 is not met at the NE corner of the site. There are also sight distance issues at the NE entrance of the Chick -Fil -A site. Please address. 2) Clarify sight distance triangles. It is not clear how measurements are taken when looking east. 3) The entrance into Martha Jefferson Hospital shall maintain, at a minimum, an 18' separation distance from the parking space to the street. Please address. These comments can be addressed at the final site plan phase. 4) The TMP # is incorrect. Please revise. 5) The southern boundary line abutting Rte 250 does not appear to be correct. It should be closer to the concrete sidewalk, according to the Albemarle County GIS. Please verify. 6) Remove E &S control plans. Also, remove the details pertaining to them. These will be reviewed with a WPO application. 7) Provide the date and source of the topographic info. For example, provide the surveyor name and date of survey. All topo should be field verified within the last year. 8) It appears there are critical slopes along the southern boundary. These are considered "managed slopes." Please show. 9) Label the catching curb on C -2. 1 with VDOT CG -6. The spilling curb and gutter should be labeled with an equivalent VDOT detail. 10) Label the VDOT curb type in the parking lot. 11) The minimum radius for a curbed island is 3' in the parking lot. Please revise. 12) Remove one parking space at both entrances to address policy of 18' separated distance between parking and street. 13) Sidewalk in front of bldg and adjacent to parking should be 5' (excluding curb and gutter). Provide concrete blocks for all parking spaces in this area. Engineering Review Comments Page 2 of 3 14) Sidewalk in all other areas shall be a minimum of 5' (excluding curb). The plan is showing 4' and the detail on C -4.0 is showing 6.5'. Please revise. 15) The asphalt entrance on the NW corner is too narrow. I recommend reducing the size of the ad since the min IMF® is 8'. The a_ frv� / r = - -1 travelway can be increased to 24' wide. 16) The NE entrance grade should not exceed 4% for a distance of 40' from the intersected street. 17) The angle at intersections shall not be less than 80 degrees. Please revise. 18) The travel lane pavement striping should be yellow for opposing traffic. Please revise at the area near median. 19) What are arrows on signal? They do not appear to be directional arrows. 20) Easements for all proposed utilities should be shown. 21) The travelway from Rte 250 should be graded to a max of 10 %. Please revise. 22) Clarify finished grading on SE corner of the site near the entrance. There is a riprap area. 23) Tie the proposed 527.5 contour at the entrance to an existing contour. It is currently shown to fill over the FES. 24) Near the entrance, label top of wall elevation where the ground elevation is 526'. 25) Is A2 - A3 necessary since A2 -A4 is already shown. 26) The retaining wall north of site has an incorrect bottom of wall elevation where the TW is 547.50. Please revise. 27) Any walls supporting roads or necessary infrastructure require engineered plans and computations. 28) Please show low maintenance ground cover for areas with greater than 3:1 slopes at retaining wall north of site. 29) Tie the 542.5 contour to an existing contour NW of site. 30) Show a minimum of 25' radius in drive thru. Engineering Review Comments Page 3 of 3 31) Detail 5 on C -4.0 should show 10' wide parking stalls to match plans. 32) Remove all landscaping on stormdrain structures. 33) Filterra does not capture enough for quality. Please provide more to treat onsite impervious areas. 34) Show limits of disturbance and label in acres. 35) What is the sign on the lower SW corner of the property? It is an existing sign. 36) Provide easements for all proposed utilities. 37) Please provide the existing DB and page number for the access easement at the NW corner of N Pantops Drive. 38) Provide existing drainage easements with DB and page # if any. 39) There is work outside the property line: retaining wall, road, sidewalk, landscaping, etc. Provide the temporary construction or grading easements. Also, provide agreements on maintenance. 40) A portion of N Pantops is to be removed. It will be clearer to define limits with hatching instead of notes. Please provide.
