Approval by the Attorney's office and receipt of the signed and notarized version of the final document, is necessary before approval of the subdivision. Rev. 92: The Attorney's office requested two changes be made on the private street maintenance agreement. a. The first than a is in the third paraaraph, where the information on the Plat is elven. The date of the plat should read "April 22, _2,016. last revised June 9.2016.". b. The second change is at the bottom of the first Pane in the last sentence where it is describina the two lots. "New Acrea e" is not the name for the parcel. I would suggest a description of "the residue of TMP 103-57". If there is a different wording our attorne would prefer to use please contact me before moving forward. c. After the two chancres listed above are made to the private maintenance agreement then submit a signed and notarized version of the document. d. Once we have received the signed and notarized document then the County will be able to approve and slan the plat. Also add the word "County" to the stream buffer note (ex. "...the Albemarle County Water Protection Ordinance." 14. [14-303 (C)] Boundary. Move the 65.64' distance measurement in the upper right corner of the plat (southeast corner) to the other side of the line so that it is more obvious that the 565.28' is measuring the distance all of the way to the P.F. label. 15. [14-303 (D)] Acreage of lots. Update the new acreage for TMP 103-65. The 0.74 acres was accidentally subtracted instead of added to the new acreage total. 16. [14-303 (E)] Dimension standards and information on all lots, streets, alleys, easements, and shared driveways. Label the" New 30' Access Easement" as "New 30' Private Street Access Easement". Also, see comments numbers 3 and 4 above for additional comments on dimension standards and easement. 17 [14-303 (H)] Monuments. Clarify why the existing southern property corner along State Route 795 (Rolling Road) is labeled as I.S. instead of being found". The plat included in deed book 4645 page 213 dated 6/5/2015 showed the pin as being set. Also, the property corner near the middle of the road frontage does not seem to have a label. Specify if this monument was "found" or "set". 18. [14-303 (L)] Public utility, drainage and sight distance easements. See Comments #3 & #4 above. In addition to those comments provide any drainage or site distance easements that exist or will be required. 19. [14-303 (0)] Signature Panels. If additional owners are added, as requested in Comment #9 above, add an owner's signature panel for that owner. 20. [14-303 (P)] Notary Panels. Provide a notary panel for each owner who is listed. Ensure that there is an area in the notary panel which specifies which owner's signature they are witnessing. 21. [14-308.1] Groundwater Assessment. A Tier 2 groundwater assessment is required for a parcel of this size. Submit an application and the fee for a Tier 2 groundwater assessment. 22. [14-309; 14-310] Soils evaluation; Health Director Approval. Soils work was submitted to Health Department on May 4, 2016. Approval of the soil evaluation by the Health Department is required before approval the subdivision will be final. 23. [14-316] Approval of entrance onto public streets. Provide verification from VDOT that there is adequate location for an entrance along the adjacent public roads for the shared private street access for the two lots. 24. [14-404] Lot location to allow access from lot onto street or shared driveway. The proposed 30' access easement will be a single point of access for both lots. This portion of Rolling Road (Route 795) is designated as a Major Rural Street by Albemarle County's Subdivision Ordinance. Since this section or Rolling Road is a major rural street, and TMP 103-57's road frontage is less than 500', Section 14-404 of the subdivision ordinance specifying a single point of access applies. This stipulates a single point of access to the public road, for the two parcels created with this subdivision, is required unless a waiver is granted. 25. [Comment] A "private street" is required for Lot 1's access through the residue of TMP 103-57. A private street may be approved by a County agent, and does not require approval of the Board of Supervisors, in a 2 -Lot subdivision if all regulations, standards and procedures are followed. See Article Il, Division 5 of the Subdivision Ordinance on Procedures for the approval of a private street, shared driveways and alleys (in its entirety but also specifically 14-232 (B), 14- 234 (A) (B) (C) & (E), & 14-235) as well as 14-410, 14412 (A) (1) and any Subdivision Ordinance or Zoning Ordinance items referenced within them. 26.
