Please find attached the Letter of Revision Application along with the required
submittal documents.
The proposed site changes include:
• Construction of a small, modular block retaining wall with a metal safety
fence.
• Extensive landscaping above and below the wall and along the adjacent
slope.
• The wall is located at the southeast building corner along the back of the
site as shown on the attached 11 x17 from the Approved Site Plan.
• The ARB has approved the plan and issued a Certificate of
Appropriateness.

GENERAL NOTES:

BEFORE BEGINNING ANY WORK. THE CONTRACTOR SHALL IMMEDIATELY NOTIFY THE

2. COORDINATE AND PERFORM ALL WORK IN A MANNER THAT ALLOWS FOR SAFE AND

CONSTRUCTION. THE SITE SHALL REMAIN AVAILABLE TO THE OWNER AT ALL TIMES.
3. THE CONTRACTOR IS SOLELY RESPONSIBLE FOR MAINTAINING THE HEALTH, SAFETY
AND WELFARE OF THE PUBLIC AT ALL TIMES WITHIN THE LIMITS OF THE PROJECT
AND ALL NECESSARY WORK AREAS. THE CONTRACTOR SHALL PROVIDE FLAGMEN,

PUDDLING OR PONDING OF WATER OCCURRING ON ANY SURFACES.
4. ALL EXCAVATIONS FOR UNDERGROUND PIPE INSTALLATION, SHORING, AND SLOPES SHALL
COMPLY WITH ALL OSHA STANDARDS FOR THE CONSTRUCTION INDUSTRY AND ALL OTHER
LOCAL, STATE AND FEDERAL CONSTRUCTION STANDARDS AND/OR REQUIREMENTS.

5. NO CONSTRUCTION SHALL COMMENCE UNTIL THE OWNER HAS SECURED ANY/ALL

Rivanna Water & Sewer Authority (RWSA)
General Water & Sanitary Sewer Notes - March 2008
1) ALL MATERIALS AND METHODS OF CONSTRUCTION SHALL COMPLY WITH THE LATEST
VERSION OF THE GENERAL WATER AND SEWER CONSTRUCTION SPECIFICATIONS AS
ADOPTED BY THE ALBEMARLE COUNTY SERVICE AUTHORITY ON JANUARY 15, 1998,
EXCEPT AS MODIFIED BELOW OR MODIFIED IN SPECIAL NOTES.
2) ALL CONSTRUCTION MATERIALS AND METHODS OF CONSTRUCTION MUST BE APPROVED
BY RWSA PRIOR TO THE START OF WORK. A PRECONSTRUCTION CONFERENCE WITH
RWSA IS REQUIRED PRIOR TO THE START OF WORK.
3) THE CONTRACTOR SHALL BE RESPONSIBLE FOR NOTIFYING MISS UTILITY
(1-800-552-7001).
4) RWSA ENGINEER (MICHELLE SIMPSON AT (434) 977-2970 EXT. 202) MUST BE
NOTIFIED THREE BUSINESS DAYS PRIOR TO THE START OF CONSTRUCTION.
5) ALL WORK IS SUBJECT TO THE INSPECTION AND ACCEPTANCE BY RWSA. NO TIE-INS
TO THE EXISTING SYSTEM SHALL BE MADE WITHOUT COORDINATION WITH AND THE
PRESENCE OF RWSA STAFF. NO WORK SHALL BE CONDUCTED ON RWSA FACILITIES ON
WEEKENDS OR RWSA HOLIDAYS UNLESS PERMITTED OTHERWISE BY RWSA IN WRITING.
6) FOR SANITARY SEWER LINE CONSTRUCTION: RWSA MAY REQUIRE BYPASS PUMPING
FOR TIE-INS TO THE EXISTING SYSTEM. ALL DOGHOUSE MANHOLES MUST BE PRESSURE
TESTED PRIOR TO CONNECTING TO THE SYSTEM.
7) THE LOCATION OF EXISTING UTILITIES IS SHOWN ON THE PLANS FROM DATA
AVAILABLE AT THE TIME OF DESIGN AND IS NOT NECESSARILY COMPLETE OR CORRECT.
THE CONTRACTOR SHALL BE RESPONSIBLE FOR THE VERIFICATION OF THE LOCATION, SIZE
AND DEPTH OF ALL EXISTING UTILITIES, BOTH SURFACE AND SUBSURFACE. THE
CONTRACTOR SHALL IMMEDIATELY NOTIFY THE ENGINEER OF ANY DISCREPANCIES

10) IN ADDITION TO MINIMUM SEPARATION REQUIREMENTS FOR UTILITY CROSSINGS, WHEN
A CROSSING IS MADE UNDER AN EXISTING FACILITY, ADEQUATE STRUCTURAL SUPPORT
SHALL BE PROVIDED FOR THE EXISTING PIPE. BACKFILLING THE AREA OF THE CROSSING
WITH COMPACTED 57 STONE TO THE SPRINGLINE OF THE EXISTING PIPE WILL BE
REQUIRED.
11) NEW WATER MAIN INSTALLATIONS SHALL BE TESTED, CHLORINATED, FLUSHED AND
WATER SAMPLES APPROVED PRIOR TO MAKING ANY PERMANENT CONNECTION TO THE
PUBLIC WATER SYSTEM. APPROVED METHODS OF FILLING AND FLUSHING NEW WATER
MAINS WILL BE REQUIRED TO ELIMINATE ANY POSSIBLE CROSS -CONNECTION TO THE
PUBLIC WATER SYSTEM.
12) ALL EASEMENTS FOR NEW RWSA FACILITIES SHALL BE RECORDED PRIOR TO
PLACING THE NEW FACILITIES INTO SERVICE.
13) NO PERMANENT STRUCTURAL FACILITIES WILL BE PERMITTED IN THE RWSA
EASEMENT. THIS INCLUDES BUILDING OVERHANGS, RETAINING WALLS, FOOTERS FOR ANY
STRUCTURE, DRAINAGE STRUCTURES, ETC.

