[ttomlin@scsengineers.com] Owner or rep.: Rivanna Solid Waste Authority- Philip McKalips [pmckalips@rivanna.org] Plan received date: 17 April 2017 Date of comments: 27 May 2017 Reviewers: Geoff Elsie — VESCP David James — VSMP County Code section 17-410 and Virginia Code §62.1-44.15:34 requires the VSMP authority to act on any VSMP permit by issuing a project approval or denial. This project is denied. The rationale is given in the comments below. The application may be resubmitted for approval if all of the items below are satisfactorily addressed. The VSMP application content requirements can be found in County Code section 17-401. A. Stormwater Pollution Prevention Plan (SWPPP) The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain (1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary. 1. Registration Statement — Correct the Longitude to be a negative number (-78.654). 2. Provide copy of General Permit when obtained and insert the permit into the SWPPP at the jobsite. B. Pollution Prevention Plan (PPP) The PPP content requirements can be found in County Code section 17-404. 1. Adequately addressed. C. Stormwater Management Plan (SWMP) VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a SWMP. This plan is disapproved, and the reasons are provided in the comments below. The stormwater management plan content requirements can be found in County Code section 17-403. 1. Change the submittal title to VSMP Plan and include WP0201700036 number on the cover sheet. 2. Remove "approval" signature block from cover sheet. Engineering is the only department that reviews/approves the VSMP plans. 3. Remove sheets 4, 8-10, and 21-22 from the VSMP plan. 4. Include note: Safety slabs (SL -1) shall be provided in any structure taller than 12' (i.e. STR Al & A2). 5. Include note: Concrete inlet shaping (IS -1) shall be specified on any structure with a 4' or greater drop. Engineering Review Comments Page 2 of 3 6. Sheet SW1/SW3 - Pipes P8 & P10 from Drainage Summary Table do not match the values (lengths & slopes) shown in profile (please correct/revise). 7. Sheet SW3 - Pipe slopes are missing on a few profiles (detail 10 and 7). 8. Sheet SW2 -I count five (5) `new' channels, four (4) are shown. Two are labeled "A21". Provide correct callout and details. Show the drainage channel details for roadside ditch leading to Str A17. Show channel profiles/cross-sections and revise the drainage calc. packet. 9. Sheet SW2 - Impervious cover calculates to be 29% of total drainage area, not 25%. 10. How is stormwater being directed to the points shown at sand filters C & D? 11. Provide the plan name & number for the Joyce Engineering plan dated 1998. Show the drainage area for the existing basin. It is not shown on drawing G2. 12. Provide documentation and certification that the "zero discharge pond" does not discharge to confirm adequate channel requirements have been met. Show that channel/flood protection requirements are met under Part II B (9VAC25-870-66). 13. Appendix K: Show/include that the basin will not discharge during 1 -yr 24 -hr storm. 14. VRRM spreadsheet indicates there is a manufactured treatment device. Please identify where this device is located and what it is specifically. Is there a detail for this divide? 15. Provide note that all sand filters will have underdrains. 16. Provide maintenance requirements for all stormwater facilities on the plans. 17. Contact Ana Kilmer for SWM agreement. 18. A ESC/SWM bond will be required. D. Erosion and Sediment Control Plan (ESCP) Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is disapproved, and the reasons are provided in the comments below. The erosion control plan content requirements can be found in County Code section 17-402. 1. Provide Deed Book and Page Number for all existing easements. 2. Provide washwater Trapping Basin at Construction Entrance - Sheet ES -2. 3. Provide Stormwater Conveyance Channel identified on the western side of site near Zero Discharge Basin (Sheet ES -2). 4. Silt Fence notation identified along contour line 740, remove or place closer to proposed Silt Fence (Sheet ES -2). 5. Provide detail for Stormwater Conveyance Channels, provide details of design per Standards and Specifications 3.17 found in Virginia Erosion and Sediment Control Handbook. 6. Provide Rip/Rap outlet detail at the Zero Discharge Basin. Show stone sizing (Sheet ES -3). 7. Provide Basin/Trap details, specs and calculations (Sheet ES -3). 8. Stormwater Conveyance Channels need to be shown and noted (Sheet ES -3). 9. Stockpile area needs Temporary Seed notation (Sheet ES -3). 10. Show Material Laydown Area with any additional controls on Sheets ES -2 and ES -3. 11. Trench Drains are shown on Sheet ES -3. Remove from Erosion and Sediment Control Plan if they are to be installed during Stormwater phase. If installed during Phase 2 of the Erosion and Sediment Control Plan, provide details and ESC protection. 12. Provide Outlet Protection for discharges from proposed Sand Filter location D to proposed EW -1. 13. Provide Outlet Protection for discharges from proposed Sand Filter location B to proposed EW -1. 14. Provide Outlet Protection for discharges from proposed forebay into proposed Sand Filter A. Discharge is from the 24" RCP. 15. ES2 - In regards to Note #9, Provide newer topo. Use latest county topo from GIS. The VSMP permit application and all plans may be resubmitted for approval when all comments have been satisfactorily addressed. For re -submittals please provide 2 copies of the complete permit package Engineering Review Comments Page 3 of 3 with a completed application form. Engineering plan review staff are available from 2-4 PM on Thursdays, should you require a meeting to discuss this review. Process; After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and check parcel and easement information based on the approved plans. The County's Management Analyst will prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash, certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to obtain all the correct signatures and forms. Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The County's Management Analyst or other staff will prepare the forms and check for ownership and signature information. The completed forms will need to be submitted along with court recording fees. After bonding and agreements are complete, county staff will need to enter project information in a DEQ database for state application processing. DEQ will review the application information based on local VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest processing, this is done electronically with the emails provided on the application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter. This should be copied to the county. After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference. Applicants will need to complete the request for a pre -construction conference form, and pay the remainder of the application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that work may begin.
