County Code section 17 -410 and Virginia Code §62.1- 44.15:34 requires the VSMP
authority to act on any VSMP permit by issuing a project approval or denial. This project is
approved. See the process for the bonding procedure below.
After approval, plans will need to be bonded. The bonding process is begun by submitting a bond
estimate request form and fee to the Department of Community Development. One of the plan reviewers
will prepare estimates and check parcel and easement information based on the approved plans. The
County's Management Analyst will prepare bond agreement forms, which will need to be completed by
the owner and submitted along with cash, certificates or sureties for the amounts specified. The
agreements will need to be approved and signed by the County Attorney and County Engineer. This may
take 2 -4 weeks to obtain all the correct signatures and forms.
Stormwater Management Facilities Maintenance agreements will also need to be completed and
recorded. The County's Management Analyst or other staff will prepare the forms and check for
ownership and signature information. The completed forms will need to be submitted along with court
recording fees.
After bonding and agreements are complete, county staff will need to enter project information in a
DEQ database for state application processing. DEQ will review the application information based on
local VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid
directly to the state. For fastest processing, this is done electronically with the emails provided on the
application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the
application, they will issue a permit coverage letter. This should be copied to the county.
After DEQ coverage is issued, via the coverage letter, the County can hold a pre - construction
conference. Applicants will need to complete the request for a pre - construction conference form, and pay
the remainder of the application fee. The form identifies the contractor and responsible land disturber, and
the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre - construction
conference will be scheduled with the County inspector. At the pre - construction conference, should
everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that
work may begin.

necessary.

1) [Comment] Include an 11"x 17" Pollution Prevention Plan map showing
concrete washout, porta johns, fueling stations, and trash containers. This will
be marked up by the contractor as construction of site progresses.

C. Stormwater Management Plan(SWMP)
VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or
disapprove a SWMP. This plan is disapproved, and the reasons are provided in the
comments below. The stormwater management plan content requirements can be found in
County Code section 17-403.
1) [Comment] The title on the plans should state"VSMP".
[Revision 1] Comment addressed.
2) [Comment] The LOD area noted on sheet C-20 is 4.6 acres. Please provide a
note on the title sheet to clarify that this area is covered under an existing
general permit and state the registration number.

3) [Comment] I compared sheet C-19 of Stonefield Block G to the approved
sheet 29 of Stonefield Block F and G. There are stormsewer pipes on Block G
that appear to be existing,but do not match what was proposed on the previous
block F&G plans. The demo sheet clarifies some revisions to the pipe layout.
Please clarify other pipes so it is clear what is already installed, what are
revisions to the previous approved plan under Stonefield Block F & G, and
what are new proposed pipes under this plan. Also, include new profiles for
stormsewers if they changed from the previously approved WPO plans.

prior to the construction of stormsewer pipes proposed with this
application.
4) [Comment] On sheet C-14, under Stormwater Narrative, a note refers to
WPO2010-23. However, another plan was approved under WPO2013-69,
where the North SWM Facility layout and vortechs stormfilter were revised.
WPO2013-69 should be noted in the narrative as well. Also, it should be noted
in this proposal that the vortechs location has been shifted slightly to a new
location. Provide revisions to the stormsewer pipes and the vortechs profiles
since it has changed from the previously approved plans.

supplemental construction exhibit for clarification.There are still some
discrepancies between profiles, plan and calculations.Please address the
following:
tra) Is the top inlet elevation 473.25,472.35 or 473.19 for Ex
188.Revise profile, calc,and plan sheet C8(second line of
spreadsheet).Note that the construction exhibit shows
473.25.
V Response: The elevations shown on Drawing Number C-8 and C-12 have been
revised to 473.25.
b) On C-12,for Ex 188 to 232,revise"upper end" to 463.34.

Revise so they all match.It appears it will 1 change from the
construction exhibit plan.
Response: The correct invert from 233 to Ex 160 is 464.50. The 457.20 invert is
coming from Ex 160.1. The invert from 233 to Ex 160 is not shown on the
construction sketch.
e) On C-12,for 235 to Ex 212 revise"lower end"to 462.86
Response: This has been revised on Drawing No. C-12.

is it already built? It is shown as a thicker lineweight, but it is not shown in
the profiles. Please clarify.

