Date and source of topography is not stated on the plan. Where did the topo originate? When was the topo generated? Was it field verified and by whom? 4. Mitigation plan is required for the stream buffer disturbances. No mitigation plan was included in submittal. 10. Staging and parking areas not shown on plan. 11. silt fence and diversion are directing stormwater into stream and road without adequate protection.Sediment traps need to be shown to capture stormwater runoff. 12. Culvert areas do not have the proper symbols for work in a stream. Live streams will require a pump-around or other system proposed . NO WORK is allowed in a live stream. Replacement pipes will be designed for the drainage area to pipe. Please have all of these item addressed prior to resubmittal. I also dropped off an updated WPCPettplication signed by the landowner and have attached to the email the USACE approval. Please let me know if you need any additional assistance from me as this has become a very time sensitive issue. Please remove all sheets that are not the erosion and sediment control plan, and title the document an erosion and sediment control plan. Please use plan size sheets for the road. This plan is too difficult to piece together at 11x17. 3. Provide source and date of topographic information 4. Show and label WPO buffer limits. This road appears to have buffer disturbances and will require a mitigation plan. 5. Show and label floodplain limits. This road appears to be crossing the floodplain and will require a Floodplain Development Permit. Any fill in the floodplain, or crossings affecting floodplain will require a special use permit. 6. Provide evidence of board approval for critical slopes disturbances or eliminate these disturbances. 7. Any proposed slopes steeper than 3:1 must have an alternative to grass as permanent stabilization. 8. The construction entrance must drain to a sediment trap. 9. Provide DC, TS and PS on the plans in disturbed areas. 10. Provide a staging /parking area and stockpile location. 11. Silt fence running down or up contours is not effective.
