Section 3 Nature of Activity
A. Approved Plans
The activities associated with the Thomas Jefferson Foundation Water and Sewer
Improvements Part II are contained in the VSMP Plan WPO 2015. A copy of the
WOW approved plans is to be maintained on the project site and made available to Federal,
State and Local officials.
B. Nature of Activities
�- The construction activities are shown on the plans referenced in Section A above.
The activities include the following:
O` • Clearing, grubbing and grading;
• Earthwork operations cuts and fills;
• Stormdrains;
• Construction of new water storage tank, and;
• Erosion and sediment controls.

Construction is intended to start in September of 2015 and continue for a six to nine

C. Construction Schedule
1. Mandatory pre-construction meeting with Albemarle County Erosion and
a 11 Sediment Control Officer(1 day).
2. Installation of Erosion and Sediment Controls(7 days).

The above schedule is general and timeframes are based on general knowledge of
the duration of time associated with the various construction activities for a project of this
magnitude. The schedule presented may be altered as required. Items 1, 6 and 9 are
mandatory meetings with the County Inspector.

Section 4 Erosion and Sediment Control Plan
A. Approved Plan
The VSMP Plan WPO2015 contains the Erosion and Sediment Control Plans. An
11" x 17" copy of the plan is contained in this Section of the SWPPP. A copy of the
••• approved VSMP Plan will be issued by the Albemarle County Erosion and Sediment
Control officer at the pre-construction meeting to be held on-site.
B. Erosion and Sediment Control Plan
�«. The erosion and sediment control plan is depicted on the VSMP Plan Sheet 4 of 6.
A narrative of the Erosion and Sediment Controls is contained on Sheet 3 of 6. Erosion
and Sediment Control details and support calculations are contained on the VSMP Plan
Sheet 5 of 6.
The intent of the plan is to permit clearing and grading to facilitate the installation
of the new water storage tank. Erosion and sediment control measures include a
construction entrance, inlet and outlet protection, level spreaders, and the perimeter
controls consisting of silt fence.
Care shall be taken to preclude transport of sediment off-site. Strict adherence to
the limits of construction/disturbance is required.
C. Special Considerations and Requirements
1. The proposed land disturbance for this project discharges to Cow Branch
tributary to Moores Creek (VAHU6 JRI5). Moores Creek has been identified
as being impaired or for which a TMDL waste load allocation has been
established for sediment or sediment related parameter or nutrients.

Therefore, the following SWPPP requirements (Part 11.A.5) must be
implemented for the land disturbing activity as follows:

a. Permanent or temporary soil stabilization shall be applied to denuded

areas within seven (7) days after final grade is reached on any portion of
the site;
b. Nutrients (e.g., fertilizers) shall be applied in accordance with
manufacturer's recommendations or an approved nutrient management
plan and shall not be applied during rainfall events;
c. Inspections shall be conducted at a frequency of(i) at least once every
four (4) business days or (ii) at least once every five (5) business days
and no later than 48 hours following a measurable storm event. In the
event that a measurable storm event occurs when there are more than 48
01111 hours between business days, the inspection shall be conducted on the
next business day; and
d. Representative inspections used by utility line installation, pipeline
construction, or other similar linear construction activities shall inspect
all outfalls.
M- D. Inspection
1. Inspections shall be performed by qualified personnel as listed in Section 8 of
the SWPPP.
�• 2. Inspection schedule shall be in accord with that listed in the General Permit
Part 11 F2 (pages 10 and I 1 of 21)of Section 11 of this SWPPP document.
3. Inspection requirements are listed in the General Permit Part II F3 (pages 11,
12 and 13 of 21)of Section 11 of this SWPPP document.
4. Inspection report shall contain the items listed in the General Permit Part II F4
�• (page 13 of 21)of Section 11 of this SWPPP document.

