Stormwater Pollution Prevention Plan (SWPPP) The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain (1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary. 1. Albemarle County must sign/date Registration Statement (ACCD/Engineering will coordinate). (Addressed: 23 -Mar 2016) 2. Provide SWPPP Certification statement (VAR10, Part III, K.4_)(Addressed: 23 -Mar 2016) 4. Certification. Any person signing a document under Part III K t or 2 shall make the following certification: 1 certify under penalty of law that I have read and understand this document and that this document and all attachments were prepared in accordance with a system designed to assure that qualified personnel properly gathered and evaluated the information submitted, Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the information, the information submitted is, to the best of my knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties for submitting false information, including the possibility of fine and imprisonment for knowing violations." 3. SWPPP is comprehensive. However, please update insert sheets to reflect revisions per ESC plan comments, Sec. C. below. Note (3/24/16)—County/Engineering may possess more recent insert sheets (SWPPP download, 7/14/15 —Also, #4, below). CD/Engineering will follow-up with FES. 4. CE -101 or CE -102 —Show initial locations of: sanitary/porta john; concrete wash-out draining to pit/area with impermeable lining; vehicle wash draining to sediment trap/equivalent; on-site fuel storage/re-fueling areas with containment (if proposed); non -hazardous solid waste dumpster (if provided); covered /protected paint -solvent storage (if present). Note: these PPP facility locations are required to be shown on SWPPP exhibit, not full-size ESC plans. These locations may change as needed (if revise locations, show via pencil edits to on-site SWPPP exhibit). VSMP: SWPPP: Stormwater Management Plan (WP0201500021) VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a SWMP. This plan is approved. Drainage and ESC Narrative for: Crozet Avenue North Sidewalk Improvements Project (REPORT), 27 -Mar -15, Water Quality Sec. explains: "The pollutant removal requirement of 0.33 lb/yr will be purchased from a nutrient bank." Until the effective date of regulations establishing application fees in accordance vnth § 62.1-44.19:20, the credit provider shall pay the Department a water quality enhancement fee equal to sixpercent of the amount paid by the applicant for the credits. Also, prior comment. 30. May have encountered reference to off-site waste area during initial review, but cannot locate reference. If an off-site waste area proposed, ensure off-site area has an approved ESC plan. The VSMP permit application and all plans may be resubmitted for preview (as .PDFs). When all comments have been satisfactorily addressed, and if there are no additional comments, please provide 2 copies of the plans, 1 copy of SWPPP. Encourage .PDF submittals that show revisions. Engineering plan review staff is available 2-4 PM on Thursdays, should you require a meeting to discuss this review. Plan reviewer is available at 434.296-5832-0069 should you have any questions. Process: Description below inaccurate. Area of disturbance <1 Ac., so VPDES permit coverage letter not required. SWPPP required as condition of Albemarle County Code. 17-405. A pre -construction meeting was held 25 -Mar. Project has received Grading Permit, and is clear to proceed to construction. After approval, county staff will need to enter project information in a DEQ database for state application processing. DEQ will review the application information based on local VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest processing, this is done electronically with the emails provided on the application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter. This should be copied to the county. After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference. Applicants will need to request a pre -construction conference by completing a form, and pay the remainder of the application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that work may begin.
