This checklist must be completed,signed and submitted with the application. The information contained in this checklist reflects the contents of the Site Plan Ordinance as of May 1,2006. The applicant is responsible for insuring that no revisions to the Ordinance have occurred since preparation of this document. The approval of the preliminary site plan is valid for (1) one year in accordance with Section 32.4.3.1 of Chapter 18 of the Albemarle County Code. Therefore, the preliminary approval shall expire on December 16, 2009. If the preliminary site plan approval expires, a new application must be filed and processed. Please address all of the requirements and conditions listed below and submit eight (8) tentative plan copies to the Department of Community Development. This letter must be submitted with the tentative plans, as a checklist, to document that you have addressed all requirements or conditions, or the tentative plan will be denied. Erosion and Sediment Control, BMP Stormwater Management, and road plans with the associated applications and fees must also be submitted with the eight (8) tentative Plans. Once the tentative plan is submitted and reviewed you will receive comments from all departments/divisions/agencies that have made comment on the tentative plan. Any further responses must be made directly to each department/division/agency that has further comment. After all aforementioned departments/divisions/agencies have granted a tentative approval, you must verify with the Planner that you may submit the final mylars (2 sets), two paper copies, the final site plan application, and final site plan fee to the Department of Community Development. Signing of the plans will occur within one week once it is determined that the final site plan mylars reflect all tentative approvals. The final site plan will be subject to all final site plan requirements (Zoning Ordinance Section 32.6), in addition to the following conditions. The Department of Community Development shall not accept submittal of the final site plan for signature until tentative approvals for the following conditions have been obtained: 1 Zoning & Current Development approval to include: 1. The plans consist of 3 sheets entitled"St.Anne's—Belfield Lower/Middle School,Faulconer Road, Charlottesville Virginia"that bear a P.E. seal date of October 3,2008. The project has been designed for an average flow of 8,800 GPD based on the estimated wastewater flow from 500 persons at the school,with a peak flow of 37 GPM based on a 2.5 peak factor. The project consists of installing approximately 850 feet of 3-inch force main,a duplex submersible pump station, and associated appurtenances.The 3-inch force main will discharge to an existing manhole near the school property line. The Albemarle County Service Authority has documented capacity in its collection system. The Pump Station is a submersible grinder pump station rated at 45 gpm at 88 feet of TDH. This sewage pump station has been designated Reliability Class 2 and will meet the requirements of this classification by the use of an onsite audio/visual alarm, an automated dialer system to relay all critical alarms to a location manned 24 hours per day, and by the use of building closure during loss of power. The evaluation of these plans has been confined to technical requirements and design criteria as stipulated in the Commonwealth of Virginia Sewage Collection and Treatment Regulations(9 VAC 25-790).The review of gravity sewers was waived due to low flow(less than 40,000 gpd). In accordance with the Code of Virginia 1950, as amended,Title 62.1, Section 62.1-44.19,this letter report is to advise that the previously mentioned plans and specifications are technically adequate and are approved,with the following condition: • Mr. Mike Waylett Page 2 Project: St. Annes Belfield Pump Station CTC PT Log# 20624 1. An Operation and Maintenance Manual is to be maintained for the facility, and the pump station is to be operated and maintained in accordance with that manual. This letter provides your authorization to construct the previously described project. This approval is valid for 5 years from the date of this approval letter. This letter also provides your authorization to operate the previously described project for 30 days following construction,prior to issuance of a Certificate to Operate. Once construction is complete,a Statement of Completion of Construction must be submitted to this office in compliance with the requirements of 9 VAC 25-790-180.0 by the licensed engineer who oversaw construction of the project. Please refer to the Office of Wastewater Engineering Website for information on the Certificate to Operate(CTO)issuance process: www.deq.virginia.gov/wastewater. Because of funding issues,the Office of Wastewater Engineering duties are being scaled back considerably. The CTO process is being changed at the moment,and the website will give the most up to date instructions. Please note: DEQ Office of Wastewater Engineering program approvals and staff knowledge of project status and project issues do not substitute for nor relieve any parties from compliance with requirements for other DEQ divisions including WQIF grant and VRLF loan programs,consent orders, permits, and other regulatory and enforcement matters of DEQ and other agencies. Please be aware that disturbance of any streams and/or wetlands may also require permitting.
