The critical slope waiver request has been reviewed. The engineering analysis of the request follows:
Description of critical slope area and proposed disturbance:
This is a Berm previously constructed to block the view of the quarry from the view from State Route 250. Due to
the age of the quarry, no approved_plans for the berm could be located in the County Filing system. However,
University of Virginia historical trial photography appears to show a berm constructed sometime after 1974 and
before 1980, most likely under Federal Mines and Minerals permit. The Berm runs parallel to the state road and is
partially covered in grass with small to medium pine trees. The applicant is proposing to relocate the entrance for
the quarry to a signaled intersection to allow a more safe and convenient access. The existing entrance will be closed
and berm restored in that area.

This proposal appears to be similar to one reviewed and approved by the ARB in 1999. Some
aspects of the proposal have changed, including the size and shape of the office, and the current
plan shows no proposed planting and includes no information on lighting.
If it is determined that the office will be visible from the Route 250 Entrance Corridor, an ARB
application will be required. To determine if the office will be visible, the applicant should
submit to -scale building elevations and site sections from Route 250 to the building. Regardless
of whether or not the building is visible, the site plan should be revised to include the planting
that was previously approved, or an alternate planting scheme can be reviewed by the ARB as
part of a new ARB application. If lighting is proposed, complete lighting information should be
included on the plan for review. If no lighting is proposed, it would be useful for the applicant to
indicate this in writing.
The current plan shows a sign proposed in the median near Route 250. A sign was approved with
the ARB's 1999 action, but a resubmittal of the sign proposal should be made to ensure
consistency with the previous approval. An alternate design may require a new ARB application
or inclusion of complete sign information with the ARB applications discussed above). Also,
the applicant should note that a separate sign permit application will be required for the sign.

1. 32.5.6 (d)] A critical slopes waiver is required for the disturbance of the critical slopes
shown on the plan. Submit a request per the requirements stated in Chapter 18 Section
4.2.5.
2. 32.5.6 (i)] Show the right -of -way lines, width, and centerline for all existing and
proposed streets and travelways.
3. 32.5.6 (m)] Show the removal of the existing entrance. Also show the distance to the
centerline of the nearest existing street intersection from the proposed ingress to the

VDOT is currently reviewing options for replacement of the Route 250 bridge to the
east of the proposed entrance. The entrance plan and the bridge plan will need to be
coordinated to ensure no conflicts will occur between the two plans.
VDOT is ready to install the signal when the entrance is constructed and will need a 3
month advance notice of the opening of the entrance to ensure the signal construction
is complete.
Please indicate the purpose of the gate. Will it be open and free flowing during
business hours or will all traffic have to stop at the gate to enter the site at all times?
SDP - 2009 -00060 Martha Jefferson Hospital —Major (Summer Frederick)
There is not currently a ditch along State Farm Blvd in this section and all drainage
from the road and shoulder will flow over the proposed parking lot. This may cause
problems and should be addressed with the plan.

The new parking lot may affect the TIA for the project. In accordance with the
Chapter 527 regulations, a letter of validity for the traffic study needs to be submitted
for this change.
