C. VSMP: SWPPP: Erosion Control Plan(WPO201500038)
Virginia Code§62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is
approved;prior comments Addressed with prior revisions.
Process:
Project is bonded. Ref.A.Kilmer email:4/4/2016 4:05 PM.
County staff will enter project information in a DEQ database for state application processing. DEQ will review the
application information based on local VSMP authority approval. At this time,the DEQ portion of the application
fees will need to be paid directly to the state. For fastest processing,this is done electronically with the emails
provided on the application. DEQ should notify applicants with instructions on how to pay fees. When DEQ
approves the application,they will issue a permit coverage letter. This should be copied to the county.
After DEQ coverage is issued,via the coverage letter,the County can hold a pre-construction conference. Applicant
will need to request a pre-construction conference by completing a form,and pay the remainder of the application
fee. The form identifies the contractor and responsible land disturber,and the fee remaining to be paid. This will be
checked by county staff,and upon approval,a pre-construction conference will be scheduled with the County
inspector. At the pre-construction conference,should everything proceed satisfactorily,a joint VSMP and grading
permit will be issued by the County so that work may begin.

C. VSMP: SWPPP: Erosion Control Plan (WP0201500038)
Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is

Process:
After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request
form and fee to the Department of Community Development (complete). One of the plan reviewers will prepare
estimates and check parcel and easement information based on the approved plans (complete). The County's
Management Analyst will prepare bond agreement forms, which will need to be completed by the owner and
submitted along with cash, certificates or sureties for the amounts specified (in process). The agreements will need
to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to obtain all the
correct signatures and forms.
Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The
County's Management Analyst or other staff will prepare the forms and check for ownership and signature
information. The completed forms will need to be submitted along with court recording fees.

1. We have included a draft of the SWPPP in our electronic submittal. An
unsigned, completed registration statement has been provided within the
SWPPP. The registration statement will be signed and dated as soon as
approval on all other VSMP/WPO comments is grated.

Process:
After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request
form and fee to the Department of Community Development (complete). One of the plan reviewers will prepare
estimates and check parcel and easement information based on the approved plans (complete). The County's
Management Analyst will prepare bond agreement forms, which will need to be completed by the owner and
submitted along with cash, certificates or sureties for the amounts specified (in process). The agreements will need
to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to obtain all the
correct signatures and forms.
Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The
County's Management Analyst or other staff will prepare the forms and check for ownership and signature
information. The completed forms will need to be submitted along with court recording fees.
After bonding and agreements are complete, county staff will need to enter project information in a DEQ database
for state application processing. DEQ will review the application information based on local VSMP authority
approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest
processing, this is done electronically with the emails provided on the application. DEQ should notify applicants
with instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter.
This should be copied to the county.
After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference. Applicants
will need to request a pre -construction conference by completing a form, and pay the remainder of the application
fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be
checked by county staff, and upon approval, a pre -construction conference will be scheduled with the County
inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint VSMP and grading
permit will be issued by the County so that work may begin.

2. There is a note on sheet C4,0 indicating that structure 100 will be tied to the existing
pipe. I understand that the 18" RCP under Shepherd's Ridge Road will be replaced with
15" HDPE, however, the note on sheet C4.0 could be interpreted that the structure will be
connected to the existing 18" RCP. It appears that this note should be removed.
This note has been removed from the plan.
3. The label for structure 214 is overlaying the spot elevation label. For clarity, one of the
labels should be shifted.

4. The hydraulic calculations have not been provided to me for review.

disapproved for reasons provided in comments below. The stormwater management plan content requirements can be
found in County Code section 17 -403

/system #2) would be 464.3'. Likewise, system #1 bottom elevation near point str. #105 enters the system
is 27'± X0.005, or 0.135'+ higher than 451.50' (INV out /Str. #103), or 451.64'±. Care should betaken in
labeling profile elevations. For example, spatial relationships shown in STR112- STR106 profile do not

(profile views) to indicate pipes continue beyond solid lines that show 96" DIA pipes ending in certain

label would be helpful since correlation is difficult. Also, 0.80 Ac. value affects water quality 2.89 lb /yr
phosphorus load reduction. This acreage must be conserved. Provide preservation/conservation easements

Increase distance to grade -level improvements if possible. Provide Private Drainage Easements on Lots 9
and 19 based on trench side -slope required to excavate or replace storage systems in the future. (Rev. 1)

