FLOODPLAIN DEVELOPMENT PERMIT
This form is intended to aid the Department in characterizing the nature of development proposed in the floodplain.
Development includes buildings or other structures,mining,dredging,filling, grading,paving, excavation or drilling
operations or storage of equipment or materials. The form responses will be used to determine whether a floodplain
impact plan or special use permit is required.

Please describe the nature of the development activity proposed within the floodplain (e.g. new structure, grading for a
playing field,driveway stream crossing, installation of a fence, etc.):
SDP2014-052 proposes to re-establish restaurant business at the intersection of State Route 240,State Route 680,US Route 250,
and Lickinghole Creek.The building footprint will be 4,000 square feet,smaller than proposed in a previously approved Site Plan,and
will not encroach on the floodplain.The entire parking area will fall within the limits of area that has previously been established as
"Existing/Non-Conforming"by the County Zoning Administrator(LOD2012-005,attached),and will continue to be maintained.
SDP2014-052 proposes no development within the floodplain.
