Ana—To clarify,the fee was submitted last week. What has changed since last week with regards to the utility bonds? I
thought you told me that we didn't need a VSMP in place to replace bonds related to water and sewer installation,only
to replace the E&S/SWM components of the plans.

Ana—Just wanted to follow up on our conversation last week. Just to recap:
1) SMC will finish the work ASAP to release the WPO bond and Water/Sewer bond for WPO-2011-00075. This
work to release the WPO bond should be completed this week. We will start the dedication paperwork for the
water/sewer.

WPO-2011-00075 Glenmore Lot 2'$`tiVater and Sewer Plan, September 14, 2011 Erosion only in the amount of
$18,400. You agreed to finish the work on this project asap and will submit a bond inspection request.The
inspection fee is$250. This bond will not be replaced.
2) The County said SMC would be allowed to replace the water and sewer bond for WPO-2011-00037 without a
VSMP permit in place. I guess I am waiting on the bond request form here, right?
WPO-2011-00037 Glenmore Section K2B Water and Sewer Plan June 13, 2011 Phase A. At this time,you cannot
replace the WPO bond. Per our meeting,Scott indicated that a VSMP application was submitted and the fee
was pending. Was the fee submitted? VSMP application needs to be approved by the County. County needs to
apply for your DEQ permit. Glenn will let me know when you can replace the wpo bond. You may contact Glenn
to discuss this wpo bond.
3) The County requested that SMC complete the work secured by WPO-2011-00037 rather than replace the surety
because no VSMP permit is in place. SMC noted that this will not work from a timing standpoint,though we are
willing to start the work as soon as possible (we were denied a pre-con since there is no VSMP in place). SMC
submitted a VSMP permit for the project last week. My request here is that you allow us to replace the surety
and perform the work once the bank is out of the picture. It doesn't make sense to me that the County will ask
SMC to start work without a permit, but the County won't let SMC replace a surety without the VSMP permit in
place. Please advise.
At this point,you cannot replace the bond. Please contact Glenn to discuss.
4) I will get in touch with the bank to see if they will give us any more time (after they give us 30 day notice that
they will not continue to post the LC—I don't want to remind them).
a.) Jonney—please send me a brief email noting the small scope of work remaining to release WPO-
2011-00075. I will use this as evidence to the bank that we are close to getting this one off bond.
b.) Ana—I will copy you on the email to them noting that we are working diligently together to replace
the WPO-2011-00037 bond. If this is not the case, let me know now before I send the email asking
you to confirm.

County Code section 17-410 and Virginia Code §62.1-44.15:34 requires the VSMP authority to
act on any VSMP permit by issuing a project approval or denial. This project is denied. The
rationale is given in the comments below. The application may be resubmitted for approval if all
of the items below are satisfactorily addressed. The VSMP application content requirements can
be found in County Code section 17-401.
A. Stormwater Pollution Prevention Plan (SWPPP)
The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must
contain(1)a PPP,(2)an ESCP,(3)a SWMP, and(4)any TMDL measures necessary.
M�1. On the registration statement,revise the acreage for limits of disturbance to include Phase B only.
2. On"Nature of activity„reduce the#of lots to"6 lots.,,
B.Pollution Prevention Plan (PPP)
The PPP content requirements can be found in County Code section 17-404.
1) Include an 11„x 17„Pollution Prevention Plan map showing concrete washout,ports johns,
fueling stations,and trash containers. This will be marked up by the contractor as construction of
site progresses.
C. Stormwater Management Plan(SWMP)
VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a
SWMP. This plan is disapproved,and the reasons are provided in the comments below. The
stormwater management plan content requirements can be found in County Code section 17-403.
1) Include Ditch 5&6 calculations and profile.This can be found in sheet 5 and 10 of approved
W P02011-37.
2) On Sheet 3, revise the purpose to refer to disturbed acreage involved in Phase B. It appears only
6 lots need to be in the narrative. Revise acreage also.See image.

