This project is denied. The rationale is given in the comments below. A. Stormwater Pollution Prevention Plan (SWPPP) The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain (1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary. 1. [Cover Sheet] Provide the WPO Application number (WP0201700039) on the cover sheet and SWPPP document. Rev 2 — Addressed. 2. [Registration statement] As presented, total disturbed area is larger than total land area development. It appears these values were swapped. Please provide corrected registration statement for next submittal. Rev 2 — Addressed. 3. [General] Low maintenance ground cover should be provided on slopes >=3:1. Rev 2 — Addressed. 4. [EP2] Topography must be field verified by designer within the past year. Verify and provide date of verification. Rev 2 — Addressed. 5. [EP3] Identify areas containing managed slopes. Rev 2 — Addressed. 6. Remove landscape plan and sewer profiles from WPO submittal. Rev 2 — Addressed. Rev 2 Additional Comments 1. Please clarify what hatching at north end of site is referencing. Pollution Prevention Plan (PPP) The PPP content requirements can be found in County Code section 17-404. Provide PPP exhibit, or identify on E&S plans, Initial planned locations for stockpiles, concrete v-41-it .i,,-t-_ portable bathroom facilities, etc. Rev 2 — Addressed. .piete section A of the PPP plan outlining pollutant activities and prevention practices. Rev 2 — Addressed. C. Stormwater Management Plan (SWMP) VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a SWMP. This plan is disapproved, and the reasons are provided in the comments below. The stormwater management plan content requirements can be found in County Code section 17-403. 1. [All] Clarify status of "SDG" at the north end of the property connecting to Ex-1. Is this structure to be removed? If so, eliminate the structure from proposed layouts and provide drainage relief for existing depression. If it is to remain, include in stormwater calculations and profiles. Rev 2 — Addressed. 2. [EP5] Side slopes for conveyance channel should be limited to 3: 1. Ensure EC-2 matting is Type lI due to velocities being >4fps. Rev 2 — Acknowledged. 3. [EP 10] Please provide context for stormsewer segments presented in LD-229 storm drain computations. It is unclear what segments this chart is referring to, and where on the overall Hollymeade property they are located. Rev 2 — Not addressed adequately. See comments below. 4. [EP 10] Provide documentation related to compliance with water quality requirements. Water Quality cannot be addressed as described. Rev 2 — Adequately Addressed. 5. [Details] Provide profile and calculations for drainage passing through 8" Duraslot drains. Provide manufacturers specifications and maintenance details. Rev 2 — Acknowledged. Rev 2 Additional Comments 1. Correct the Scaling on sheet EP-7. It is difficult to see stormsewer structures referenced in comment 3 above due to cutoff at bottom of sheet. It is also difficult to see which symbols are associated with each structure/pipe. Some labels are covered by features/lines, some labels are duplicated elsewhere on the sheet (pipe 5 is noted in 2 places). 2. Structure labels still do not match those as presented in the LD-229 spreadsheet (no reference for structure ST-1A). D. Erosion and Sediment Control Plan (ESCP) Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is disapproved, and the reasons are provided in the comments below. The erosion control plan content requirements can be found in County Code section 17-402. 1. [SP4 & SP5] Provide soil boundaries Rev 2 — Addressed. 2. [SP4 & SP5] Provide inlet protection for inlets along the south curb of Laurel Park lane. Rev 2 — Addressed. [SP4 & SP5] Provide perimeter silt fence or indicate temporary seeding for areas adjacent to property boundaries. Rev 2 — Addressed. Rev 2 Additional Comments 1. Revise construction sequence item #7 to reference the appropriate ST for removal. Currently states ST-1 will be removed following consult from County when it should mention ST-2. Since only minor changes are needed, please submit 4 signed copies of the plan set and 1 additional completed copy of the SWPPP for approval. Process; After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and check parcel and easement information based on the approved plans. The County's Management Analyst will prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash, certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to obtain all the correct signatures and forms. After bonding and agreements are complete, county staff will need to enter project information in a DEQ database for state application processing. DEQ will review the application information based on local VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest processing, this is done electronically with the emails provided on the application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter. This should be copied to the county. After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference. Applicants will need to complete the request for a pre -construction conference form, and pay the remainder of the application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that work may begin.
