IMoNv- County of Albemarle Department of Community Development Memorandum To:Ray Burkholder From:Gerald Gatobu, Principal Planner Division: Zoning and Current Development Date:August 12, 2009 Subject: SDP2009 -00059 Collins Medical Center Final Site Plan The County of Albemarle Division of Zoning and Current Development will grant or recommend approval of the final site plan referred to above once the following comments have been addressed: [Each comment is preceded by the applicable reference, which is to the Subdivision/Zoning Ordinances unless otherwise specified.] 32.5.6 The preliminary site plan shall contain the following information: 1. Maintenance Factor Light Loss Factor LLF (0.0672) shown on sheet C11 [lighting plan page] needs to be 1 and not 0.672. Please revise. 2. Lumen levels: Light fixtures emitting more than 3000 lumens must be full cut off. Please make sure both fixtures are full cut off. The lighting fixtures shown on the page C11 are not full cut off. 3. The standard lighting note needs to be added to the site plan. The standard lighting note reads as follows: "Each outdoor luminaire equipped with a lamp that emits more than 3000 lumens shall be a full cut off luminaire. The spillover of lighting from luminaires onto public roads and property in the residential or rural area shall not exceed on -half footcandle. " 4. Indicate on the luminaire schedule site plan the number of lumens emitted by the wall mounted light fixture 8410WALocation and dimensions of all existing and proposed improvements including: 5. The conservation plan checklist on page C10 needs to be checked and signed. 6. The dumpster pad needs to be at least 10' x 18'. See Albemarle County Code Sections [4.12.13.e and 4.12.19] these two sections were cited on the preliminary site plan approval letter. 7. A 10 foot parking setback line [for off street parking] from Westfield Road [State Route 1425 ] needs to be delineated on the site plan. The Erosion & Sediment Control and Stormwater Management Plans (WPO200900036) submitted 18 September 2009 have received Engineering Review and meet Albemarle County minimum checklist items for approval. Please submit 5 paper copies of the Plan to Current Development Engineering. The E &SC bond amount is $11,900.00. The SWM bond amount is $33,300.00. The forms and instructions to post the Water Protection Ordinance Bond can be found on the Community Development Department Web site on www.albemarle.org. A completed copy of the Standard Stormwater Maintenance Agreement and fee for recordation for the stormwater facilities is required prior to final approval.[17 -304E] The forms and instructions can be found on the Community Development Department Web site on www.albemarle.org. You may contact Ana Kilmer (Albemarle County Department of Community Development) at ext. 3246 for further inforniation on bonding procedures and Maintenance Agreements. Once the E &SC bond and the SWM bond have been posted, you may contact the Department of Community Development to arrange a pre - construction meeting with a County Erosion & Sediment Control Inspector. Should the County Inspector find the limits of disturbance increased or the need for additional control measures required to protect the site, additional fees will be required.
