[14-302 (A)(3)] Please provide the deed book and page number in the State Route 726 label. 14-302 (A)(11)] The deed book referenced as the instrument that created the property does not appear to be correct. Please state the deed book and page number for the most recent plat associated with TMP 13600-00-00-02800. 3. [14-302 (A)(15)] Please state the name and addresses of each owner of record and holders of any easements affecting the property. 4. [14-302 (113)(6)] The TMP numbers shown on Sheet 2 appear to be incorrect for the subject property and adjacent parcels. The TMP shows up as 13600-00-00-02800 in Albemarle County GIS. The plat states the TMP as "136-A-28." Please clarify why the TMP has an "A" for TMPs 136-28, 130-42D, and 130-42A. 5. [14-303 (A)] Please add "and new Lot A" after "tax map 136 parcel 28" in the consent to division on Sheet 1. 6. [14-303 (0)] Please add a signature panel for the agent to the Sheet 1. 7. [14-308.1 and 17-1002] The Tier 2 groundwater assessment has been initiated. A copy of the report will be forwarded to the applicant when completed. 8. [14-309 and 14-310] Staff has contacted the Virginia Department of Health (VDH) regarding the notes on Sheet 1 which state that a soils evaluation was conducted by Roger Nelson, AOSE #101, and submitted/approved by VDH for on -site sewage systems. Additional comments regarding soil evaluations or other information required by VDH may be forthcoming. 9. [14-316] A copy of the plat has been forwarded to VDOT for approval of entrances onto public streets. Additional comments from VDOT may be forthcoming. 10. [14-414] The property lies in the Albemarle County Service Authority (ACSA) jurisdictional area for water to existing structures only. A copy of the plat has been forwarded to ACSA for review.