RETAINING WALL CONTRACTOR TO FACILITATE FUTURE FENCE POST INSTALLATION. CONTRACTOR SHALL
VERIFY PROPER SPACING REQUIREMENTS PRIOR TO INSTALLATION.
2. ASSEMBLY & INSTALLATION - REFER TO INSTRUCTIONS PROVIDED WITH UNITS FOR SPECIFIC INFORMATION
RELATED TO THE ASSEMBLY OF THE SLEEVE-ITTM SYSTEM AND THE CORRECT INSTALLATION PROCEDURE.
WHEN THE SEGMENTAL RETAINING WALL HAS BEEN CONSTRUCTED TO TWO FEET FROM TOP NOT INCLUDING
THE CAPSTONE:
STEP 1: PREPARE A LEVEL AREA APPROXIMATELY 24" WIDE X 36" DEEP BEHIND THE WALL FACE. THE
PREPARED AREA SHOULD BE 24" BELOW
THE PROPOSED TOP OF WALL (NOT INCLUDING THE CAP STONE).
STEP 2: TAKE THE TWO SLEEVE HALVES, ONE FRONT (NO SLOTS) AND ONE BACK (WITH SLOTS) AND LAY THEM
ON A LEVEL SURFACE WITH THE IN (SMOOTH FINGERS) AND THE OUT (RAISED FINGERS) OPPOSITE EACH
OTHER. INTERWEAVE THE TWO SLEEVE HALVES BY PUSHING THE IN FINGER SETS UNDER THE OUT FINGER
SETS. FLIP THE SLEEVE OVER AND FOLLOW THE SAME PROCEDURE ON THE OTHER SIDE. STAND THE UNIT
VERTICALLY AND USE THE TWO PLASTIC TIES TO SECURE THE SLEEVE HALVES INTO A CYLINDRICAL UNIT.
STEP 3: PLACE THE PLASTISOL COATED CANTILEVER BASE ON THE PREPARED AREA WITH THE VERTICAL
UPRIGHT ABOUT 6" FROM THE TAIL OF THE BLOCK.
STEP 4: SLIDE THE SLEEVE (OVER THE VERTICAL LEG (THE UNCOATED PORTION) WITH THE SLOTTED SIDE OF
THE SLEEVE FACING AWAY IFROM THE WALL FACE.
STEP 5: SLIP THE UNCOATED END OF EACH STRUT THROUGH THE SLOTS LOCATED IN THE BACK OF THE
SLEEVE AND CONNECT THEIM TO THE TOPMOST TRANSVERSE BAR ON THE VERTICAL LEG INSIDE THE SLEEVE.
CONNECT THE COATED ENDS OF THE STRUTS TO THE COATED BASE PORTION OF THE STEEL CANTILEVER ON
THE SECOND TRANSVERSE IBAR FROM THE REAR OF THE BASE.
STEP 6: REPOSITION THE ENTIRE SYSTEM AS NEEDED BY LIFTING IT USING THE TOP TRANSVERSE BAR OF THE
VERTICAL PORTION OF THE STEEL CANTILEVER INSIDE THE SLEEVE AFTER ASSEMBLY. MAKE SURE THE WALL
BATTER FOR ANY REMAININ(G COURSES OF BLOCK IS ACCOUNTED FOR WHEN POSITIONING THE SLEEVE-ITTM IN
ITS FINAL LOCATION.
STEP 7: PLACE ENOUGH 3/4"' CLEAN STONE AROUND THE SYSTEM TO STABILIZE IT. SET THE LID IN PLACE WITH
THE HANDLE PERPENDICULAR TO THE WALL FACE. USE THE HANDLE AS THE CENTER LINE MEASURING GUIDE
TO ENSURE THAT THE NEXT' SLEEVE-ITTM IS PLACED WITH THE PROPER SPACING REQUIREMENTS AS
DIRECTED BY THE FENCE SPECIFICATIONS.
STEP 7: PLACE ENOUGH 3/4"' CLEAN STONE AROUND THE SYSTEM TO STABILIZE IT. SET THE LID IN PLACE WITH
THE HANDLE PERPENDICULAR TO THE WALL FACE. USE THE HANDLE AS THE CENTER LINE MEASURING GUIDE
TO ENSURE THAT THE NEXT SLEEVE-ITTM IS PLACED WITH THE PROPER SPACING REQUIREMENTS AS
DIRECTED BY THE FENCE SPECIFICATIONS.
STEP 8: WHEN INSTALLING GEOGRID AROUND THE SLEEVE-ITTM SYSTEM, SLIT THE GOEGRID PERPENDICULAR
TO THE WALL FACE JUST ENOUGH TO FOLD AROUND THE SLEEVE ENSURING THAT THE GRID IS PROPERLY
ATTACHED TO THE WALL FACE EVERYWHERE WITH THE EXCEPTION OF WHERE THE SLEEVE IS. THIS METHOD
IS ACCEPTABLE BY GEOGRID MANUFACTURERS WHEN OBSTACLES SUCH AS FENCE POST FOUNDATIONS ARE
PRESENT.

PORTION OF THE CANTILEVER BASE. DO NOT INSTALL POSTS BETWEEN VERTICAL LEG AND WALL FACE. FILL
CAVITY COMPLETELY WITH CONCRETE. WHEN CONCRETE CURES, TOPSOIL OR OTHER SURFICIAL COVER MAY
