The County's Management Analyst will prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash, certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to obtain all the correct signatures and forms. After bonding and agreements are complete, county staff will need to enter project information in a DEQ database for state application processing. DEQ will review the application information based on local VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest processing, this is done electronically with the emails provided on the application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter. This should be copied to the county. After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference. Applicants will need to complete the request for a pre -construction conference form, and pay the remainder of the application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that work may begin. Plan received date: 6 Feb. 2017 Date of comments: 21 Feb. 2017 Reviewers: Bobby Jocz [Rev -1] County Code section 17-410 and Virginia Code §62.1-44.15:34 requires the VSMP authority to act on any VSMP permit by issuing a project approval or denial. This project is denied. The rationale is given in the comments below. The application may be resubmitted for approval if all of the items below are satisfactorily addressed. The VSMP application content requirements can be found in County Code section 17-401. A. Stormwater Pollution Prevention Plan (SWPPP) The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain (1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary. 1. [General] Confirm accurate existing topography is provided. All topography should be at least visually field verified by the designer within the last year. 2. [General] Display extent of 100yr floodplain and WPO buffer {County Code 17-403(8)1. 3. [General] Plans show construction/grading occurring on the adjacent parcel. Please include these parcels and owner's signature on future submittals, or obtain temporary construction easement. 4. [SWPPP] Provide copy of DEQ coverage letter when obtained {County Code 17-405 1. 5. [SWPPP] Correct Table of Contents to reflect section headings in document. The SWPPP seems to be missing section 5 "Stormwater Management Plan", replacing it as the "Pollution Prevention Plan". 6. [SWPPP]The approved SWM plan will need to be included within the SWPPP when plans are ready to be approved {County Code 17-405}. 7. [SWPPP]The approved Erosion and Sediment Control Plan will need to be included within the SWPPP when plans are ready to be approved {County Code 17-405}. 8. [SWI] Update plan sheets and SWPPP with current WPO submittal number [WP020170001I 9. [SW5] Separate the SWM plan and E&S Phase II details on sheet SW5. The SWM plan should have its own sheet. 10. [SW6] Remove the "Frame Shed" diagram located over the parking lot on the southeastern portion of the site. According to plans this will already be demolished. 11. [SW8] Level spreader detail should be included as SWM detail, not on E&S detail sheet. 12. [All] Please more clearly label proposed and existing contour elevations. Engineering Review Comments Page 2 of 3 B. Pollution Prevention Plan (PPP) The PPP content requirements can be found in County Code section 17-404. 1. [SWPPP] Provide a Pollution Prevention Plan exhibit outlining expected locations of construction staging, parking areas, expected pollutant sources and pollution prevention practices. C. Stormwater Management Plan (SWMP) VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a SWMP. This plan is disapproved, and the reasons are provided in the comments below. The stormwater management plan content requirements can be found in County Code section 17-403. 1. [General] A stormwater management maintenance agreement, along with deed and plat for stormwater management easements encompassing both the proposed conserved forest and open space and the underground stormwater management facility, must be recorded prior to final VSMP approval. Contact Ana Kilmer, Management Analyst II, at (434) 296-5832 ex. 3246 for more information regarding the Stormwater Management Agreement. 2. [General] Provide Pre -development drainage areas. 3. [SW7] Please verify the correct drainage areas are represented in the VRRM spreadsheet. According to E&S narrative and presented drainage areas only 0.29 Ac will be protected as conserved forest and open space; however, this area is indicated as being 0.32 acres on the VRRM spreadsheet. 4. [SW7] Junction box and outlet structure details (diagrams 1&2) do not match what is presented on the structure profile and subsequent calculations. Please correct. 5. [SW7] Include long term maintenance notes for conserved forest and open space, level spreader, and underground stormwater management facility on plan sheet. 6. [All] The SWPPP and plan sheets indicate that water quality requirements will be met through conservation of conserved open space; however, the E&S narrative and hydrologic/hydraulic calculation packet state that water quality requirements will be met through the purchase of offsite nutrient management credits. Please clarify how 0.47lb/yr of phosphorous removal will be accounted for on site. 7. [Level Spreader] The level spreader appears to discharge water outside the parcel into areas not able to be included as conserved open space due to the lands not being owned by the parcel holder. The level spreader must discharge into areas that can be conserved as open space. Please correct or provide justification. 8. [Level Spreader] The water authority must approve construction of stormwater features over water and sanitary sewer easements. D. Erosion and Sediment Control Plan (ESCP) Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is disapproved, and the reasons are provided in the comments below. The erosion control plan content requirements can be found in County Code section 17-402. 1. [Sequence of construction] Indicate when demolition and initial grading activities will be occurring. 2. [Section B 11 of E&S narrative] Maintenance schedule task #7 states removal of controls is contingent on approval of "city" E&S inspector. Please change this to "County E&S Inspector". 3. [SW4 & SW5] Silt fence should be added along the southeast edge of the site. 4. [SW8] Provide tree protection detail as specified in E&S plan. Engineering Review Comments Page 3 of 3 The VSMP permit application and all plans may be resubmitted for approval when all comments have been satisfactorily addressed.
