Re: Road Bond reduction request for Carroll Creek Road — SUB201500202
Thank you for the documentation regarding the bond reduction request for Carroll Creek Road. The Road
Bond under SUB-201500202 is currently at the original amount of $425,180. This bond can be reduced to
an amount of $128,810.
The remaining bond amount can be released after addressing the following deficiencies:
As -Built Survey and Bond Inspection Comments
1. As -built drawing does not identify the culvert crossing Carroll Creek Drive east of the
telecom pad. Revise as -built survey to include this culvert. Provide a detail for the
culvert extension. The culvert enters from the south as CMP but exits as HDPE. It is
unclear how these culverts are connected under the road and this is typically not
acceptable. Additionally, provide positive drainage at the outlet to the stormwater
facility.
2. Identify the minimum constructed separation between the asphalt trail and the road edge -
of -pavement. Field measurements indicate separation to be approximately 3.5 ft, not
5-ft as designed. Revise road cross section to reflect as -built separation and provide
certification that this is not a safety issue.
3. The 5-ft shoulder on the north side of the road was not installed with the approved width.
It is approximately 1.5-ft before the 2:1 slope begins. Revise road cross section to
accurately reflect as -built shoulder width and provide certification that the reduced
shoulder width is not a safety issue. Note 13 on Sheet C 1 of the approved plans state
"exceptions will not be allowed."
4. Contours on the as -built survey were measured to be a 2:1 slope. There are areas that
exceed 2:1 slope. Note 6 on Sheet C1 states "The maximum allowable slope is 2:1...".
Correct areas where slopes exceed 2:1.
5. Provide upstream and downstream invert elevations for the box culvert on the as -built
survey. Only one elevation is provided and it isn't clear if this is the elevation at the
inlet, outlet or some other location within the culvert.
6. Confirm box culvert span and height dimensions. Field measurements indicated these
dimensions were not 10%0" as designed. If dimensions aren't as designed, a revised
flood study may be required to confirm backwater elevations.
7. Two water meter valve lids/casings protrude above the asphalt surface near the ACSA

entrance. Lower lids/casings so that they are flush with the road surface to avoid
damage during snow removal.
8. Drainage ditch on the northeast side of Carroll Creek Drive to the stream was not
constructed as designed. Runoff discharges into the wooded area. Install the
conveyance channel.
9. Drainage ditch on the southeast side of Carroll Creek Drive from the ACSA access road to
the lower ditch was not installed. Even though some of the drainage appears to have
been rerouted to the culvert across from the ACSA entrance, signs of erosion are
already noted. Install a conveyance channel to adequately convey drainage to the
lower channel and ultimately to the stream. The outlet of the lower channel to the
stream is heavily eroded and may require a riprap outlet.
10. There is up to a foot of soil retained by silt fence above both headwalls. Correct the
grading so the silt fence does not act as a retaining wall. Final grades should not
exceed 2:1 slope.
11. Wingwall lengths are shorter than designed, which has resulted in grades exceeding 2:1 at
the ends of the wingwalls, where erosion is occurring. Grade the slopes to be less than
2:1 and provide stabilization or extend wingwalls to retain the soil. Note 6 on Sheet
C1 states "The maximum allowable slope is 2:1...". Correct areas where slopes
exceed 2:1.
12. Provide a copy of the DEQ permit for stream impacts, including supporting
documentation.
13. Adequate riprap was not installed at the outlet (there was a hole at the outlet with minimal
riprap over a considerable area). Submit riprap sizing calculations prior to installing
riprap to confirm sizing is correct. Additionally, install the riprap to the limits shown
on the approved plan. Thickness of riprap shall meet VDOT minimum standards.
14. Sediment deposits were observed in front of the wingwalls at both the up- and
downstream ends of the box culvert. Remove sediment from within the stream limits.
15. The shoulder is slightly higher than the edge of pavement on the south side of the road,
which could cause water to pond on the road. Adjust grade of shoulder to ensure
positive drainage away from roadway.
16. Periwinkle groundcover is required on the 2:1 slopes. All areas are not fully stabilized.
Bond cannot be released until 90% stabilization and groundcover has been
established.
17. Correct hatching on as -built survey at the telecom pad. Asphalt hatch is shown over the
grassed island and isn't shown over all of the paved areas.
