As previously discussed I wanted to provide you any changes to the plan that will be needed per the ordinance
in addition to the previously discussed relocation of the tower within proposed lease area, revised north arrow,
revised tree chart to show no trees being taken down except the 3 dead ones, and the slight modification with
the revised location of the access road.
I offer the following comments on the proposed site plan (if these notes are already addressed on the plans, just tell me
what sheet and where I may find them):
1) [5.1.40(a)4(g)] Setbacks. Assure the setbacks depicted on C -1 are revised to the new location of the tower. Also assure
these setbacks are measured from edge of the facility (which shall include the ground equipment and/or any part of the
facility — see County Ordinance Section 5.1.40(c)3.
2) [5.1.40(a)4(h)] Access ways. Revise location of access way as discussed in the field.
3) [5.1.40(a)4(i)] District boundaries. Assure the Ag Forestal district boundaries are labeled and depicted on the plan.
Also, assure the conservation easement is labeled on the adjacent properties as this will most likely come up at the
PCBOS meeting per County Ordinance Section 5.1.40(b)6, thus it shall be depicted & labeled on the plans.
4) [5.1.40(a)6(e)] Balloon tests. Provide balloon test photos for the proposed facility as discussed at the site (for all
adjacent neighbors, to also include photo sims from Mr. JC. Wolf's property, Mr. Izard's property, David Bass's property,
the view along Rte 29 southbound, and the view fronting the site from Rte 29 (at the entrance of Mooreland Lane) and any
other photos you feel are needed. Assure you identify the camera type, film size, and focal length of the lens for each
photo.
5) [5.1.40(a)6(e)9] Photo simulations. Provide photo simulations as discussed at the site for all adjacent neighbors, to also
include photo sims from Mr. JC. Wolf's property, Mr. Izard's property, David Bass's property, the view along Rte 29
southbound, and the view fronting the site from Rte 29 ( Mooreland Lane), and any other photo sims you feel are needed.
6) [5.1.40(a)4] Design. The design of the facility, including the specific type of support structure and the design, type,
location, size, height, and configuration of all proposed antennas and other equipment. On the plan provide the size and
exact dimensions of the RRH and OVP devices. The equipment and size will be counted toward the total number of
arrays for the facility and the size of the antenna.
7) [5.1.40(b)6] If the facility would be located on lands adjacent to a conservation easement or open space easement, the
aciliU shall be sited so that it is not visible from any resources specifically identified for protection in the deed o�
easement. The Johnson property is adjacent to a few properties with these types of Conservation Easements: an Open
Space Easement (DB 4544 -118) and a Historic Preservation and Open Space Easement (DB 4104 -218).
While Mr. Bass was ok with the modified location after the balloon test due to the reduced visibility, ultimately the BOS
will have to make the finding that the structure is not visible from the historic house as it's identified for protection in the
deed of easement. Also, Mr. Izard's property is also under a Conservation Easement for Open Space preservation, see
D134544 -118. I'm still evaluating the deed relating to Mr. Izard's property with relation to it specifically identifying any
resource for protection. I wanted to give you a heads up on this aspect of the proposal, as we had not discussed this yet.
8) [Comment] Sheet G -1 of the site plan, revise the local map and the vicinity map so they are legible. Currently they are
not legible.
9) Also take into consideration VDOT's comments during your revisions (see attached).
Their comment #1 — shall be noted on the plan

Their comment #3 — shall be provided prior to SP approval.
Their comment #4 - — shall be noted on the plan
When do you anticipate providing the revised site plans, assure the above are incorporated into the revised plans.
