My inspection determined that corrections are required. The necessary repairs and /or corrections are as follows: 1) Install and pave with asphalt, at no steeper than 15 %, the 10' wide vehicular access and pedestrian path from the parking lot to the rip -rap channel that is to be installed at the northeast end of the stormwater basin as shown on the approved plan. The plan states pedestrian paths to be paved. 2) Install the vehicular access across the emergency spillway as shown on sheet 16 of 20 of the approved plan. The plans show a 5:1 slope on each side of the spillway with a 10' wide flat spillway section. 3) Install rip -rap channel and EC -3B slope matting material as shown on the approved plan where the water enters your pond from the adjoining property. 4) A new problem has been created with a channel that was not shown on your plans or the plans for Pantops Place, Phase 2 (Jefferson Heights Cottages). This channel discharges into the stormwater basin and is eroding a channel that will carry sediment material into the basin. This will need to be addressed before we can convert the sediment basin into your stormwater facility. 5) The barrel pipe from the riser structure in the sediment basin /stormwater facility has a leak and will need to be repaired. Also, the dewatering pipe on the side of the riser will need to be removed and the hole sealed. A small dewatering pipe with stone around it is to be installed on the gate valve at the bottom of the riser for dewatering. This detail can be found on sheet 16 of 20. The stormwater facility is to have a wet surface elevation of 488.0 with the aquatic and safety bench to be at 487.0. You may need to have your contractor field verify these elevations because we do not want to install the aquatic American Senior Housing, LC' c/o Mr. Thomas Frank June 10, 2008 Page 2 of 2 plants required around the aquatic bench of the stormwater facility only to have them dic. The planting list is located on sheet 11 of 20 and the plants location is shown on sheet 12 of 20. Installing the plants cannot take place until all items have been repaired and the site is stabilized. Planting should be the last item you complete. 6) The slope from the stormwater basin to the back of the units is eroding in areas and will need to be repaired. Roof drains from the units along the top of the slope are discharging onto the slope which is not helping matters, and there are two pipes in this slope that discharge onto a rip- rap /stone pad that is washing out along the side into the pond. The area around these two pipes is very steep and someone has placed rip -rap in the area to hold the slope back, however, it does not appear that it is working. This arca will need to be addressed by either adding to the pipe to soften the slope or install a retaining wall to hold the slope properly. Storm drain structures to be repaired: 1) Structure A -6. Relocate steps in manhole to make structure accessible. 2) Structure A -5. Remove bucket and debris from bottom of structure and shape with cement mortar the bottom of the structure to prevent ponding water. 3) Structure A -3. Shape bottom of structure with cement mortar to prevent ponding water. 4) Structure B -1. Install steps to make the structure accessible.