D.Erosion and Sediment Control Plan (ESCP)
Virginia Code§62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP.
This plan is disapproved. The erosion control plan content requirements can be found in County
Code section 17-402.
1) Silt fence near toe of SB3 need to follow contour lines. It is ineffective when installed down a
slope.
The VSMP permit application and all plans may be resubmitted for approval when all comments have
been satisfactorily addressed. For re-submittals please provide 2 copies of the complete permit package
with a completed application form.
Engineering plan review staff are available from 2-4 PM on Thursdays, should you require a meeting to
discuss this review.
Process;
After approval,plans will need to be bonded. The bonding process is begun by submitting a bond estimate
request form and fee to the Department of Community Development. One of the plan reviewers will
prepare estimates and check parcel and easement information based on the approved plans. The County's
Management Analyst will prepare bond agreement forms,which will need to be completed by the owner
and submitted along with cash,certificates or sureties for the amounts specified. The agreements will need
to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to
obtain all the correct signatures and forms.
Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded.
The County's Management Analyst or other staff will prepare the forms and check for ownership and
signature information. The completed forms will need to be submitted along with court recording fees.
After bonding and agreements are complete,county staff will need to enter project information in a DEQ
database for state application processing. DEQ will review the application information based on local
VSMP authority approval. At this time,the DEQ portion of the application fees will need to be paid
directly to the state. For fastest processing,this is done electronically with the emails provided on the
application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the
application,they will issue a permit coverage letter. This should be copied to the county.
After DEQ coverage is issued,via the coverage letter,the County can hold a pre-construction conference.
Applicants will need to complete the request for a pre-construction conference form,and pay the remainder
of the application fee. The form identifies the contractor and responsible land disturber,and the fee
remaining to be paid. This will be checked by county staff,and upon approval, a pre-construction
conference will be scheduled with the County inspector. At the pre-construction conference, should
everything proceed satisfactorily,a joint VSMP and grading permit will be issued by the County so that
work may begin.

Here are my final comments:
1) Revise the riser structure in SB. It appears to be embedded in embankment.Also see how perforated pipe detail
will be affected.

/3) The emergency spillway should be lined with stone (to the toe).Check E&S and SWM plans to ensure this. I
believe all basins are showing earthen access road for emergency spillway on SWM plans.

plans reflect changes to address your comments as described below for final approval:
A. Stormwater Pollution Prevention Plan(SWPPP)
1. Thank you for your approval of the SWPPP report,pending its final signatures,in your e-mail 7/8/15.
B. Pollution Prevention Plan(PPP)

C. Stormwater Management Plan(SWMP)and D. Erosion and Sediment Control Plan(ESCP)
1. Sediment basin#1's riser structure was revised to have a rim elevation of 301.00.This rim elevation is
consistent with direction provided by Ms. Roberge of County Engineering to Mr. Murray of Collins Engineering
in a phone conversation dated July 8,2015.With respect to the perforated pipe,the detail shown on sheet 8
was updated to reflect its lower solid pipe being buried below ground(see'Riser/Emergency Spillway').The
invert elevations shown on sheet 9 are still acceptable.The standpipe can still easily be installed,it is simply
designed at a slightly further offset from the riser.With its anchoring to the riser at top and its lower pipe being
firmly buried in the ground,the design continues to function as intended and the risk of it separating from the
riser is no greater than before with the slightly increased offset.
2. The safety fence surrounding sediment basin#1 has been removed from the ESC sheets.A note mandating
signs on individual posts reading,"Warning,quick sand, keep out"has been added to sheet 6 in the basin's
detail replacing the fencing.The fencing surrounding the SWM facility continues to be shown on the SWM
plans.
3. The sediment basins and stormwater management facilities have been revised to show spillways with riprap
linings that extend to the toe of the dams.
4. The existing storm pipe, previously not labeled on sheet 6, is now called out to be abandon,and removed if
found necessary.
5. Proposed outlet protection has been added and labeled on sheet 8 for the proposed culvert outfalling into
sediment basin#1's forebay.
6. Sheets 5 and 10 have been added to the back of the set for the culverts and ditches.A note referencing this has
been added in bold text below the sheet index on the cover sheet.
Please also note the plans have been revised to mandate the contractor replace the concrete weir for sediment basin#1
with a riprap weir in accordance with a separate phone conversation with Ms.Roberge on July 9,2015.The plans have
also been revised to add the verbiage,"or equivalent"with respect to the Hanson trashrack.This minor change will allow
the contractor to install StormRax trashracks instead,which was previously deemed acceptable by Ms.Roberge.