162 is a 30" pipe. The profile for str 162 to 161 should also be 30." Please
revise on plan and profile.

9) [Comment] For the north swm facility please update the tables labeled
Albemarle Place Pre/Post Development Runoff Analysis, Albemarle Place
BMP Computations Summary and Albemarle Place SWM Facilities
Storage Volume. Please include with the proposed plans. This can be
obtained from sheet C-25 WPO2010-23. Update calculations pertaining to
the north SWM facilities.

D. Erosion and Sediment Control Plan(ESCP)
Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an
ESCP. This plan is disapproved, and the reasons are provided in the comments below. The

to WPO-200900074. This should be coordinated with the approved

Response: The plans reflect current field changes that have occurred. All tops have been
noted on this set of plans.

County Code section 17 -410 and Virginia Code §62.1- 44.15:34 requires the VSMP authority to
act on any VSMP permit by issuing a project approval or denial. This project is denied. The
rationale is given in the comments below. The application may be resubmitted for approval if all
of the items below are satisfactorily addressed. The VSMP application content requirements can
be found in County Code section 17 -401.
A. Stormwater Pollution Prevention Plan (SWPPP)
The SWPPP content requirements can be found in County Code section 17 -405. A SWPPP must
contain (1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary.
1) Include the registration statement in the SWPP booklet.

B. Pollution Prevention Plan (PPP)
The PPP content requirements can be found in County Code section 17 -404.
1) Include an 11 "x 17" Pollution Prevention Plan map showing concrete washout, porta johns,
fueling stations, and trash containers. This will be marked up by the contractor as construction of
site progresses.
C. Stormwater Management Plan (SWMP)
VSMP Regulation 9VAC25- 870 -108 requires the VSMP authority to approve or disapprove a
SWMP. This plan is disapproved, and the reasons are provided in the comments below. The
stormwater management plan content requirements can be found in County Code section 17 -403.

2) The LOD area noted on sheet C -20 is 4.6 acres. Please provide a note on the title sheet to clarify
that this area is covered under an existing general permit and state the registration number.
3) I compared sheet C -19 of Stonefield Block G to the approved sheet 29 of Stonefield Block F and
G. There are stormsewer pipes on Block G that appear to be existing, but do not match what was
proposed on the previous block F &G plans. The demo sheet clarifies some revisions to the pipe
layout. Please clarify other pipes so it is clear what is already installed, what are revisions to the
previous approved plan under Stonefield Block F & G, and what are new proposed pipes under
this plan. Also, include new profiles for stormsewers if they changed from the previously approved
WPO plans.

The VSMP permit application and all plans may be resubmitted for approval when all comments have
been satisfactorily addressed. For re- submittals please provide 4 copies of the complete permit package
with a completed application form. All revisions to calculations shall be resubmitted, signed, sealed and
dated.

Process;
After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate
request form and fee to the Department of Community Development. One of the plan reviewers will
prepare estimates and check parcel and easement information based on the approved plans. The County's
Management Analyst will prepare bond agreement forms, which will need to be completed by the owner
and submitted along with cash, certificates or sureties for the amounts specified. The agreements will need
to be approved and signed by the County Attorney and County Engineer. This may take 2 -4 weeks to
obtain all the correct signatures and forms.
Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded.
The County's Management Analyst or other staff will prepare the forms and check for ownership and
signature information. The completed forms will need to be submitted along with court recording fees.
After bonding and agreements are complete, county staff will need to enter project information in a DEQ
database for state application processing. DEQ will review the application information based on local
VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid
directly to the state. For fastest processing, this is done electronically with the emails provided on the
application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the
application, they will issue a permit coverage letter. This should be copied to the county.
After DEQ coverage is issued, via the coverage letter, the County can hold a pre - construction conference.
Applicants will need to complete the request for a pre - construction conference form, and pay the remainder
of the application fee. The form identifies the contractor and responsible land disturber, and the fee
remaining to be paid. This will be checked by county staff, and upon approval, a pre - construction
conference will be scheduled with the County inspector. At the pre - construction conference, should
everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that
work may begin.
