And with this submittal what should we include in the packet. This is more of a refresh questions.
1.) A new SWPPP form. —Yes, form Attached
2.) VSMP Application as a minor amendment with a$200 fee and signed by the owner. Yes, form Attached
3.)VRRM spreadsheet or a portion thereof. Or is this something submitted digitally. Print/submit relevant portions: si

The Deed of dedication for access to and around the Stormwater Facilities is something submitted upon its recordin
locating the farm road that leads to the site from State Route 20.

I spoke with Jeremy concerning sediment trap construction prior to permit modification. I recommend that you esti
and work closely with Jeremy. In my view, since conversion of field/meadow to stabilized turf does not trigger wat
requirements, you may construct and stabilize the three traps shown on plans brought to plan review session 9-Jul, I
modification. Again, please work closely with Jeremy as we would expect stabilization within a matter of days one
set. Please use permanent seed mix, lime, fertilizer, straw mulch, and a warm/cool weather seed mix depending on
performed. Areas downslope of traps must be protected with silt fence. We appreciate your asking about this.
