Ray,
I was able get a determination from the Chief of Current Development on the validity of the previously approved
Critical Slopes Waiver on this site. The Critical Slopes Waiver, approved with SP200500033 by the Planning
Commission on March 14, 2006, will remain valid so long as the Final Site Plan which was approved on this site
does. The Final Site Plan (SDP200500136) was approved April 18, 2007, and is valid for 5 years from that date.
It is understood that the development you are proposing is different than the previously approved site plan,
however, if the disturbance to the critical slopes remains in general accord with the previously proposed
disturbance, then the waiver remains valid. This Critical Slopes Waiver was approved with the following
condition:
1. Peak rate and velocity of runoff shall be controlled to pre - development levels.
I would suggest that you print this email and submit it along with your preliminary site plan so that it is clear this
determination has been made.

not be considered final. However, the Site Review Committee has attempted to identify all issues that
could affect approval of the proposed project.
Please make the revisions that have been identified as necessary for preliminary approval by the Site
Review Committee. If you choose not to make the requested revisions, please submit in writing
justification for not incorporating such revisions. Submit eight (8) full size copies and one (1) 1 l " x 17"
copy to the Department of Community Development including responses to each of the attached
comments of the Site Review Committee by Monday, June 1 2009. Failure to submit this information
by this date will result in suspension of the review schedule. Review will resume when revisions are

The County of Albemarle Division of Zoning and Current Development will grant or recommend approval of
the preliminary site plan referred to above once the following comments have been addressed: [Each comment is
preceded by the applicable reference, which is to the Subdivision/Zoning Ordinances unless otherwise
specified.]
32.5.6 The preliminary site plan shall contain the following information:
a. The name of the development; Include the site plan number SDP2009 -00031 to the site plan for
current and future reference. Please consider having a cover sheet with pertinent site information.
Owner, zoning, tax map and parcel number and present use of adjacent parcels; Please add the zoning
of adjacent properties
b. Written schedules or data as necessary to demonstrate that the site can accommodate the proposed use,
including:
Maximum number of employees; Include the number of employees to be working at the dental
office.
Maximum amount of impervious cover on the site; please add the amount of impervious cover on the
site plan in square feet.
If a landscape plan is required, maximum amount of paved parking and vehicular circulation areas
Clearly delineate the amount of paved parking and vehicular circulation areas. The amount of
paved parking and vehicular circulation areas will be used for landscape calculations]
e. Existing landscape features as described in section 32.7.9.4.c. provide an existing landscape features
sheet per section 32.7.9.4.c. of the zoning. ordinance.
n. Location and dimensions of all existing and proposed improvements including:
Outdoor lighting: Proposed outdoor lighting should meet the outdoor lighting regulations found
in section 4.17 of the Zoning Ordinance. A lighting plan that meets required outdoor lighting
regulations must be submitted at final site plan stage (if outdoor lighting is proposed)
Signs: please remove the proposed sign location shown on the site plan. Sign location shall be
approved with/during sign permit application.
Signature Panel: Please add the attached signature /approval panel to the site plan.

The preliminary site plan for Collins Medical Center, submitted on April 22, 2009, has been reviewed.
Current Development Engineering has the following comments:
Preliminary Site Plan Comments
1. Please correct the date on the plan's title block.
2. Please provide a modified simple method spreadsheet to show the required removal rate for the
drainage area.
3. The angle of the dumpster pad creates an awkward backup situation. It is recommended that the
dumpster pad be rotated 90 degrees and centered within the 24' aisle width. [18- 32.7.2]
4. Please show site distance lines at the entrance onto Westfield Road, easements may be required.
Final Site Plan Comments
The following comments are not required to be addressed at this time. They are provided for informational
purposes only.
5. Depending on the depth and size of the proposed detention pipe, an easement may be required on
TMP 61W-02-0B-5+. Please see Albemarle County Design Standards Manual Section 6 for
required easement widths.
6. The stormwater management plan must meet MS -19 of the Virginia Erosion and Sediment Control
Handbook. Please provide information showing that the pipe and channel downstream is adequate.

Reviews Comments: [tall has reviewed this preliminary site plan for conformity with the Comprehensive Plan, including the'
Neighborhood Model. Staff recommends that the building be moved close to the street and the
parking be placed behind the building in order to be in compliance with the Neighborhood Model
principle recommending relegated parking. If the building were moved closer to the street, it would
also form an even line of building fronts along the street with the buildings to the east and west.
Staff is also concerned that, with the addition of the entrance to this property, there will be three
entrances very close to each other.

Below are VDOT's comments on the Site Plans for the May 21 2009 Site Review
Committee Meeting:
SDP - 2006 - 00041 Briarwood Phase 1A - 1, 1B - 1, 8 (Elizabeth Marotta)
1. Engineering comments were provided by VDOT on 4 -23 -09 for the additional phase
of the project.
SDP - 2009 - 00030 Forest Lakes Office Park - Major (Summer Frederick)
1. No comments
SDP - 2009 - 00031 Collins Medical Center - Preliminary (Gerald Gatobu)
1. Final plans need to show the proposed entrance profile.
2. Final plan needs to show details for CG -6, CG -11, and CG -12B and detectable
warning.
3. A Land Use Permit will be required for construction within the public road right
of way.
SDP - 2009 -00033 University Village Retirement Community(Phase I) Summer
Frederick
1. The proposed connection to Route 29 needs to be a minimum of 510 feet from the
existing intersection or line up with the intersection at the signal.

2. This site plan exceeds the threshold to require a Chapter 527 Traffic Impact
Analysis.
3. The proposed road must provide a connection to the existing Boulders Road to
meet the connectivity requirements of SSAR's, 24VAC 30 -92. Without the
connection the road may not be eligible for acceptance into the State Secondary
System of Highways.
4. The CG -12 detail needs to be updated and show the proper detectable warning.
5. The proposed Functional Classification needs to be indicated on the plan.
6. If the connection is made to Boulders Road, additional traffic will need to be
shown to properly determine the design speed and pavement structural section of
the road.
7. Provide the horizontal alignment data on the roads.
8. Street design data on sheet R -4 lists University Village Road but the plans show
University Village Dr. Is this the same road?
9. The sight distance on University Village Dr. / Rd. need to be based on the correct
design speed on the road.
10. Pavement design on University Village Dr. needs to be designed in accordance
with the AASHTO method if this is designed as a collector road.
11. Storm sewer pipes need to be as perpendicular as possible to the centerline of the
road and cannot exceed 45 degrees.
12. Street Trees on University Village Dr. need to be located outside of the clear zone
of the road if this road is to be classified as a collector.
SDP - 2009 -00034 Martha Jefferson Outpatient Care Center (Proffit) -Major (Gerald
Gatobu)
1. It appears that the parking and detention facility for this site is in conflict with a
previous plan for the widening of Proffit Road and the construction of a
roundabout at this intersection. The road plan indicates that there has been a
dedication of right of way for the road widening and intersection improvement.
Please request the applicants provide a written description of revisions with re- submissions. If
you have any questions or comments, please contact me prior to sharing these comments with
the applicants.
