RE: Cascadia—Blocks 4-7(WPO 2013-00032)—Interim VSMP Plan
Dear Glenn,
Please find attached two copies of the Interim Plan to address your concerns summarized in your letter dated
May 7, 2015 regarding a Notice to Comply for the Cascadia WPO 2013-00032. We have been working with the
contractor to verify their construction schedule and have performed additional survey work to verify grades,
basin sizes, etc. and offer the following responses:
1) Sediment is being diverted to the existing pond, and it has not been fitted for sediment control. This
needs to be converted, and the dam and outlet works cleaned up so they can be inspected completely.
RESPONSE: The conversion of the existing Fontana pond has been completed!
2) Sediment Basin 2 was started late, it has not been installed and excavation work has hit rock. The
basin has proven too large to install in a reasonable amount of time, and the entire area draining to it
should not have been disturbed. Provide an interim plan to implement.
RESPONSE: The contractor has installed the Sediment Basin 2 outlet works and has begun installing
the dam. Due to an initial 28-day stop work order,the large size of the basin,the depth of excavation
(up to 30' in spots),and the encountering of rock, the contractor is behind schedule on the pond. In
the past six weeks,the contractor has coordinated an interim plan verbally with Jeremy Glode. This
plan indicates the pond to be constructed in three separate phases as shown on the attached marked-
up pond plan. The contractor is working closely with the geotechnical engineer to ensure each phase of
the dam construction is properly.
3) Sediment Basin 4 has not been installed. There is a haul road to SB-2 in this location. Install the basin
and reroute the haul road.
RESPONSE: The contractor has placed a stockpile in the excavated area for SB-4. This was completed
since he could not place excavated material in the area of the entry road as planned since the Route 20
pond construction was delayed. We have updated the plan to include as-built topography of the
stockpile area and proposed grading for a temporary gravel outlet sediment trap which will remain in
place until the contractor can move the stockpile and construct the final grades for SB-4. First,the
contractor will install a rip-rap ditch from the old sediment trap adjacent to the Broadus parking lot up to
the top of the slope(removing the existing temporary slope drain). At the top of the slope,the
contractor will install a 3'-high gravel outlet. A temporary 3'-high diversion dike will be constructed at
the edge of the haul road to direct water towards the gravel outlet. On the northeast side of the gravel
outlet,the contractor will construct the diversion dike as shown on the approved plans. There will some
minor excavation at the gravel outlet to provide additional wet storage in proportion to the drainage
area. Please note that since not all the upstream area has been cleared and there are existing brush

barriers in place that are directing upstream runoff to the Route 20 pond,the entire 15-acre maximum
drainage area to SB-4 will not be realized until the stockpile is moved and SB-4 is constructed.

4) Sediment Basin 3 has been installed incorrectly. The side slopes are too steep, the riser crooked and
too short, there is no base, the dewatering device is placed incorrectly, the floor of the basin is incorrect,
the barrel is too steep, the stream bank has been disturbed, and the outlet placed too low. The spillway
is not dimensioned correctly, and fabric not placed correctly. There is nothing about this that follows
plan. Correct to follow plans or revise plans with interim plan.
RESPONSES:
1. RISER: The contractor has repaired the riser to be per plan.
2. SPILLWAY: The spillway has been lowered to provide adequate freeboard. The rip-rap spillway
has been installed with filter fabric underlay and appears to be acceptable.
3. VOLUME: As shown on the interim plan,a portion of the area draining per plan to SB-3 is to not
be cleared and is to continue to bypass SB-3. As a result, approximately 2.7 acres of area drains
to SB-3. Currently, SB-3 has adequate storage volume to treat this lesser area. The area that is
shown to be uncleared may be cleared after storm sewer is extended to the top of the hill and
the upstream runoff can be conveyed to the storm sewer system which drains to the Route 20
pond. Until such time as the storm is extended,the area as shown on the plans is to NOT be
cleared. A note has been added to this effect.
4. OUTLET PIPE:The outlet pipe was installed at a downstream invert of 441.91,while the creek
invert is at 442.38. Although the invert is lower than the plan invert elevation of 443.10, I see no
adverse impact on the sediment basin, since the wet storage elevation of the basin is 445.50,
and will always have a head of two to three feet above the creek. In any event,the contractor will
clean up the outfall area and provide rip-rap stabilization.
5. SCHEDULE: SB-3 is complete.
5) Sediment trap 3 is installed incorrectly. Sides are too steep, dimensions are incorrect, spillway is
incorrect, etc. The fill and pad placed upslope of the trap is not per plans and must be removed. Correct
the trap to follow plans.
RESPONSE: We as-builted ST-3 on May 12, 2015 to verify the trap was built per plan. We did find that a
few corrections that needed to be made. The contractor is working to correct the side slopes on the
upstream dam face to be 2:1 max and correct the spillway. The excavated slope will continue to be at
1:1 as per the approved plan,and as allowed in the Virginia Erosion and Sediment Control Handbook.
The contractor will also be hydroseeding the 1:1 slope to obtain grass growth. We would anticipate the
1:1 slope to be stabilized soon after hydroseeding takes place. At our site meeting on May 21,2015,
Jeremy had asked about the dimension of the bottom of the trap. The bottom of the trap scales to 70'
on the plan. the as-built length is measured as 67'. In my opinion,the trap bottom is in accordance with
the approved plans.
6) Two interim sediment basins/traps and stone weirs have been installed in place of SB-2 which are not
adequate to contain the flow. These need to be engineered per code and redone with the interim plan.

RESPONSE: The two interim sediment basins were a temporary measure. See response to#2 above.
7) The entrance was rack is set up incorrectly, with a garden hose. The plans call for a 1"tap and 1.5"
hose. Install per plan.
RESPONSE: A proper water connection has been provided.