5. The inspection report and any actions taken must be retained by the operator
as part of the SWPPP for at least three years from the date that general permit
coverage expires or is terminated. The inspection report shall identify any
incidents of noncompliance. Where an inspection report dos not identify any
incidents of noncompliance, the report shall contain a certification that the
"" construction activity is in compliance with the SWPPP and this general
permit. The report shall be signed in accordance with the General Permit Part
.. III K(page 17 of 21)of Section 11 of this SWPPP document.
6. Inspection Log/Report is presented in Section 12 of this SWPPP document.
E. Rainfall Event
1. The location of the on-site rain gauge that will be used to determine the
occurrence of a measurable storm event for the purposes of ESC and Pollution
Prevention inspections will be provided by the Contractor and identified on
the record set of plans or in other appropriate SWPPP documents for this land

disturbance(construction)activity.
•• 2. The rain gauge shall be observed daily at 1:00 PM to determine the
occurrence of a measurable storm event (i.e., 0.25 inches of rainfall or greater
.w. in a 24 hour period). A log book shall be maintained to record observation
information which shall include (i) the date, (ii) the time, (iii) whether or not
rainfall is occurring at the time of the observation, (iv) the amount of
accumulated rainfall in the gauge,if any, and(v)whether or not an inspection
is required based on the amountof accumulated rainfall in the gauge.
3. If there is no rainfall occurring at the time of the observation, the observation
information shall be noted in the log book and the rain gauge emptied and
replaced. An inspection is required if there is 0.25 inches or more
accumulation noted in the rain gauge.

4. If there is rainfall occurring at the time of the observation, the observation

Section 5 Stormwater Management Plan
A. Approved Plan
The VSMP Plan WPO 2015 contains the Stormwater Management Plan and consists
of Sheet Number 6 of 6. An 11" x 17" copy of this sheet is contained in this Section of
the SWPPP. A copy of the approved VSMP Plan will be issued by the Albemarle County
Erosion and Sediment Control officer at the pre-construction meeting.
B. Stormwater Management Plan
Stormwater quality is not required for the improvements proposed with the

Montalto Water Tank. The runoff reduction spreadsheet calculations for redevelopment
indicate that the minimal amount of additional impervious area created as part of this
project does not require storwmater quality measures to be implemented and meets the
removal requirements as shown. A copy of the runoff reduction spreadsheet is provided
on the VSMP Plans.
A stormwater runoff analysis was performed for the 1.29 acre Montalto tank site to
determine the pre and post-developed peak discharge for the project. The access road
�. and parking area for the new storage tank were analyzed using the Virginia Runoff
Reduction spreadsheet for re-development. The access road and parking area were
treated as a disconnected impervious area with sheet flow to alb soils. The analysis
yielded a reduced curve number (CN) for the post developed condition of 58 from the
pre-developed curve number of 59. The nature of the site will remain unchanged and the
time of concentration is not affected. Therefore due to the reduced CN in the post-
..
developed condition the runoff from the site has been reduced and the requirements of
MS-19 are met.

Section 6 Pollution Prevention Plan
A. An 11" x 17" plan of the site showing pollution activities and practices is contained
in this Section of the SWPPP.
B. Sources of Pollutants and Locations, Controls and Prevention Practices
1. Sources and Locations
a. Staging Area will support vehicular parking and provide temporary
storage of building materials.
b. Construction activities, including water tank construction, cut & fill
operations, landscaping and seeding.
c. See Table No. 1 on page 6-12.
2. Prevention Practices
a. Good Housekeeping. Good housekeeping practices include activities
that are intended to maintain a clean site and keep equipment in good
r- working order to prevent stormwater quality problems from occurring.
Daily cleanup and inspections are the most effective means of achieving
good housekeeping. For the most part, good housekeeping is a day-to-
day activity that does not require a large expenditure of time or expense,
and should be implemented on an ongoing basis. Examples of good
housekeeping practices are:
(i) Tools and materials should be returned to designated storage areas
after use.
(ii) Waste materials should be collected and properly disposed after
the completion of each job, shift, or day as appropriate.

