Based on my review and recent site inspections I can confirm that the facility was constructed in general conformance with the approved plan. The only change observed was the field adjustment of a proposed circular orifice into a square cut. We analyzed this change against the original design and found that it met all of the original design requirements and will control outflows from the basin as intended. All trees shown on the plan were accounted for and to the best of our knowledge all trees are alive at this time. We have taken core samples of the bio-filter and confirmed that the depth of material is also as required on the plans. The site was monitored over the winter and we observed the draw down of several runoff producing storm events and observed that the bio-filter drained down within the required timeframe. Please let me know if any additional information is required from your office for completion of the bond release. Best Regards, ,,,,, ,.,t , sem°. .11P ' 0 J ,S I'M S I-ITMP 3 `_; ic. Justin Shimp, P.E. 1 Short Inspection Comments Report for: WPO201200087 SubApplication Type: ALBEMARLE HEALTH & REHABILITATION CENTER Erosion and Sediment Control Plan No Sub-Application Types for Selected Application Type Inspection Date:11/20/2014 Inspector:Jeremy Glode Regular Inspection Inspection Status:Compliant Inspection Comments: 1. SF okay 2. Most diversions okay. Contractor notified the one primary DD needs to be fixed by weekend; this is at top of CE that diverts site runoff into basin. It was taken out to put in curb (which is now drying) 3. Inlet protection needs to be installed at bottom of CE and at repaired DD inlet. 4. N/A 5. Basin looking good 6. Slopes around perimeter stabilized w/straw/seed - areas will need to be touched up 7. CE mediocre, street is getting dusty - contractor warned 8. Contractor staying w/in limits. Inspection Type: Inspection Date:11/21/2014 Inspector:Jeremy Glode Regular Inspection Inspection Status:Verbal Warning Inspection Comments: Repair of the diversion at top of CE not completed. I warned contractor to put in by the weekend. The road is beginning to get dirty, requested a cleanup. And told contractor that the CE will need repair after weekend rains. Inspection Type: Inspection Date:12/29/2014 Inspector:Jeremy Glode Regular Inspection Inspection Status:Compliant Inspection Comments: Overall okay: 1) SF okay; 2) DDs working, DD at top of CE will required continued maintenance; 3) IP functional; 4) N/A; 5) Basin functioning well; 6) perimeter of site stabilized; 7) CE needs maintenance, asphalt was supposed to be put down but has not. 8) Limits good. Inspection Type: Inspection Date:01/21/2015 Inspector:Jeremy Glode Regular Inspection Inspection Status:Compliant Inspection Comments: Asphalt laid on lower CE - will eventually need an extension up the hill, but this is a good first start. Aggregate CE extended all the way to back of site. SAF added to top of back slope (above area of future retaining wall). RWD needs maintenance. Slope stabilization on right of CE (was regraded). Gas tanks contained. Concrete washout will especially be needed once brickwork begins. Inspection Type: Inspection Date:01/26/2015 Inspector:Jeremy Glode Regular Inspection Inspection Status:Compliant Inspection Comments: CE is good. Mulch is needed on left side of CE around where a new DI is going. RWD needs continued maintenance. Inspection Type: Inspection Date:02/03/2015 Inspector:Jeremy Glode Regular Inspection Inspection Status:Compliant Inspection Comments: CE and RWD needs maintenance. A couple of DIs are getting rebuilt and will need protection. Also a lot of trash is blowing around onsite (basin and outfall area), and needs cleanup. Inspection Type: Inspection Date:02/11/2015 Inspector:Jeremy Glode Regular Inspection Inspection Status:Compliant Inspection Comments: Repaired DI installed - DI protection needed. RWD needs maintenance, SF near CE DI. Mulch/seed added to perimeter slopes, will need to be watched. Trash has been picked up. Sewer being installed in upper CE; CE will need repair after installation. Inspection Type: Inspection Date:06/11/2015 Inspector:Jeremy Glode Regular Inspection Inspection Status:Notice to Comply Inspection Comments: Issued NTC: South side of project - A curb inlet has been removed and is unprotected. Two DIP have been installed incorrectly and the drain is very dirty. Concrete dumped onsite w/out use of a concrete washout. RWD is not installd correctly; we went over how to fix. Dirt pile near the RWD @ entrance needs to be removed. Do maintenance on curb inlet @ bottom of CE. Inspection Type: Inspection Date:07/16/2015 Inspector:Jeremy Glode Regular Inspection Inspection Status:Verbal Warning Inspection Comments: RWD needs maintenance. DIP needs to be re-trenched. concrete trash needs to be picked up around the site. A DD or SF is needed on south side of project b/w curb inlet and retaining wall. Concrete washout and gas tank hold needs to be repaired. Inspection Type: Inspection Date:07/29/2015 Inspector:Jeremy Glode Regular Inspection Inspection Status:Verbal Warning Inspection Comments: Site looking okay; lower curb inlet cleaned out, RWD shaped up. Cleanout and re-install concrete washout. 