This project is denied. The rationale is given in the comments below. The application may be resubmitted for approval if all of the items below are satisfactorily addressed. The VSMP application content requirements can be found in County Code section 17-401. A. Stormwater Pollution Prevention Plan (SWPPP) The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain (1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary. 1. Please update any E&S or SWM sheets if changes are made with the next submission. Rev.1: Comment addressed. 2. Ensure that the offsite disposal area is added to the SWPPP. Rev.1: Comment addressed. B. Pollution Prevention Plan (PPP) The PPP content requirements can be found in County Code section 17-404. 1. PPP appears satisfactory at this time. C. Stormwater Management Plan (SWMP) VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a SWMP. This plan is disapproved, and the reasons are provided in the comments below. The stormwater management plan content requirements can be found in County Code section 17-403. 1. Detention pipe is located offsite. Please show the agreement that allows this or move the pipe on the property. Also, if these pipes convey offsite water, they will need to be in an easement. Rev. 1: Please show this easement on sheet C5 to show that all grading occurs within the easement. 2. Grading occurs offsite in the NE corner. Please show the agreement or temporary easement that Engineering Review Comments Page 2 of 4 allows this proposed grading. Rev.l: Comment addressed. 3. Level 2 bioretention requires 2 forms of pre-treatment. Please illustrate the 2 forms being used. Rev.l: Comment addressed. 4. Provide infiltration test results or underdrain in BMP #2. Rev.l: Comment addressed. 5. Please provide maintenance requirements for detention pipe on the plan to include annual inspections, cleanings and necessary repairs. Rev.l: Comment addressed. 6. The actual Q Dev on PDF page 51 was whited out. Please correct so the number is shown. Rev.l: Comment addressed. 7. Provide summary/narrative on plan sheet that describes compliance with water quality and quantity requirement (show pre vs post quantity and phosphorus removal required vs achieved quality). Supporting calculations can remain in the provided SWM report. Rev. 1: Summary not provided on the plans. 8. Provide an easement around the BMP facilities that includes access for maintenance. Rev.l: Please label this easement, "SWM facility easement". Provide a definition on the plan and plat for SWM=stormwater management. Show the easement around the facilities only (access not necessary, it is provided in the SWM Maintenance Agreement). Please also label the easement around the detention pipes as "SWM facility easement'. Ensure that the easement around the detention pipes extends to the right -of way. (see attached sketch) 9. Please ensure that water drains away from the buildings. Provide spot elevations in the graded ditches. Show that these graded ditches will not erode over time. Rev.l: Comment addressed. 10. Provide detail for PG-4 flume and show the proposed flow/velocity vs. its capacity. Rev 1: Comment addressed. 11. Show equations used for inlet calculations or use standard VDOT Appendix 9B-1 LD-204 Stormwater Inlet Computations. Rev.l: Comment addressed. 12. Ensure that drainage in the southern corner of the site does not runoff into neighboring properties. It should be directed to the point of analysis 2 on site. Rev.l: Comment addressed. 13. A Stormwater Management (SWM) Maintenance Agreement and a SWM Easement Plat with a SWM Deed of Dedication will be needed before the VSMP plan can be approved. Please contact Ana Kilmer to begin the process of completing the Maintenance Agreement. The fee for the easement plat will be the one shown for the easement plat required with a site plan. We recommend you begin this process as soon as possible. Rev.l: Comment still valid. D. Erosion and Sediment Control Plan (ESCP) Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is approved, and the reasons are provided in the comments below. The erosion control plan content requirements can be found in County Code section 17-402. 1. Please provide a separate existing conditions sheet and ESC Phase 1 sheet. Insert the ESC Phase I sheet with the other ESC sheets of the plan set. Rev.l: Comment addressed. 2. Please remove all "field observed steep slopes". Only the steep slopes from the County GIS should be shown. Rev.l: Comment addressed. 3. Show a wash rack on plans with the construction entrance. Ensure that water from wash rack is directed to the sediment trap. Rev.l: Comment addressed. 4. A paved construction entrance is required for this site. Gravel will not be allowed. Please note this on the plans. Rev.l: Comment addressed. Engineering Review Comments Page 3 of 4 5. Show dust control and temporary seeding on ESC Phase 1. It is currently only shown on Phase 2. Rev.1: Comment addressed. 6. Show the stockpile, concrete washout, staging area, etc. on Phase 1 as well as Phase 2. Rev.1: Comment addressed. 7. Please explain why RWD diversion was used in the NW corner of the site. There does not appear to be a road or travel way. 8. It appears the required outlet protection on structure 13 will be offsite. Please obtain/provide a temporary access easement/agreement. Rev.1: Comment addressed. 9. Ensure that grading/construction of retaining wall in the NW corner of the site does not encroach on the steep slopes. Rev 1: Comment addressed, however, please note that if preserved slopes are disturbed during construction, a stop work order will be issued. 10. Please label and darken the contours for the proposed sediment traps. Rev.1: Comment addressed. 11. Show OP symbol at structure 13 Out. Rev.1: Comment addressed. 12. The landscape plan shows proposed trees outside of the limits of disturbance and in the steep slopes in the NW corner. Please remove. Rev.1: Comment addressed. The VSMP permit application and all plans may be resubmitted for approval when all comments have been satisfactorily addressed. For re -submittals please provide 2 copies of the complete permit package with a completed application form. Engineering plan review staff are available from 2-4 PM on Thursdays, should you require a meeting to discuss this review. Process; After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and check parcel and easement information based on the approved plans. The County's Management Analyst will prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash, certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to obtain all the correct signatures and forms. Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The County's Management Analyst or other staff will prepare the forms and check for ownership and signature information. The completed forms will need to be submitted along with court recording fees. After bonding and agreements are complete, county staff will need to enter project information in a DEQ database for state application processing. DEQ will review the application information based on local VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest processing, this is done electronically with the emails provided on the application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter. This should be copied to the county. After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference. Applicants will need to complete the request for a pre -construction conference form, and pay the remainder of the application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction Engineering Review Comments Page 4 of 4 conference will be scheduled with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that work may begin.