(iii) Indoor work areas should be neat, uncluttered, and well-ventilated
to discourage outdoor work and to allow leaks and spills to be
quickly detected and controlled.
(iv) Outdoor work areas should be swept regularly (not hosed) and kept
neat and clean.
(v) Occasionally outdoor work areas may need cleaning beyond
sweeping. In such cases, all wash waters should be contained,
collected, and properly disposed.
(vi) Outdoor waste or trash receptacles should be covered and emptied
regularly and the adjacent areas inspected for misplaced or wind-
,.
blown litter.
b. Proper Materials Handling and Storage. Materials handling and storage
practices relate to controlling the potential for leaks, spills and losses on
materials delivered, used, and stored at a facility. Spills and leaks of
materials can accumulate in soils or on surfaces and be carried away in
*• stormwater runoff or authority non-stormwater discharges.
(i) Materials Use
• Only obtain the amount of materials needed to finish a
particular job;
■- • Limit waste generation by keeping good records and reviewing
activities;
• Recycle materials whenever possible; and
• Read and follow manufacturer directions for use of materials
and review the associated Material Safety Data Sheet(MSDS).

(ii) Materials Storage
• Store materials indoors or in a covered area where exposure to
stormwater is minimized;
• Store lead-acid batteries indoors and within secondary
containment;
• Use hazardous materials storage lockers with spill containment
or flammable materials lockers when appropriate;
• Locate storage areas away from vehicle and equipment paths to
reduce the potential for accident-related leaks or spills;
• Do not store drums or other containers close to storm drain
inlets;
• Provide informational signing, labels, restricted access, locks,
inventory control, overhead coverage, and secondary
containment for all hazardous material storage areas or
container units; and
• Conduct regular inspections for leaks and control dates.
c. Proper Waste Handling. Waste handling practices relate to properly
controlling, collecting, storing, and disposing of wastes that are
generated at a facility. All facility personnel should be aware that
disposing any waste (including wash waters) into a storm drain inlet or
stormwater conveyance (e.g., streets) is considered illegal dumping.
Likewise, disposing of waste (including wash waters) onto a paved or
..� unpaved surface such that it may be carried to a storm drain inlet or
stormwater conveyance (e.g., streets) is also considered illegal dumping.

(i) Sweep or vacuum (dry methods) work areas to collect metal,
wood, and other particulates and debris frequently;
(ii) Limit waste generation by keeping good records and reviewing
.� activities;
(iii) Recycle materials whenever possible;
(iv) Separate and segregate different types of wastes;
(v) Store waste materials indoors or in a covered area where exposure
�• to stormwater is minimized;
(vi) Arrange for regular waste disposal;
(vii) Use hazardous materials storage lockers with spill containment or
flammable materials lockers when appropriate;
(viii) Locate the waste storage area away from vehicle and equipment
paths to reduce the potential for accident-related releases;
(ix) Provide informational signage, labels, restricted access, inventory
controls, overhead coverage, and secondary containment for all
hazardous waste storage areas or container units; and
(x) Conduct regular inspections for leaks and control dates.
d. Spill Prevention. Spill clean-up can be labor-intensive and costly
involving expenses to contain the spill, collecting the spilled substance,
proper disposal of spill materials, and report filing to regulatory
agencies, not to mention possible monetary fines. Spills and leaks are
some of the most significant sources of water pollution and are, in most
cases, avoidable.

