After reviewing what you are proposing, you can follow the tier 1 assessment. Here is how I made the determination as this is a case by case scenario. 1) The building expansion is approximately 696 sf. With an estimate of 50 people at the church, the design of the drainfield is 50 persons x 5 gallons /person = 250GPD. 2) With that information, I can also use 5 gallons /person /day for toilet & faucet usage. You fall way below the 2000 gallons /day and tier 1 is a reasonable groundwater assessment requirement. I recommend obtaining a permit to drill the well with the Health Dept. Once approved, drill a well and complete a GW -2 form. Submit this form to the health dept and the county. State the coordinates of the well location. Sec. 17 -1001 Tier 1 assessments. A Tier 1 assessment shall consist of the owner drilling a well on the lot and submitting the following information to the program authority: (i) a Virginia well drilling completion report (form GW -2) for each well drilled; and (ii) the latitude and longitude coordinates of each well's location. The information submitted must be accepted as complete and accurate by the administrator prior to issuance of the building permit.
