Subject: State Farm Boulevard -VSMP Permit Plan Review Comments
A. Stormwater Pollution Prevention Plan (SWPPP)
1. SWPPP has not signed by owner or designed operator. Please have all documents signed in the
SWPPP prior to final approval. The'operator"is generally the owner of the project for the approval
process and then can be transferred at any time after approval with the"delegation of authority"form.
Kimley-Horn submitted SWPPP to the County on June 11, 2015.
2. The SWPPP Construction Site Notice form is incorrect and states the site drains into Meadow
Creek. This site drains into the Rivanna River.
Kimley-Horn updated the Site Notice per the comment.
B. Pollution Prevention Plan(PPP)
1. PPP Plan was not included in submittal package. Please see Albemarle County forms:
Kimley-Horn submitted the PPP to the County on June 11, 2015.
C. Stormwater Management Plan (SWMP)
1. The purchase of Credits will need to be approved by the County Attorney's Office. Please contact
Ana Kilmer akilmeranalbemarle.orq for required information and documentation for credit purchases
prior to finalizing the purchase.
Purchase of the Credits are currently under review by the County Attorney's office.
D. Erosion and Sediment Control Plan (ESCP)

1. Please show the E&SC protection measures on the grading plan for adequate review.The E&SC
plan and the Grading plan are at different scales and could not be overlaid for review.
Kimley-Horn added E&S items to the appropriate sheets.
2. Phase 2 E&SC plan does not appear to have any E&SC measure shown. Please explain or
remove from submittal.
Kimley-Horn utilized a single phase concept.
3. Please show the staging, stockpile, and parking areas for equipment, supplies, and employees.
Responsibility of Contractor and will not be shown.
4. Please show limits of disturbance and state the area of disturbance plan.
Please see E&S plans.

Subject: State Farm Boulevard-VSMP Permit Plan Review Comments Updates
A. Stormwater Pollution Prevention Plan (SWPPP)
All comments addressed on previous submissions.
B. Pollution Prevention Plan (PPP)
1. PPP Plan has been included with SWPPP but does not show the proposed locations for any of the
items listed in the PPP document. Please show locations on PPP for: Solid Waste Disposal; Sanitary
Facilities; Water Source; Stockpile locations; Equipment cleaning, maintenance, and repair; Concrete
wash down areas.
Kimley-Horn submitted updated plan sheet show the proposed locations of the items requested
above.
C. Stormwater Management Plan (SWMP)

D. Erosion and Sediment Control Plan (ESCP)
1. The site fence shown down slope is a Diversion and will need a catchment at the bottom of the
diversion to capture sediment and dissipate energy. This office recommends showing Right of Way
Diversions across the proposed disturbance in a manner that will direct the stormwater flows into the
grass areas for filtering. The Check dam locations should suffice for right of way locations.

Kimley-Horn updated the location of the rock check dams to divert water from the edge of pavement
to the silt fence an allow water to filter out as discussed on July 30,2015.
2. Plan approval will require the staging, stockpile and parking areas for equipment, supplies and
employees. Please have responsible party submit plans for right of way locations.
Contractor will submit proposed location for staging, stockpile, and parking areas.
3. Limits of disturbance could not be found on the plan submittal, narrative states the total area for
this project is 0.75. The staging and stockpile areas may increase the limits of disturbance and will
need to be corrected.
Kimley-Horn has updated the plans to show updated limits of disturbance which has increased to
1.19 acres.