Spill prevention, control, and cleanup applies to all materials and wastes
— not only hazardous substances. The toxic water quality effects from
spills of hazardous substances (e.g., acids, oils, greases, fuels, solvents,
pesticides) are commonly understood. However, non-hazardous
materials — for example, sand, litter, corn oil, sweeteners, soaps, and
food products, among others—can also greatly impact water quality.
(i) Placing bollards, berms and containment features around structures
or areas where fluids are stored, so releases can be prevented,
easily detected, and controlled;
(ii) Using drip pans for maintenance operations involving fluids and
under leaking vehicles and equipment waiting repair;
(iii) Placing spill kits in areas where fluids are stored or in areas where
activities may result in a spill;
�— (iv) Providing training for proper use of materials and equipment used
during operations and maintenance activities;
(v) Providing training for proper use of spill response equipment and
supplies; and
(vi) Conducting outdoor maintenance activities on gravel or paved
surfaces to allow for easy detection, control, and cleanup of spills.
�,.. C. Non-Stormwater Discharges
1. All support activities are located on-site. There are no off-site support
activities.
2. There are no commingled discharges associated with this project other than
those authorized in C(3).

3. The following non-stormwater discharge from construction activities are
authorized by the General Permit when discharged in compliance with the
General Permit(see Section 11 of this SWPPP).
a. Discharges from firefighting activities;
b. Fire hydrant flushings;
c. Waters used to wash vehicles or equipment where soaps, solvents, or
detergents have not been used and the wash water has been filtered,
settled, or similarly treated prior to discharge;
d. Water used to control dust that has been filtered, settled, or similarly
treated prior to discharge;
e. Potable water sources, including uncontaminated waterline flushings;
f. Routine external building wash down where soaps, solvents or
detergents have not been used and the wash water has been filtered,
settled, or similarly treated prior to discharge;
g. Pavement wash waters where spills or leaks of toxic or hazardous
materials have not occurred (or where all spilled or leaked material has
been removed prior to washing); where soaps, solvents, or detergents
�•• have not been used; and where the wash water has been filtered, settled,
or similarly treated prior to discharge;
h. Uncontaminated air conditioning or compressor condensate;
i. Uncontaminated ground water or spring water;
j. Foundation or footing drains where flows are not contaminated with
process materials such as solvents;

k. Uncontaminated excavation dewatering, including dewatering of
trenches and excavations that have been filtered, settled, or similarly
treated prior to discharge; and
1. Landscape irrigation.
4. Prohibition of Non-Stormwater Discharges. Except as provided in items 1, 2
and 3 above, all discharges covered by this general permit shall be composed
entirely of stormwater associated with construction activities. All other
discharges including the following are prohibited:
a. Wastewater from washout of concrete;
••• b. Wastewater from the washout and cleanout of stucco, paint, form release
oils, curing compounds, and other construction materials;
c. Fuels, oils, or other pollutants used in vehicle and equipment operation
and maintenance;
d. Oils, toxic substances, or hazardous substances from spills or other
releases; and
e. Soaps, solvents, or detergents used in equipment and vehicle washing.
D. Persons Responsible for Pollution Prevention Practices
The qualified personnel listed in Section 8 of this SWPPP will be the person(s)
responsible for Pollution Prevention Practices.
E. Responses and Reporting Practices
1. Unauthorized Stormwater Discharges. Pursuant to § 62.1-44.5 of the Code of
Virginia, except in compliance with a state permit issued by the department, it
shall be unlawful to cause a stormwater discharge from a construction activity.

2. Reports of Unauthorized Discharges. Any operator who discharges or causes
or allows a discharge of sewage, industrial waste, other wastes or any noxious
or deleterious substance or a hazardous substance or oil in an amount equal to
or in excess of a reportable quantity established under either 40 CFR Part 110,
40 CFR Part 117, 40 CFR Part 302, or § 62.1-44.34:19 of the Code of
Virginia that occurs during a 24-hour period into or upon surface waters or
who discharges or causes or allows a discharge that may reasonably be
expected to enter surface waters, shall notify the Department of
Environmental Quality of the discharge immediately upon discovery of the
discharge, but in no case later than within 24 hours after said discovery. A
written report of the unauthorized discharge shall be submitted to the
department and the VSMP authority within five (5) days of discovery of the

discharge. The written report shall contain:
"" a. A description of the nature and location of the discharge;
.. b. The cause of the discharge;
c. The date on which the discharge occurred;

d. The length of time that the discharge continued;
e. The volume of the discharge;
f. If the discharge is continuing, how long it is expected to continue;
MOW g. If the discharge is continuing, what the expected total volume of the
discharge will be; and

h. Any steps planned or taken to reduce, eliminate and prevent a recurrence
of the present discharge or any future discharges not authorized by this
general permit.

