This project is denied. The rationale is given in the comments below. The application may be resubmitted for approval if all of the items below are satisfactorily addressed. The VSMP application content requirements can be found in County Code section 17-401. A. Stormwater Pollution Prevention Plan (SWPPP) The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain (1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary. 1. Per County Code Section 17-405 A. 1., please provide a signed copy of the registration statement. 2. Be sure to update the E&S and SWM sheets if any changes are made. 3. Sections 8 & 10 did not have any contact information. Please provide the information or make a note that it will be provided by the contractor at a later date. 4. Please ensure that Section 9, Signed Certification, is actually signed. B. Pollution Prevention Plan (PPP) The PPP content requirements can be found in County Code section 17-404. 1. Provide a copy of a plan showing the PP elements. Ensure that all PP elements are shown on this plan (staging/stockpile areas, washout areas, sanitary facilities, etc). C. Stormwater Management Plan (SWMP) VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a SWMP. This plan is disapproved, and the reasons are provided in the comments below. The stormwater management plan content requirements can be found in County Code section 17-403. 1. The County recommends you wait to resubmit this plan until you receive Initial Site Plan approval. 2. Please change the title of the plan from Initial Site Plan to VSMP Plan. 3. Please add a note or explain in the narrative why the existing conditions on Sheet C2.0 do not match predevelopment conditions on Sheet C6.1. 4. The Energy Balance requirement cannot be waived for the north outfall. The County suggests you Engineering Review Comments Page 2 of 3 look at B.1. and B.4. of 9VAC25-870-66 for channel protection. 5. County GIS shows steep slope overlays and 100 yr. floodplain on this site. Please show them on the plan. 6. Please review pipe nos. 203 and 241. The design computations differ from the profiles. 7. Double check inlet nos. 212 and 240. The lengths in the computations differ from the profiles. 8. Please add a note to ensure that there will be inlet shaping on any structure with a 4' or greater drop and safety slabs in any structure taller than 12'. 9. Please provide maintenance notes for SWM pond. Example: Section 9.2 of BMP Design Spec No. 15. D. Erosion and Sediment Control Plan (ESCP) Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is disapproved, and the reasons are provided in the comments below. The erosion control plan content requirements can be found in County Code section 17-402. 1. Per Section 3 of the County Design Standards Manual, a paved wash rack is required. Please note it on the plans and provide a detail. A detail is provided in the manual. 2. The silt fence on the NW side of the site is inappropriate. It crosses contours and is in the middle of the disturbed area for Phase II. The County suggests the use of a diversion dike and right-of- way diversion to a sediment trap in the western corner of the site. The silt fence at that western corner of the site appears inadequate (1/4 AC drainage area per 100 ft of silt fence). 3. There should be some perimeter controls on the northern end of the site (downstream from the basin) for both Phase I & 11. 4. Provide E&S sediment trap/basin design calculations and details. 5. In Phase II, provide silt fence at the entrance along Rt. 250 to contain any sediment and prevent vehicular access from the new entrance since it is not a construction entrance. 6. Provide outlet protection for STR 100. The VSMP permit application and all plans may be resubmitted for approval when all comments have been satisfactorily addressed. For re -submittals please provide 2 copies of the complete permit package with a completed application form. Engineering plan review staff are available from 2-4 PM on Thursdays, should you require a meeting to discuss this review. Process; After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and check parcel and easement information based on the approved plans. The County's Management Analyst will prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash, certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to obtain all the correct signatures and forms. Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The County's Management Analyst or other staff will prepare the forms and check for ownership and signature information. The completed forms will need to be submitted along with court recording fees. After bonding and agreements are complete, county staff will need to enter project information in a DEQ Engineering Review Comments Page 3 of 3 database for state application processing. DEQ will review the application information based on local VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest processing, this is done electronically with the emails provided on the application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter. This should be copied to the county. After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference. Applicants will need to complete the request for a pre -construction conference form, and pay the remainder of the application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that work may begin.
