On October 4, 2017, the County of'Albemarle requested the Virginia Department of Health (via the Albern arlc
County Health Department) review the proposed subdivision plat identified above. This letter is to inform you
that tlac aabove referenced subdivision plat is approved for Individual inosite Sewage Systems in
aaecord aaace With the provisions of the Code, of Virginia, tlieVervage handling and Disposal Reglrlcrtaons,
and local ordinances.
This request for subdivision, review was submitted pursuant to the provisions of § 32.1-163.5 of the Code q
Virginia which requires the Heahh Department to accept private sail evaluaations and designs friar an
Authorized Onsite Sail favaluator (AOSE ) or as Professional I-ingincer working in consultation with anOSf for
residential development, `his subdivision vv�aas ccrlified as being in compliance with the Board ofHeaalth's

issued in reliance upon that certification.

The Department of Transportation, Charlottesville Residency Transportation and Land Use
Section, has reviewed the above referenced as submitted by, Dominion Engineering dated
June 15, 2017, revised September 11, 2017 and find it to be generally acceptable.
A VDOT Land Use Permit will be required prior to any work within the right-of-way. The
owner/developer must contact the Charlottesville Residency Transportation and Land Use
Section at (434) 422-9399 for information pertaining to this process. If further information is