E. Documentation of the applicant's acquisition of nutrient credits shall be provided to the VSMP authority and
the Department in a certification from the credit provider documenting the number of phosphorus nutrient
credits acquired and the associated ratio of nitrogen nutrient credits at the credit - generating entity. Until the
effective date of regulations establishing application fees in accordance with § 62.1- 44.19:20, the credit
provider shall pay the Department a water quality enhancement fee equal to six percent of the amount paid by
the applicant for the credits. Such fee shall be deposited into the Virginia Stormwater Management Fund
established by § 62.1 - 44.15:29.
C. VSMP: SWPPP: Erosion Control Plan (WPO201500038) -A single comment remains
Virginia Code §62.1- 44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is disapproved
for reasons provided in comments below. The erosion control plan content requirements can be found in County Code
section 17 -402.
1. Revise top of dam elevation, ST #3. (Rev. 1) Addressed.
2. Phase 1 ESC Plan, C3.2 shows installation of perimeter controls and traps, but not rough grading while
phase 2 appears controlled with SF only. Although sequence makes clear that rough grading occurs with
phase 1, please show rough grade (contours) upslope of diversion dikes /traps on C3.2. (Rev. 1) Addressed.
3. Revise sequence of installation, phase 1, note 1: Pre - construction to take place at county office building.
(Rev. 1) Addressed.
4. C3.0 - Provide Paved Wash Rack detail -ref. ACDSM. /pg. -8. (Rev. 1) Addressed.
5. Transfer /copy bottom trap dimensions from design tables, C3.1 to C3.2, to aid construction and inspection.

Process:
After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request
form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and

check parcel and easement information based on the approved plans. The County's Management Analyst will
prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash,
certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the County
Attorney and County Engineer. This may take 2 -4 weeks to obtain all the correct signatures and forms.
Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The
County's Management Analyst or other staff will prepare the forms and check for ownership and signature
information. The completed forms will need to be submitted along with court recording fees.
After bonding and agreements are complete, county staff will need to enter project information in a DEQ database
for state application processing. DEQ will review the application information based on local VSMP authority
approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest
processing, this is done electronically with the emails provided on the application. DEQ should notify applicants
with instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter.
This should be copied to the county.
After DEQ coverage is issued, via the coverage letter, the County can hold a pre - construction conference. Applicants
will need to request a pre - construction conference by completing a form, and pay the remainder of the application
fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be
checked by county staff, and upon approval, a pre - construction conference will be scheduled with the County
inspector. At the pre - construction conference, should everything proceed satisfactorily, a joint VSMP and grading
permit will be issued by the County so that work may begin.

B. VSMP: SWPPP: Stormwater Management Plan (WPO201500038)
VSMP Regulation 9VAC25- 870 -108 requires the VSMP authority to approve or disapprove
a SWMP. This plan is disapproved for reasons provided in comments below. The
stormwater management plan content requirements can be found in County Code section
17 -403.
1. Str. 206 appears to be a MH (C5.1) with 15" DIA pipe upstream, 24" DIA pipe
downstream (pipe storage I — pipe storage 2 profile; C7.0), but weir plate in Str. 206 detail
(C6.1) shows weir plate within 72" storage system # 1. Coordinate design across plan
sheets. Note: other weir plates are within storage systems.
Storage systems and weir plate details have been revised.
2. C6.1 — All weir plates: Although details are NTS, approximate scale is helpful. Recommend
adjust NTS 10" orifice to closely approximate 10 ", relative to 6' pipe. Adjust leader line
arrow to point to top of weir plate in Str. 104 detail.
The weir plate details are still not to scale but they have been revised to
accurately reflect the design as shown in the attached calculation booklets.

0.80 Ac. Value affects water quality 2.89 lb/year phosphorus load reduction. This acreage
must be conserved. Provide preservation /conservation easements for 0.80 Ac. On Sheet

A preserved woods and open space exhibit has been added to sheet C0.0 and
includes area callouts for each section of preserved woods.

18. C4.0 — With request to 96' cul -de -sac, storage system at Int. Road A/B appears
problematic. VDOT comment on facilities within public RW is relevant. VDOT may
prohibit SWM facilities within RW. Also, schematic (plat) plan view shows all three
stormwater storage systems virtually touching sidewalks. Increase distance to grade -level
improvements if possible. Provide Private Drainage Easements on Lots 9 and 19 based on
trench side -slope required to excavate or replace storage systems in the future.
The unpaved island has been removed from the cul -de -sac, and it now has a
radius of 30'. The revised storage systems no longer encroach into this area or
into VDOT right -of -way.
19. Remove Sheets: C 1. 1, C 1.2, C 1.3 (or delete all except Typ. F &C + Typ. Storm Sewer MH

ST #3 dam elevation has been revised.
2. Phase I ESC Plan, C3.2 shows installation of perimeter controls and traps, but not rough
grading while phase 2 appears controlled with SF only. Although sequence makes clear
that rough grading occurs with phase 1, please show rough grade (contours) upslope of
diversion dikes /traps on C3.2.
Phase "IA," sheet C3.1, has been added to the plan which includes intermediate
grading.
