RE: SDP201700059 Charlottesville Orthopedic Center - Minor Site Plan Amendment
Dear Sir:
Your Minor Amendment application has been reviewed. In order for the amended site plan to be
approved the following revisions are required:
1. [32.5.2.1(c), 32.5.2(i), 32.5.2(o)] Label Seminole Lane on the plan as public right-of-way. Per
County records Seminole Lane is proposed to be a Public Road; however, at this time it is not
accepted into the State's Secondary System.
2. [32.5.2(n), 32.5.2.1(c), 32.5.2.2(a), 21.7(c)] Buffer zone adjacent to residential and rural areas
districts. "No construction activity including grading or clearing of vegetation shall occur closer
than twenty (20) feet to any residential or rural areas district... " On the plan assure the 20'
buffer is depicted and labeled. Currently the "second floor deck (by others)" is depicted within
this buffer. Once revised to be outside of the 20' buffer area if portions of it remain, assure the SF
of the disturbance to install the improvement is included in the disturbance area of the site.
3. [32.7.91 Landscaping. Revise the plant list to provide caliper and height for all shrubs. The
minimum caliper required is 1.5" for Pieris Floribunda, 1" for Buddleia Davidii, and 1" for
Callicarpa Dichotoma. All shrubs utilized as screening shall be a min of 18" tall at the time of
planting.
4. [32.7.91 Landscaping. Provide the minimum height at planting of all trees. Red Maple shall be
10' tall and English Oak shall be 11' tall.
5. [32.7.91 Landscaping. Revise the canopy calculations for all plantings and assure they meet the
canopy calculations provided in the Albemarle County Approved Plant Canopy Calculations
document - LINK Also, please combine the two plant calculations charts, so staff can easily
check which canopy calculations go with which plantings.
6. [32.5.2(n)] Existing and Proposed Utilities. On the landscape plan depict, label, and dimension
all utilities/ utility easements. Provide all existing easements with deed book page reference
numbers.
7. [32.5.2(d)] Topography. On the plan clearly label and depict all slopes as "managed slopes".
8. [32.5.2(a)] General Information. Label all adjacent properties with current owner names, zoning
district, and present uses. The last sheet in the site plan attempts to do this; however, much of the
information is out of date.
9. [32.5.2(n)] Existing and Proposed Improvements. Revise the last sheet in the site plan to either
omit the use label or accurately label the building use, as "1 story automobile showroom" is not
being proposed.
10. [32.5.2(n), 4.12.18(a)] Existing and Proposed Improvements. Is the covered wash area being
removed? If so, revise sheet C2.0 (5 of 12) to accurately depict the proposal. If it is to remain,
provide a statement that lists the clearance height.
11. [4.12.18(c)] Designation of Loading Space. Will the 60' parallel loading area suffice for delivery
vehicles entering and exiting the site when the parking spaces adjacent to the loading area are
occupied by customer vehicles? If not, revise. On the plans, provide a cutsheet/diagram of a
typical delivery vehicle and its turning radius.
12. [4.12.16(c)] Minimum Design of Parking Spaces. Parking spaces adjacent to 22' wide travelways
shall be 10' wide and 18' long. Currently the spaces fronting Seminole Lane do not meet this
requirement. Revise.
13. [4.12.16(c)] Handicapped Parking Spaces. Van access aisle widths are required to be 8' wide.
Currently the plans depict the access as 6' wide. Revise.
14. [Comment] On sheet 1 provide the site plan number: "SDP2017-00059 Minor Site Plan
Amendment"
15. [Comment] ACSA approval is required prior to County approval of the plan.
16. [32.7.2.11 Vehicular Access to Site. Each entrance onto any public street shall be designed and
constructed as required by the standards of the Virginia Department of Transportation. VDOT
approval of the entrance to the site shall be required prior to final site plan approval.
17. [32.6.2(k) & 4.171 Lighting. Is any new outdoor lighting proposed with the site plan amendment?
If so, a full lighting (including building mounted fixtures) must be depicted. Please include a
photometric plan for these fixtures, luminaire schedule, and cut sheets for each proposed fixture.
ARB - Margaret Malinewski
On November 20, 2017, the ARB approved this proposal with a number of conditions to be handled
by staff, as follows.
1. Revise the elevation drawings to include a material/color schedule keyed to the elevations.
2. Provide paint samples for the two shades of grey that are proposed.
3. Reconsider the proposed colors to be more in keeping with the originally proposed color palette.
4. Clarify on the drawings where the charcoal grey metal is proposed to be located.
5. Provide the VLT and VLR of the proposed window glass. Add the standard window glass note to
the architectural drawings: Window glass in the Entrance Corridors should meet the following
criteria: Visible light transmittance (VLT) shall not drop below 40%. Visible light reflectance (VLR)
shall not exceed 30%. Specifications on the proposed window glass should be submitted with the
application for final review.
6. Revise the plan to show the location of mechanical equipment. Show that visibility of the
equipment will be eliminated.
7. Add the standard mechanical equipment note to both the site and architectural drawings: Visibility
of all mechanical equipment from the Entrance Corridor shall be eliminated.
8. Revise the plan to include a dumpster enclosure detail. Show that the enclosure is compatible with
the building design.
