The proposed project is to construct an approximately 7,400 SF auxiliary gym addition to the existing Henley
Middle School. The addition includes a large and a small athletic spaces, storage, toilets, and circulation.

complete without the appropriate checklist, completed, signed, and included with the required submittal
materials indicated on the checklist.
