Dear Ms. Kilmer,
I have reviewed the construction drawings and performed as-built surveys of the bio-retention facility at
Albemarle Health Care Center. Based on my review and recent site inspections I can confirm that the
facility was constructed in general conformance with the approved plan.The only change observed was the
field adjustment of a proposed circular orifice into a square cut. We analyzed this change against the
original design and found that it met all of the original design requirements and will control outflows from the
basin as intended.All trees shown on the plan were accounted for and to the best of our knowledge all
trees are alive at this time. We have taken core samples of the bio-filter and confirmed that the depth of
material is also as required on the plans.
The site was monitored over the winter and we observed the draw down of several runoff producing storm
events and observed that the bio-filter drained down within the required timeframe.
Please let me know if any additional information is required from your office for completion of the bond
release.

Water Protection Plans submitted 10 December 2013 have received Engineering Review. The plans meet
Albemarle County minimum checklist items for engineering approval. However, the SWM agreement
information has not been submitted for review and approval. The WPO file cannot be released for bonding
and issuance of a grading permit until this item has been adequately addressed. Please contact Mrs. Ana
Kilmer for additional information required to finalize the Storm Water Management Agreement for
recordation.
The County requires a Stormwater ManagementBMP Facilities Maintenance Agreement document
recorded with the "Clerk of Circuit Court".

The Water Protection Plans (WP0201200087) submitted 11 June 2013 have received Engineering
Review and do not appear to meet Albemarle County minimum checklist items for approval.
Please adequately address the following comments for final approval:
A) Stormwater Management and Mitigation Plan
1) Pipe 9131 is shown as both an outlet and an inlet. Please show flow direction arrow so inspector
and contractor know the pipe is an inlet and not an outlet. An outlet would short circuit the
stormwater system.
2) Please explain the purpose of the second outlet structure from the sediment basin or remove it.
3) A stormwater maintenance agreement will be required. Information can be found on the County
Web Site.
4) Please request a Thursday meeting to explain how the SWM basin meets or exceeds State Water
Protection requirements as outlined in VSMH and Albemarle County Design Standards Manual.
Plan/calculation does not appear to capture /protect the entire site.
B) Erosion Control Plan
1) Narrative E &SC notes are missing the first note: "The plan approving authority must be notified
one week prior to the pre - construction conference, one week prior to the commencement of land
disturbing activity, and one week prior to the final inspection."
2) Please correct/update the Stormwater Management portion of the Project description in the
narrative.
3) Plan appears to need inlet protections along entrance road with Mill Creek Drive.
4) Please show the "Albemarle County Paved Wash -rack Detail" on the plans.
5) Diversions have not been working properly on existing sites. Please deign the diversions for the
10 -year storm event and with self - cleaning velocities.
6) Diversion from ST #2 to inlet protection along entrance road doesn't appear to work as shown.
Please show how the water will enter the inlet without bypassing the structure.
7) Please submit temporary off -site grading easements or letters of intent from adjacent parcel owners
that will or might be disturbed or trespassed on for construction.
8) Please show Safety Fence with warning signs per VESCH around sediment control structures due
to the proximity of children in the area.
9) Temporary construction road entrance appears to be for one -way traffic. Will there be flagmen to
direct traffic or is the other Temporary Construction Entrance to be utilized as an exit?

10) Please show a RWD Std. & Spec. 3.11, VESCH where vehicles are to cross stormwater flows.
11) Please provide MS -19 information for outlet of sediment basin.
12) Please note on the plans in the locations of internal perimeter sediment control structures
"Sediment control structures will not be removed without the written permission of the County
WPO Inspector ".
13) Plan appears to show sediment trap #1 will not allow for the construction of a portion of the
building until after the drainage area is fully stabilized and the WPO inspector has released written
documentation for removal. This office recommends moving the perimeter control structure ST -1
out of the final fill area to allow for the construction of that portion of the building and parking
area.
14) ST -2 appears to be in conflict with the installation of STR -2. STR -2 appears to have an invert @
10' below the ST -2. Please move ST -2 so the Stormwater System may be installed prior to final
site stabilization.
15) Additional sediment trap may be required on the northern boundary if the Diversion is not
installed properly or fails across the Swale area. This department recommends showing a sediment
trap off -site in the swale to protect the development.
16) STR -9171 does not appear necessary. Please route off -site and pervious areas from above the site
into existing swale on County property TMP 91 -13.
17) Please show E &SC protection for the off -site 24" CMP Culvert on TMP 91 -12B.
18) Please show the grading that will be required for the new inlet and outlet of the 24" CMP culvert
to replace the existing 6" PVC pipe.
19) A drainage channel maybe required to meet MS -19 requirements from the basin through TMP 91-
12B.
20) Off -site drainage easements may be required for the outfall from the basin to Cow Branch.
Due to extent of requested changes for approval, additional comments or conditions may be
forthcoming.
Once these comments have been addressed, please submit 2 copies of the revised plans, calculations, and
narratives to Current Development Engineering along with the required review fee and transmittal form.
Current Development Engineering is available from 2:30 -4 PM on Thursdays to discuss these review

[17- 204.f] An application for an erosion and sediment control plan that requires modifications, terms, or conditions to be
included in order for it to be approved shall be deemed to be withdrawn if the owner fails to submit a revised plan addressing the
omitted modifications, terms or conditions within six (6) months after the owner is informed of the omitted information as
provided under paragraph (B).
