A. Stormwater Pollution Prevention Plan (SWPPP)
The SWPPP content requirements can be found in County Code section 17 -405. A SWPPP must contain (1)
a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary.
1. Albemarle County must sign/date Registration Statement (ACCD/Engineering will coordinate).
2. Provide SWPPP Certification statement (VAR10, Part III, K.4.):
4. Certification. Any person signing a document under Part III K 1 or 2 shall make the following
certification:
"I certify under penalty of law that I have read and understand this document and that this document
and all attachments were prepared in accordance with a system designed to assure that qualified
personnel properly gathered and evaluated the information submitted. Based on my inquiry of the
person or persons who manage the system, or those persons directly responsible for gathering the
information, the information submitted is, to the best of my knowledge and belief, true, accurate, and
complete. I am aware that there are significant penalties for submitting false information, including the
possibility of fine and imprisonment for knowing violations."
3. SWPPP is comprehensive. However, please update insert sheets to reflect revisions per ESC plan
comments, Sec. C. below.
4. CE -101 or CE -102 —Show initial locations of. sanitary /porta john; concrete wash -out draining to
pit/area with impermeable lining; vehicle wash draining to sediment trap /equivalent; on -site fuel
storage /re- fueling areas with containment (if proposed); non - hazardous solid waste dumpster (if
provided); covered /protected paint - solvent storage (if present). Note: these PPP facility locations
are required to be shown on SWPPP exhibit, not full -size ESC plans. These locations may change
as needed (if revise locations, show via pencil edits to on -site SWPPP exhibit).
5. Future Reference (or additional sidewalk projects: State Farm Blvd; Barracks Rd; Hydraulic Rd), please:
a. Number pages of document for ease of reference.
b. Runoff coefficients (Rational or Modified Rational Method) are not used for compliance.
Use NRCS /TR -55 /curve numbers instead. —ref 9VAC25- 870 -72 (24 -hr storm/NOAA
Atlas 14).

B. VSMP: SWPPP: Stormwater Management Plan (WP0201500021)
VSMP Regulation 9VAC25- 870 -108 requires the VSMP authority to approve or disapprove a SWMP. This
plan is disapproved for reasons provided in comments below. The stormwater management plan content
requirements can be found in County Code section 17 -403.
Drainage and ESC Narrative for: Crozet Avenue North Sidewalk Improvements Project (REPORT),
27- Mar -15, Water Quality Sec. explains: "The pollutant removal requirement of 0.33 lb /yr will be
purchased from a nutrient bank." Two items:
a. Ref. 9VAC25- 870 - 69/62.1- 44.15:35 — Documentation of acquisition of credits required.
(https:Hlegl.state.va.us /c ip bin/legp504.exe ?000 +cod +62.1- 44.15C35).
E- Documentation of the applicant's acquisition of nutrient credits shall be provided to the VSNP authority and the
Department in a certification from the credit provider dornmenting the number of phosphorus nutrient credits
acquired and the associated ratio of nitrogen nutrient credits at the credit- generating entit},. Until the effective date
of regulations establishing application fees in accordance vath § 62.1 - 44.19:20, the credit provider shall pay the
Department a tinter quality enhancement fee equal to six percent of the amount paid by the applicant for the credits -
Such fee shall be deposited into the Virginia Storrnwater Management Fund established by § 62.1 -44.15:29 -
b. Report TOC indicates Appendix D contains Water Quality Calcs. Appendix D lists data.
Provide calculations (VSMH11999 Vol. II, Appendix 5D Worksheets /situation 3).
2. Report/MS -19 Compliance — Provide pre- /post - development Qto values /calculations.
C. VSMP: SWPPP: Erosion Control Plan (WP0201500021)
Virginia Code §62.1- 44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is disapproved
for reasons provided in comments below. The erosion control plan content requirements can be found in County Code
section 17 -402.
1. Please remove the following sheets from ESC/VSM Plans (site, road, traffic): CA -002A, CR -102,
CT -101, CT -102, CT -500, CT -501, CT -502, X01, X02, X03.
2. CA -004: Revise uppermost Crozet Ave detail label to read Sta. 10 +00 to Sta. 15 +65.28.
3. CA -004: Please clarify if symbols 7 and 8 relate to sidewalk. Also, furnish a more descriptive
Sidewalk Detail.
4. CA -004 —If Inset B is for sidewalk, symbols 10 and 11 are undefined (should these read 7 and 8 ?).
For clarity, please provide a more readily readable sidewalk detail; for example:

shall match that of the sidewalk, however, the area wl II not be considered part of the minimum
width required for sidewalk.
It trees are planted in paved pedestrian areas, they shall be planted in tree wells that include
a planting grate, approved by the resident engineer. However. the width of sidewalk passing
the tree well shall be at least 3 feet in width.

14. CG -100 — Future reference: runoff coefficients used with Rational Method are not used with TR-

15. CG -201 —If 18" RCP label is unintentional, remove. Meaning is unclear. —Also, next comment.

30. May have encountered reference to off -site waste area during initial review, but cannot locate
reference. If an off -site waste area proposed, ensure off -site area has an approved ESC plan.
The VSMP permit application and all plans may be resubmitted for rp eview (as .PDFs). When all comments have
been satisfactorily addressed, and if there are no additional comments, please provide 2 copies of the plans, 1 copy
of SWPPP. Encourage .PDF submittals that show revisions.
Engineering plan review staff is available 2 -4 PM on Thursdays, should you require a meeting to discuss this review.
Plan reviewer is available at 434.296 -5832 -0069 should you have any questions.
Process:
After approval, county staff will need to enter project information in a DEQ database for state application
processing. DEQ will review the application information based on local VSMP authority approval. At this time, the
DEQ portion of the application fees will need to be paid directly to the state. For fastest processing, this is done
electronically with the emails provided on the application. DEQ should notify applicants with instructions on how to
pay fees. When DEQ approves the application, they will issue a permit coverage letter. This should be copied to the
county.
After DEQ coverage is issued, via the coverage letter, the County can hold a pre - construction conference. Applicants
will need to request a pre - construction conference by completing a form, and pay the remainder of the application
fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be
checked by county staff, and upon approval, a pre - construction conference will be scheduled with the County
inspector. At the pre - construction conference, should everything proceed satisfactorily, a joint VSMP and grading
permit will be issued by the County so that work may begin.
