This project is denied. The rationale is given in the comments below. The application may be resubmitted for approval if all of the items below are satisfactorily addressed. The VSMP application content requirements can be found in County Code section 17-401. A. Stormwater Pollution Prevention Plan (SWPPP) The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain (1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary. 1. Please have the property owner fill out and sign the registration Statement. 2. The Certification in Section #9 is not signed. The owner will need to sign the Certification. B. Pollution Prevention Plan (PPP) The PPP content requirements can be found in County Code section 17-404. 1. Item appears adequately addressed at this time. C. Stormwater Management Plan (SWMP) VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a SWMP. This plan is disapproved, and the reasons are provided in the comments below. The stormwater management plan content requirements can be found in County Code section 17-403. 1. VRRMsMread sheet indicates 0.8 lbs. of credits will need to be purchased. Please contact Ana Kilmer or information about nutrient credit purchasing documentation and requirements. D. Erosion and Sediment Control Plan (ESCP) Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is approved, and the reasons are provided in the comments below. The erosion control plan content requirements can be found in County Code section 17-402. 1. State DEQ requirements appear adequately addressed at this time. Engineering Review Comments Page 2 of 2 The VSMP permit application and all plans may be resubmitted for approval when all comments have been satisfactorily addressed. For re -submittals please provide 2 copies of the complete permit package with a completed application form. Engineering plan review staff are available from 2-4 PM on Thursdays, should you require a meeting to discuss this review. Process; After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and check parcel and easement information based on the approved plans. The County's Management Analyst will prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash, certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to obtain all the correct signatures and forms. Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The County's Management Analyst or other staff will prepare the forms and check for ownership and signature information. The completed forms will need to be submitted along with court recording fees. After bonding and agreements are complete, county staff will need to enter project information in a DEQ database for state application processing. DEQ will review the application information based on local VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest processing, this is done electronically with the emails provided on the application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter. This should be copied to the county. After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference. Applicants will need to complete the request for a pre -construction conference form, and pay the remainder of the application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that work may begin. VaRRM .xls references bioretention #2 (6.b.) while design appears to reflect certain Level 1 specifications, including: a. Treatment Vol. (T,) —CDA=6765sf; Rv, DA `B' =.7625, Tv =1.25*6765*.7625=12 =537cf. Revise Tv. b. Planting plan: A single 1" caliper tree is insufficient. Prepare a "planting template to include turf, herbaceous vegetation, shrubs, and/or trees to achieve surface area coverage of at least 90% within 2 years. If using turf, must combine with other types of vegetation" [BMP spec]. Turf with a single tree is an inadequate Level 2 planting plan (limited diversity). 2. Provide schedule 40 PVC underdrain cleanout. c. 15" DIA PVC standpipe top =494.3' which is >1.0' above bioretention invert =493.00'. Max, per spec, 1'. Revise. d. Sheet C811 —bioretention data table: provide L' x W' bioretention facility. e. Sheet C811 —bioretention data table: surface area =276sf does not check against effective storage depth =1.65', using either 429cf or 365cf. Confirm and show Level 2 bioretention surface area calculation. f. Include all relevant bioretention construction and maintenance notes found in Virginia DEQ Stormwater Design Specification #9/Sections 8 and 9. 3. VaRRM/D.A. A. (Hydrologic report): Filterra does not route to a wet pond but to a 72" DIA pipe that does not meet wet pond specifications. Underground pipe is a detention (not water quality) measure. Revise. Existing 48" RCP (Commonwealth Drive): Depict existing contours accurately. Inlet to pipe is obscured, partially or completely. 5. Existing 48" RCP: Furnish notes that describe in detail: 1) how accumulated sediment at inlet will be removed without release through the line, which may occur without care. Downstream properties will be impacted if more than trace sediment passes through the 48" DIA pipe during restoration of capacity of this line; and, 2) furnish structural design elements that restore hydraulic performance of this line while reducing, as much as possible, long-term maintenance needs. This is not a public maintenance responsibility —see item #5. 6. Existing 48" RCP: VDOT email (Troy Austin), Friday, May 24, 2013 9:47 AM (partial text): "Basically, the pipe under the road and in the right of way will be maintained by VDOT to the extent that it impacts the integrity of the road at this location. There should be a junction box on each side of the right of way delineating the extent of VDOT's maintenance extents for the pipe (unless it is located within a dedicated drainage easement). This situation needs to be investigated a little more to determine absolutely whether or not this off-site pipe is a VDOT pipe but my suspicion is that it is not." 7. Hydrologic report: Please number report pages (this/future reports —hand-written pg # acceptable) 8. Hydrologic report: Furnish Drainage Map that shows DA -5, DA -2 —include labels for impervious/pervious, consistent with VaRRM .xls. —see items #9-11 9. Label Limits of Disturbance. (0.96 Ac.) 2. Show DC symbol, plan view. Dust control is listed in ESC Legend, but not shown on plans. 3. Provide depression (or ST) downslope of sharp bends in temp diversion, since these points are prone to failure. (-ref. Pantops Corner project) 4. Revise DD plan view symbols to read DV, consistent with ESC legend. 5. Revise CE symbol, ESC legend, to read PCE, for paved construction entrance. 6. Provide L' X W' floor dimensions for sediment trap (ST). 7. Provide SAF between LOD and Trophy Chase Apartment complex parking lot. 8. Provide SF down slope of LOD/ST, upslope of drainage channel, to protect drainage way. 9. Show soil divide; label soil types. 10. Show ESC measures needed to support steps required to restore capacity of 48" DIA storm line. 11. C 12: Include detail for paved construction entrance with wash rack. 12. C12: Revise Elev.