Discharges reportable to the department of the VSMP authority under the
immediate reporting requirements of other regulations are exempted from this
requirement.
3. Reports of Unusual or Extraordinary Discharges. If any unusual or
extraordinary discharge including a "bypass" or "upset," as defined herein,
should occur from a facility and the discharge enters or could be expected to
enter surface waters, the operator shall promptly notify, in no case later than

within 24 hours, the department and the VSMP authority by telephone after
the discovery of the discharge. This notification shall provide all available
details of the incident, including any adverse effects on aquatic life and the
known number of fish killed. The operator shall reduce the report to writing
and shall submit it to the department and the VSMP authority within five (5)
days of discovery of the discharge in accordance with Part III I 2. Unusual
and extraordinary discharges include but are not limited to any discharge
resulting from:

a. Unusual spillage of materials resulting directly or indirectly from
processing operations;
b. Breakdown of processing or accessory equipment;

c. Failure or taking out of service of some or all of the facilities; and
d. Flooding or other acts of nature.
4. Reports of Noncompliance. The operator shall report any noncompliance
which may adversely affect surface waters or may endanger public health.
a. An oral report to the department and the VSMP authority shall be
provided within 24 hours from the time the operator becomes aware of
the circumstances. The following shall be included as information that
shall be reported within 24 hours under this subdivision:

(i) Any unanticipated bypass; and
�... (ii) Any upset that causes a discharge to surface waters.
b. A written report shall be submitted within five (5) days and shall
contain:
(i) A description of the noncompliance and its cause;
(ii) The period of noncompliance, including exact dates and times, and
if the noncompliance has not been corrected, the anticipated time it
is expected to continue; and
(iii) Steps taken or planned to reduce, eliminate, and prevent
reoccurrence of the noncompliance.
The department may waive the written report on a case-by-case basis for
reports of noncompliance under General Permit Part III I (see section 11
of this SWPPP) if the oral report has been received within 24 hours and
no adverse impact on surface waters has been reported.
c. The operator shall report all instances of noncompliance not reported
under General Permit Part III I 1 or 2 (see Section 11 of this SWPPP) in
writing as part of the SWPPP. The reports shall contain the information
listed in General Permit Part III I 2 (see Section 11 of this SWPPP).
NOTE: The reports required in General Permit Part III G, H and I (see
Section 11 of this SWPPP) shall be made to the department and the
VSMP authority. Reports may be made by telephone, email, or by fax.
For reports outside normal working hours, leaving a recorded message
shall fulfill the immediate reporting requirement. For emergencies, the
Virginia Department of Emergency Management maintains a 24-hour
telephone service at 1-800-468-8892. The contact at the Albemarle

d. Where the operator becomes aware of a failure to submit any relevant
-r facts, or submittal of incorrect information in any report, including a
registration statement, to the department or the VSMP authority, the
operator shall promptly submit such facts or correct information.
5. Notice of Planned Changes
a. The operator shall give notice to the department and the VSMP authority
as soon as possible of any planned physical alterations or additions to
the permitted facility or activity. Notice is required only when:
(i) The operator plans an alteration or addition to any building,
•� structure, facility, or installation that may meet one of the criteria
for determining whether a facility is a new source in 9VAC25-870-
. 420;
(ii) The operator plans an alteration or addition that would
significantly change the nature or increase the quantity of
pollutants discharged. This notification applies to pollutants that

b. The operator shall give advance notice to the department and VSMP