9. Revise the plans to clearly indicate all light poles to be demolished, relocated and/or retained.
Show all proposed lights. Provide a photometric plan. Show that all new and relocated fixtures meet
ordinance requirements. Shift lights and poles to allow for complete rows of shrubs along the parking
rows. Clarify and coordinate the notes regarding relocated light poles and relocated power poles.
10. Increase the planting size of the maples to 3%2" caliper.
11. Show all utilities and their associated easements on the landscape plan. Resolve utility/landscape
conflicts without reducing the quantity of landscaping proposed.
12. Revise the spacing of the Kousa Dogwoods to 20' apart.
13. Add shrubs along the north and south parking rows.
14. Revise the plant schedule to show shrub planting size as 24" high.
15. Add the plant health note to the landscape plan: All site plantings of trees and shrubs shall be
allowed to reach, and be maintained at, mature height; the topping of trees is prohibited. Shrubs and
trees shall be pruned minimally and only to support the overall health of the plant.
16. Limit the number of proposed plants for any one species to a maximum of 25% of the total
proposed for that plant type.
17. Clarify the reason for the off -site shrubs. Provide evidence of an off -site planting easement.
18. Revise the site plan to show the sidewalk against the back of the curb to allow people to enter the
building more easily.
19. Consider alternative vegetation along the west elevation that better fits the available planting
area.
Fire and Rescue — Shawn Maddox
1. A note must be shown on the plans that indicate a Knox Box will be installed with the location
approved by the fire marshal's office.
2. If the building is to become sprinklered a fire department connection must be shown on the plans
within 100' of the existing hydrant.
3. A fire flow test will be required prior to final acceptance. Once building construction type is
provided a fire flow calculation can be provided.
Engineering — Bobby Jocz
Comments on plans dated 1019117
1. [General] Provide date and source of topographic data. Ensure topography has been field verified
by designer/engineer within the past year (18-23.6.2).
2. [Sheet 1.0] Identify limits of critical/managed/preserved slopes located on the site (18-32.5.2).
3. [Sheet 1.0] Ensure all demolition activates are included in LOD (Patchwork can continue to be
excluded). There appears to be asphalt removal activities located outside the LOD at the east corner
of the existing building.
4. [Sheet 1.0] Show all existing on site easements with deed book and page references, locations,
dimensions (18-23.6.2).
5. [Sheet 1.0] Provide silt fence along the southwest portion of the site adjacent to the parking lot
areas to be disturbed.
6. [Sheet 1.1] Provide paved construction entrance detail.
7. [Sheet 1.1] Provide safety fence detail.
8. [Sheet 2.0] Please clarify curb type/detail for the edge of the parking lot on the northeast side of
the site. It is unclear what type/configuration of cub/gutter is being used. As presented, it appears two
layers of curbing is to be installed. Label all noncontiguous sections of curbing. Recommended use
of CG-6 curb and gutter wherever stormwater is to be conveyed.
9. [Sheet 2.0] Storm sewer system (inlet/pipe) are not presented in final site condition. Please correct.
Ensure final site conditions are correctly represented on all appropriate sheets.
10. [Sheet 2.0] Dumpster pad as presented does not match detail on sheet 2.1. Please correct
11. [Sheet 2.0] Parking width/depth/isle width are to be 9xl8x24' or 1Oxl8x20' (where a 2' grass
overhang is provided, spaced can be 16' deep). It appears that some isle widths do not meet the
minimum width. Please correct/clarify.
12. [Sheet 4.0] Paving plan does not accurately match demolition plan or proposed site conditions.
Please update plan or legend.
AC SA —Alex Morfison
I have reviewed the above referenced Minor Site Plan Amendment and would like to provide the
following comments:
1. CO.0: Update the water and sewer utility provider to the Albemarle County Service Authority.
2. CO. 1: Add the ACSA Water and Sewer General Conditions.
3. C2.0: Provide fixture counts for the proposed revisions so the water meter size can be verified. If
an upgrade to a 1" is required it will need to be shown on the plans and the AC SA will need to
quote connection fees for budget purposes.
4. C2.0: Call -out RPZ backflow protection on the domestic water service.
5. C3.0: Show ACSA utilities so any conflicts can be identified and commented on.

Staff has provided references to provisions of Chapter 18 of the Code of the County of Albemarle
The Code is kept up to date by the County Attorney's office. The Code may found on the County
Attorney's website which may be found under "Departments and Services" at Albemarle.org.
In accord with the provisions of Section 32.4.3.5 of Chapter 18 of the Code if the developer fails to
submit a revised final site plan to address all of the requirements within six (6) months after the date
of this letter the application shall be deemed to have been voluntarily withdrawn by the developer. If

Dear Mr. Perez:
The Department of Transportation, Charlottesville Residency Transportation and Land Use
Section, has reviewed the above referenced plans as submitted by Line + Grade Civil
Engineering, dated October 9, 2017, and offers the following comments.
Land use
1. Please provide a right turn lane and left turn lane warrant analysis as shown in VDOT's
Road Design Manual appendix F-96.
2. Please provide trip generation data.
Please provide a copy of the revised plan along with a comment response letter. If further
information is desired, please contact Willis C. Bedsaul at 434-422-9866.
A VDOT Land Use Permit will be required prior to any work within the right-of-way. The
owner/developer must contact the VDOT Charlottesville Residency Land Use Section at (434)
422-9399 for information pertaining to this process.