2. Repair inlet protection around site. 3. The diversion across the construction entrance (leading into inlet and basin), needs to be fixed so runoff doesn’t’ go down the road instead of into the inlet. Runoff currently is bypassing the inlet because of the way it is constructed. 4. Add stone to roadway. 5. Add silt fence to stockpile area above the basin. 6. The turnout area on the construction entrance (where a container is parked below the basin), needs stabilization. Albemarle Health Care Center LLC 11 June 2013 3 July 2013 Max Greene PAWO ►.11► IZIZIJ:m [Justin @ shimp- engineering.com] [fax Unknown] The Water Protection Plans (WP0201200087) submitted 11 June 2013 have received Engineering Review and do not appear to meet Albemarle County minimum checklist items for approval. Please adequately address the following comments for final approval: A) Stormwater Management and Mitigation Plan 1) Pipe 9131 is shown as both an outlet and an inlet. Please show flow direction arrow so inspector and contractor know the pipe is an inlet and not an outlet. An outlet would short circuit the stormwater system. 2) Please explain the purpose of the second outlet structure from the sediment basin or remove it. 3) A stormwater maintenance agreement will be required. Information can be found on the County Web Site. 4) Please request a Thursday meeting to explain how the SWM basin meets or exceeds State Water Protection requirements as outlined in VSMH and Albemarle County Design Standards Manual. Plan/calculation does not appear to capture /protect the entire site. B) Erosion Control Plan 1) Narrative E &SC notes are missing the first note: "The plan approving authority must be notified one week prior to the pre - construction conference, one week prior to the commencement of land disturbing activity, and one week prior to the final inspection." 2) Please correct/update the Stormwater Management portion of the Project description in the narrative. 3) Plan appears to need inlet protections along entrance road with Mill Creek Drive. 4) Please show the "Albemarle County Paved Wash -rack Detail" on the plans. 5) Diversions have not been working properly on existing sites. Please deign the diversions for the 10 -year storm event and with self - cleaning velocities. 6) Diversion from ST #2 to inlet protection along entrance road doesn't appear to work as shown. Please show how the water will enter the inlet without bypassing the structure. 7) Please submit temporary off -site grading easements or letters of intent from adjacent parcel owners that will or might be disturbed or trespassed on for construction. 8) Please show Safety Fence with warning signs per VESCH around sediment control structures due to the proximity of children in the area. 9) Temporary construction road entrance appears to be for one -way traffic. Will there be flagmen to direct traffic or is the other Temporary Construction Entrance to be utilized as an exit? Engineering Review Comments Page 2 of 2 10) Please show a RWD Std. & Spec. 3.11, VESCH where vehicles are to cross stormwater flows. 11) Please provide MS -19 information for outlet of sediment basin. 12) Please note on the plans in the locations of internal perimeter sediment control structures "Sediment control structures will not be removed without the written permission of the County WPO Inspector ". 13) Plan appears to show sediment trap #1 will not allow for the construction of a portion of the building until after the drainage area is fully stabilized and the WPO inspector has released written documentation for removal. This office recommends moving the perimeter control structure ST -1 out of the final fill area to allow for the construction of that portion of the building and parking area. 14) ST -2 appears to be in conflict with the installation of STR -2. STR -2 appears to have an invert @ 10' below the ST -2. Please move ST -2 so the Stormwater System may be installed prior to final site stabilization. 15) Additional sediment trap may be required on the northern boundary if the Diversion is not installed properly or fails across the Swale area. This department recommends showing a sediment trap off -site in the swale to protect the development. 16) STR -9171 does not appear necessary. Please route off -site and pervious areas from above the site into existing swale on County property TMP 91 -13. 17) Please show E &SC protection for the off -site 24" CMP Culvert on TMP 91 -12B. 18) Please show the grading that will be required for the new inlet and outlet of the 24" CMP culvert to replace the existing 6" PVC pipe. 19) A drainage channel maybe required to meet MS -19 requirements from the basin through TMP 91- 12B. 20) Off -site drainage easements may be required for the outfall from the basin to Cow Branch. Due to extent of requested changes for approval, additional comments or conditions may be forthcoming. Once these comments have been addressed, please submit 2 copies of the revised plans, calculations, and narratives to Current Development Engineering along with the required review fee and transmittal form. Current Development Engineering is available from 2:30 -4 PM on Thursdays to discuss these review comments. Please contact Max Greene at 434 - 296 -5832 ext. 3283 or email m ,-reene @albemarle.ora to schedule an appointment.
