Plan review staff are also available at 434-296-5832 (-x3069) should you have questions. Process: After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and check parcel and easement information based on the approved plans. The County's Management Analyst will prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash, certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to obtain all the correct signatures and forms. Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The County's Management Analyst or other staff will prepare the forms and check for ownership and signature information. The completed forms will need to be submitted along with court recording fees. After bonding and agreements are complete, county staff will need to enter project information in a DEQ database for state application processing. DEQ will review the application information based on local VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest processing, this is done electronically with the emails provided on the application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter. This should be copied to the county. After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference. Applicants will need to request a pre -construction conference by completing a form, and pay the remainder of the application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that work may begin. County forms can be found on the county website forms center under engineering; htt2://www.albemarle.ora/deptforms.asp?dgpartment=cdengmMo Thank you /434.296-5832 —x3069 WP0201500055_Branchlands MemCare_VSMP_022116rev2 pF A vtr1Q COUNTY OF ALBEMARLE Department of Community Development 401 McIntire Road, Room 227 Charlottesville, Virginia 22902 -4596 Phone (434) 296 -5832 Fax (434) 972 -4126 VSMP Permit plan review Project: Branchland Whistler House —VSMP Plat /Plan preparer: Craig Kotarski, Timmons Group 919 2' St. SE, Charlottesville, VA 22902 craig.kotarskiAtimmons.com Owner: Wial LLC, 12095 Gayton Rd, Richmond, VA 23238 [Graham Aldelman] Owner Rep: L. It appears, for example, that Elev. 423.7' provides <43.4 cy storage, when it may provide < 80.4 cy. Please Add note or revise volume(cy) heading to indicate incremental storage (/1' interval). Revise values as necessary. (Rev. 1) Addressed. 2. C3.1 - Revise ESC Legend, soil stabilization blanket =13M. (Rev. 1) Addressed. 3. C3.1 -Label floor of sediment trap, plan view (16' x 52'). (Rev. 1) Partially addressed /misunderstanding. As follow -up: please add sediment trap floor dimensions to plan view: (16' x 52). 4. C3.1 - Provide vegetative cover hardier than grass for proposed 2:1 slopes along SW edge of building. (ACDSM 8.A.2.); Ref, Phase II. (Rev. 1) Addressed. 5. C3.IIC5.1 - Specify material (Elev. 42 P to 424') to replace excavated material beneath biorctention since C3.1 stipulates removal to Elev. 421.00'. (Rev. 1) Addressed. 6. Show soil stockpile location, if any. (Rev. 1) Addressed. Engineering plan review staff are available from 2 -4 PM on Thursdays, should you require a meeting to discuss this review. Plan review staff are also available at 434 - 296 -5832 (- x3069) should you have questions. This may take 2 -4 weeks to obtain all the correct signatures and forms. Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The County's Management Analyst or other staff will prepare the forms and check for ownership and signature information. The completed forms will need to be submitted along with court recording fees. Engineering Review Comments Page 5 of 5 After bonding and agreements are complete, county staff will need to enter project information in a DEQ database for state application processing. After DEQ coverage is issued, via the coverage letter, the County can hold a pre - construction conference. Applicants will need to request a pre - construction conference by completing a form, and pay the remainder of the application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre - construction conference will be scheduled with the County inspector. At the pre - construction conference, should everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that work may begin. VSMP: SWPPP: Stormwater Management Plan (WP0201500055) VSMP Regulation 9VAC25- 870 -108 requires the VSMP authority to approve or disapprove a SWMP. This plan is disapproved for reasons provided in comments below. The stormwater management plan content requirements can be found in County Code section 17 -403 1. C0.0 — Revise Title: Branchlands Memory Care and Assisted Living, VSMP WPO201500055 2. Remove sheets C1.0, C1.1, L -001. Please mark through sheet numbers in index. 3. C3.1 — Revise IP @ Nyoplast overflow structure. Bioretention may not be installed until all upslope areas are stabilized. IP will not be required for bioretention overflow structure. 4. C4.0/4.1 — Revise sheet titles to `Site Layout' or similar. A VSMP may not approve a site plan. 5. C0.0 —Edit index. Ref. item #4. 6. C4.1 —Label pipe structure #105. 7. C4.1 —Label DI structure #108. Engineering Review Comments Page 2 of 5 8. C4.1 - Revise bottom of bioretention label to read floor elevation (or grade elevation) of bioretention. 9. C4.1 -Label bioretention clean -out. Label bioretention overflow structure. 10. LD -229: Storm Sewer Design Computations -in this (or separate table) list pipe structures # 109, 107. 11. C6.0 -Pipe Str. # 109 (6.83' of 15" HDPE), 107 (22.74' of 15" HDPE) @ 0.00% slope is problematic. Revise to provide positive slope at least 0.5 %. 12. C4.1 -There appears to be no MH access to either ADS "Stormtech" System (Area to W), or 3 -line, 125' L 60" DIA storage system (Area to S). Provide manway access to each system. 13. C4.1 - Provide notes, labels, or detail that shows how the two 50' lines of ADS system make connection, with particular emphasis on Access for maintenance. Ensure that each line is accessible. Provide same for 60" DIA, 125' L detention system (Area to S). All detention system lines must be accessible. Design should ensure debris accumulation will not block, back -up, or short- circuit either system. 14. C4.1 - Provide new Str. #104 away from biofilter; use Std. MH. The biofilter 24" Nyoplast overflow structure (dome drain) will be upstream of this MH. The Nyoplast structure is ineligible as an inline storm system element. Organic debris is likely to collect at the base of the overflow structure. Any obstruction of the 24" (dome drain) overflow structure will compromise the entire Area to W storm runoff system. 15. C5.1 - Recommend revise elevation of 4" perforated underdrain to 424.33' (biofilter). 16. C5.0 or C5.1 -Add Note: Bioretention may not be installed until all upslope areas are stabilized. IP will not be required for bioretention overflow structure. 17. C5.0 or C5.1 - Provide bioretention planting schedule. 18. C5.1 - Revise `bottom of biofilter 427.50' to read top of biofilter Mix. 19. C5.1 -Label elevation/top of biofilter cleanout: set Elev. at least 4 -6" above 3" hardwood bark mulch. 20. C5.1 -Label %slope of line between biofilter overflow structure and new MH (2% Min). Also, items #14, 29. 21. C5.1 - Provide plan view, Str. #106. Plan view /design must provide means of clearing pipe #107 of debris. 22. C5.1 -WEIR Plate in Str. 106 Detail -Show top of structure (top of weir /top of storage are shown). C6.0 shows Str. 106: MH -1 W/ WEIR PLATE, TOP = 430.04'. 23. C6.0 -Pipe #207, 205: Increase DIA to 15" Min. (Yard inlet system 12" DIA, is the exception). 24. Revise LD -229 to reflect pipe DIA increase, item #23. 25. C4.IIC6.0 -It is unclear how Str. 112, listed as MH -1, functions. Str. is located in parking area that routes to UG Detention, but is not identified as a grate or DI. Confirm design; revise profile label as necessary. 26. C6.0 -Str 204 -218 profile: Revise label to read INV IN 435.00 (213) PROP.
